INTRODUCTIONIn health and care home, there are several things require maintain quality, standard, socialcare, etc. It assists to implement appropriate caring and development of each person who takeappropriate services under care home (Hassink, Grin and Hulsink, 2018). Present study based on case of Liverpool Home care where on the basis of ethnicbackground proper caring implemented for 65 years old to 98 years old. After inspection of CareQuality Commission, it has been seen that commissioner highlighted special measure to makechanges in care home. Therefore, report cover implementation to make changes that are required to considerplan CQC within next three months. It includes factors that drive change in health and social careservices that are applied to present case scenario. Furthermore, this report discusses aboutstrategy and criteria for measuring recent changes in health and social care. At last, reportexplains about key principles of change management with using Kotter's eight steps. LO 1Q1. Key factors that drive change in health and social care services that applied in present casescenario (P1.1)In order to determines key factors that drive changes in health and social care services inpresent case of Liverpool, following elements determines:Health act 1999: With respect to drive change in health and social care services forpresent case of Liverpool, Health Act 1999 applied. This act aimed to improve qualityof care provided to patients and determines provision which intended to improvecoordination of care. In this context, different changes can be applied as per futuredate and changes that are core for present care (Al-Shorbaji, Borycki and Winter,2017). Minority ethnic community needs: Many cares of Liverpool determines differenthealth social services. Therefore, it is difficult to deal with minority which iscomplicated. Home cares requires understanding different languages to deal withdifferent background. It is become easier to get interpreters and support workers.1

Cares also work with appropriate system as well as their right options (Al-Shorbaji,Borycki and Winter, 2017). Lifestyle factors: Another element which create major impact and biggest challengein front of Liverpool is lifestyle factor. It needs to be successfully understand by eachcarer of home care. This is because, for proper caring they must know about theirpatient lifestyle (Jewkes, Flood and Lang, 2015). NHS and Community Care Act 1990: Under this act, National Health Care Serviceand Community Care determines any Adult those aged over 18 who eligible forrequires services from local authority who has right to full assessment of their needsand services which provided by Liverpool. Individual also tailored to meet assessedneeds to ascertained appropriateHealth and social care Act 2001: According to this act, in local areas care trust canbe established voluntarily. Hence, in Liverpool it is important to make arrangementsto work jointly.
