Introduction
• Health & social care refers to a term that related to services which are provided
by care providers in order to fulfil actual health needs of people.
• It include different types of activities and procedures which are required to be
carried out in order to make an individual well-being.

The Approaches And Interventions
• There are some approaches and interventions to support individuals with their
specific needs such as:
• Self-help: It is a most important part of daily life. An individual take care of their
own for being healthy, and it also include family, children and friends. But
Graves was not able to help his own. For example, of a person suffering from
dementia then family members should help patient to conduct daily routine work
like walking, taking bath, eating and intake of medicine on time for improving
their condition.

• Partnerships: Everybody needs someone to take care of. A person will depend
on the other for certain help.
• Evidence-based practice: Disable people want to intensify their capabilities, by
choosing treatments.
• Advocacy: It refers to speak for the someone. People who are learning
disabilities need more support and care to get their wishes.

• Person-centred care: It helps to develop skills, knowledge and confidence to be
more effective and so they can manage things effectively.
• Positive Behaviour Support: Adult social care takers should have right
knowledge and skills to use this approach.
• Legislation – Mental Capacity Act 2005: This act is for those people who can
not take stand for themselves and make decisions.

The Effectiveness Of Intervention Strategies
• Physical: People such as children, women, older person and physically disable
person have their own specific needs.
• Social: Activeness of health care workers and social workers can reduce
problems and condition of Graves with specific needs. And improvement and
potential development can also be seen.

Potential Impact Of Emerging Developments
• Potential tensions: Protection is very important for those who are disable and
are not able to move properly, help can be provide to them.
• Emerging developments: Government can improve outcomes related to health
of people who are disabled by improving affordable healthcare services, access to
quality and make best use of the available resources.

Conclusion
From the above report, it has been analysed that health and social care consist overall
activities and procedures for providing care to an individual in order to fulfil relayed
specific needs. It includes to follow various social policies and legislation including
Human Rights Act 1998, Disability Discrimination Act 2005, Mental Health Act
2007 and Health & Social Care Act 2012.
