INTRODUCTION
The given assignment will focus upon a case study of Mr. Martin Chen. He is 79 years of old
widower who currently lives with his youngest daughter and her husband after his wife’s death.
Recently he was admitted to hospital for colonoscopy due to PR bleeding. When complete check
was done it was identified that Mr. Chen showed some haemorrhoids that were banned to be
used during procedure. So as a result, instead of being discharged after hospitalization he was
admitted for further five days. Due to his extremely high BP, high blood glucose level. Due to
this uncontrolled BGL he was required to be provided with insulin. Mr. hen was getting slowly
aggressive and as a result a code black emergency was declared by nurse this is because he was
trying to go home by leaving ward. He had past medical history of Hypercholesterolaemia,
Hypertension, Trans ischaemic attack 2018, and T2 diabetes mellitus. He is allergic to NSAIDs,
Sulphonamides and Onion. This paper will focus upon care need of Mr. Chen, along with this,
the benefits of clients remaining at home will also be analyzed. Further, the benefits of client
transitioning to aged care facility will also be analyzed and identified. Moreover, barriers to
patient care will be known for the factors which will be within the home care facility and the
aged care facility.
MAIN BODY
Two care needs identified
The two care needs which Mr. Chen needs are medical needs and psychological needs.
Balanced diet and nutritional needs: This is the first thing which is to be taken concern of. It is
one of the most important need to be fulfilled by Mr. Chen as his blood pressure and diabetes
were increasing in uncontrolled manner. As he was suffering from diabetes so it was important to
provide balanced diet so that all the main nutritional requirement can be fulfilled and further his
diabetes can be controlled. Focusing upon fulfilment of nutritional needs through balanced diet
can help in controlling excessively increasing hypertension and blood pressure by reducing salt
intake in diet.
Psychological needs: mental and emotional wellbeing of Me Chen is one of the most important
need to be focused upon. This need can be identified by observing his aggressive behavior while
he was at the hospital. As he is a strong Buddhist believer, so when care is provided it is

extremely important to ensure that his social, psychological and emotional needs are fulfilled so
that his aggressive behaviour can be controlled. Along with this, special care from the family
members should be taken into consideration so that he gets well easily, and this improves his
health, and he is also able to live his life in the manner which he wishes for.
Benefits of Client Remaining at Home
There are various benefits of Mr. Chen remaining at home as the family will be able to take care
of him in his tough times. The family, when in this period of life, will be able to take care of him
and will help in fulfilling his requirements and needs and will be able to help him in doing the
work of their own choice that is Mr. Chen will have his own independent life apart from being
the patient within hospitall. This is because Mr Chen is scared of leaving his loved ones and was
not at all ready to atay at hospital. Providing all kinds of care, balanced diet at home would
direclty help in fulfilment of his psychological needs in an approprite manner (Kruger & et.al.,
2018). The major thing which is being observed is that at home there is special care taken off by
the patient such as Mr. Chen will also be taken care of during his health and during this problem
which he is facing at the age of 79. Remaining at home would further benefit Mr Chen in
remaining healthy and bring improvement within their overall health because by living near
family members can help in fulfilment of psychological needs. The level of safety is also
increased at home as the family takes care of the person and in this case Mr. Chen if provided the
care facility at home by the family members will be in safe hands and being the parent of Chua,
she knows how to take care of her father therefore Mr. Chen will be in safe hands. Not only this
as she is well aware of his nature and guidelines in terms of diet provided by nutritionist she will
be able to fulfil nutritionist requirement of Mr Chen in much better manner. Provision of care at
home can further help in effectively fulfilment of requirement of nutritional needs and balanced
diet needs. This will also help Mr. Chen to recover fast from the problem and the health issue
which he is facing from. Further remaining at home can further help in ensuring that he is being
provided with appropriate diet and all the main nutritional requirements are being fulfilled in an
appropriate manner.
The other benefits of remaining at home are that Mr. Chen will also be comfortable in making
conversations with the people and his family members of what his requirements are (Lee & et.al.,
2017). Also, staying and taking care of the patient at home will also help Mr. Chen in
