INTRODUCTION
Health and Social care covers all the organisations, procedures and practices that are
necessary to support an individual’s journey and provide them with better healthcare and social
support (Jack and et. al., 2016). Moreover, it is crucial to determine the role of this aspect within
an individual’s journey to acquire a detailed insight on its implementation within a person’s life.
Thus, in regards to this, the report below outlines and evaluating local community resources and
provision supporting integrated care planning. Along with this, the report also undertakes
description of local unmet needs related to healthcare provision within local area. Furthermore,
explanation of difference between health and social care providers is also very much provided
within the report.

Integrated care refers to coordination of both health, as well as social care services in
context of fulfilling individuals’ needs that require medical or social assistance. Majorly, it helps
in dealing with complex health related problems in a seemingly effective manner. Hence, in
relation to the local area, below are several community resources and provisions supporting
integrated care planning: Finance Associations:
One of the most efficient local community resources for integrated care planning is the
financial associations. These organisations are responsible for providing necessary funding to
individuals, particularly old age people to get appropriate medical treatment of them (Cruess and
et. al., 2014). Moreover, they work closely with different healthcare institutions forming a care
network for healthcare planning.
Health and Social Care Homes:
Another prominent local community resource is health and social care homes. There are
times when individuals have no familiar individuals to take effective care and ensure full medical
assistance. Thus, in this case,, these institutions provide necessary treatment and shelter to such
individuals and work together for providing integrated care to people in need and operate as a
collective unit with shared financial, technical and human resources within the community.
