Question One
Interprofessional collaboration refers to the practice that involves coming together with a
team of healthcare specialists to deliver quality patient care. The practice is understood to reduce
medical errors in that it improves clinical communication between pharmacists, nurses and other
healthcare providers thus reducing medical errors such as overdosage and wrong prescription
(Verspuy & Van Bogaert, 2018). Also, interprofessional collaboration is an important practice in
the provision of quality care in that it encourages team-based and patient-centered care that
include primary doctor, nurse, and any other relevant team members who foster patient-centered
care thus quality care as the team members are able to consult each other and come up with the
best treatment option for the patient. Besides, practice fosters team mentality and promotes
comprehensive patient care thus increasing the safety of the patient (Reeves, 2016).
The current trend and issue that is facing health care is the spread of coronavirus.
Coronavirus disease is a respiratory virus that has shown to spread via droplets from an infected
person during coughing, sneezing or speaking. The disease has shown the ability to spread very
first and many people throughout the world have been infected by this deadly virus disease. The
disease has no cure or vaccine and has killed thousands of people thus becoming a pandemic. In
order to effectively manage the disease, there is a need for more interprofessional collaboration
within the healthcare system.
Question two
Patients-Centered Medical Home (PCMH) –It can be described as a model of care where
patients are involved in a direct relationship with a selected primary care physician. The chosen
care provider’s main functions are to manage a cooperative team of healthcare specialists and
