INTRODUCTION
Health and Social care covers all the organisations, procedures and practices that are
necessary to support an individual’s journey and provide them with better healthcare and social
support (Jack and et. al., 2016). Moreover, it is crucial to determine the role of this aspect within
an individual’s journey to acquire a detailed insight on its implementation within a person’s life.
Thus, in regards to this, the report below outlines and evaluating local community resources and
provision supporting integrated care planning. Along with this, the report also undertakes
description of local unmet needs related to healthcare provision within local area. Furthermore,
explanation of difference between health and social care providers is also very much provided
within the report.

P1. Outlining local resources and provision supporting integrated care working
Integrated care refers to coordination of both health, as well as social care services in
context of fulfilling individuals’ needs that require medical or social assistance. Majorly, it helps
in dealing with complex health related problems in a seemingly effective manner. Hence, in
relation to the local area, below are several community resources and provisions supporting
integrated care planning: Finance Associations:
One of the most efficient local community resources for integrated care planning is the
financial associations. These organisations are responsible for providing necessary funding to
individuals, particularly old age people to get appropriate medical treatment of them (Cruess and
et. al., 2014). Moreover, they work closely with different healthcare institutions forming a care
network for healthcare planning.
 Health and Social Care Homes:
Another prominent local community resource is health and social care homes. There are
times when individuals have no familiar individuals to take effective care and ensure full medical
assistance. Thus, in this case,, these institutions provide necessary treatment and shelter to such
individuals and work together for providing integrated care to people in need and operate as a
collective unit with shared financial, technical and human resources within the community.
 Regional Care Initiatives:

There are several voluntary organisations as well within the local community which work
towards supporting the needy in terms of health and social care. They maintain regional records
and provide social support, fulfil food requirements and arrange funding if medical assistance is
required by these individuals.
P2. Description of current local unmet needs related to health, care and support services
There are times when despite several resources and appropriate care services, certain local
needs of people in terms of health, care and support activities remain unfulfilled. Moreover,
these unmet needs sometimes create major issues in a person’s health and hence, are necessary to
be identified. Therefore, below is a description of certain current local unmet needs that are
related to health, care and support services: Emergency Services:
One of the biggest needs that is unmet within the local area is the emergency services. The
locality has a range of different individuals who belong to different age ranges and having
mental and physical illnesses. However, there is a gap within the desired and actual emergency
services which could tend to their urgent needs appropriately. Furthermore, there is a lack of
medical staff and professional nearby as well, which makes it quite complex to tend to major
issues within the local area.
 Transportation:
Another prominent unmet need of individuals within the local area is related to
transportation. The standard of living within the area is not that prominent that each individual
own a private vehicle. Hence, in case of emergencies, ambulances and other transportation
assistance is sought by the people living in the locality. Hence, this refrains the individuals to
reach and acquire immediate medical assistance as and when required. Furthermore, there are
times when individuals cannot go for their appointments with GP as they lack necessary
transportation services within the area for this purpose (Volkan, 2014).
 Equipments:
This unmet need is related to the necessary and imperative medical equipment availability
within homes to deal with crucial health related problems and handling the same till the medical
emergency team arrives. There is a serious lack in the level of equipments and other necessary
resources and healthcare kit within the community and hence, it causes a lot of problems when
an emergency life situation arises.

Health and social care are two very different parameters that contribute one another to
provide support to individuals needing medical or social assistance. However, it is necessary to
gather necessary knowledge about the differences between the providers of both these elements
in order to understand their nature and mode of working. Therefore, these pointers of differences
are explored below:

Other than the differences between professionals of both these fields, it is crucial that types
of interagency care provision in relation to meeting the needs of individuals are also identified
(McAlearney and et. al., 2015). Hence, below is a description of the same in regards to health
and social care services: Information Management:
One of the most prominent interagency cares which is provided is related to health
information management. NHS partners up with several other social care professionals in order
to ensure that a full medical history of the patient is present within their records.
 Funding:
Another prominent interagency care that is affiliated towards better services to individuals is
related associated with funding. NHS has partnered up with a range of different organisations
which are responsible towards providing better and integrated care to the patients. Basically, the
idea is associated with regulate funding for communities and people who are unable to support
themselves.
D1. Evaluation of local resources and provision in terms of meeting needs of an identified
individual
It is crucial that the local resources are evaluated in relation to meeting needs of specific
identified individuals within the local area in order to determine their effectiveness in achieving
this target. Hence, below is an evaluation of all these resources.
 Finance Associations:
For an old individual, who might be dependent on others for their daily financial and medical
needs, this resource could be very useful if it is channelized in an appropriate manner. The
reason for this is that what restricts individuals from getting a sound medical treatment for
themselves is related to lack of funding and hence, financial associations could ensure that these
individuals get necessary medical treatments covered.
 Health and Social Care Homes:
These are one of the most effective resources in terms of meeting needs of individuals. For a
specific individual, say an old person with a range of medical issues, it becomes necessary that
proper attention is provided and directed towards this individual on daily basis to fulfil the
necessary medical requirement (Glasby and Dickinson, 2014). Hence, health and social care

homes could effectively tend to this aspect and could ensure that such individual spends his days
with proper diet, medical assistance and social support, which makes this resource very effective.
 Regional Care Initiatives:
The scope of these initiatives is very limited, as they cover a very small geographical area,
particularly a small community. However, in terms of assistant and service, they tend to the food
and other social requirements of individuals, such as hygiene, proper clothing, social interactions
and so forth. In terms of specific identified individual, they would be ensuring that the old man
could get proper nursing and assistant to perform the daily tasks such as fetching groceries or
preparing food items in an appropriate manner. Moreover, they could also tend to their social
support by enhancing their community engagement and supporting him through medical
treatments (Billings and De Weger, 2015).
CONCLUSION
Thus, it is concluded from the report above that health and social care is very important
to support individual’s journey and effective support throughout. It is necessary to outline, as
well as evaluate local care provisions to determine the support which individuals acquire from
these services. Moreover, current local unmet needs must also be analysed in detail to identify
the limited scope of healthcare in local area. Lastly, it is crucial to explain the factors that
differentiate a healthcare and social care workers, along with types of interagency care provision
available for needs of individuals
