INTRODUCTION Person Centred Practise refers to the services which are provided to an individual who needs special care and treatment so as to recover from the illness from which he/she is suffering from. The company chosen for the completion of this assignment is NHS which was founded in the year 1948 and is headquartered in London. It provides different types of health related services to their service users. This assignment will cover holistic approach to person centred practise along with current policies, legislations, and regulations in relation to effective person centred practise (Edvardsson,2015). Lastly, the assignment will reflect on own practise within health and social care along with the ways of developing own skills and behaviours in relation to health and social care, support provision. PART 3 A. Develop a reflective log book and provide a written account using two or more scenarios from your work place on how you have provided holistic person-centred care. The person-centred care is about considering the needs of people and providing them proper health & social care services. It all about the professionals care to the people and managing their health as the different disease posses different types of care and services. As for professionals, theGDPR (General Data Protection Regulation) 2018has been formed and it was designed to protect the data and personal information of patients. It applies across the country in each organisation and it also includes the transfer of data into safe hands. There are many organisation run over GDPR policies (Ward, 2018). There are some other policies which also applied in the UK. The Care Act, 2014-It comprises of laws and rules which are applied within the health care organisations and provide that the primary responsibility of such organisations is to promote well-being of individuals seeking treatment. Many amendments have been made in this act for makingit effectivewith theapplicationwithinthe organisation.NHSshouldavoid any negligence from its part. NHS policies for patient centric-There is a policy implemented within NHS which focuses on development of good practice on patient centred care planning, to take the consent of patients and work in an integrated manner by producing personalised services for each one of

Scenario 1 When working with NHS, my aim is to provide the person-centred care to my patients according to their difficulties and different conditions and their needs. In patient-centred care, to providing effective treatment by using this care allows me to support the patient in recovering soon as the treatment are providing in rightful manner. When I was working with NHS as a health practitioner, I have maintain and developed a person-centred plan and GDPR legislation for one of the patients. The patient was a smoking addict and want to quit smoking as he was aware of bad influences of cigarette which could be appearing on his health (Sharp, McAllister and Broadbent, 2016). It can be said that while providing this kind of care it was quite easier for me as they are person (patient) provides equivalent contribution in the growth of the person in rightful manner. In his treatment, I have communicated with the patient on the regular basis and assure him that his data would be confidential according to GDPR legislation. During treatment, I try to developed his knowledge on the impact of smoking on health. This influences the interest of patient towards the treatment. My treatment includes schedules, medication and treatment to cure his addiction and the result comes positive as the patient recovers soon. Scenario 2 The other example are also taken from my own experience, when I was working in NHS. The patient that is related to person-centred care is for an alcoholic patient. The patient was working in a well known multinational company and he was suffering from alcoholism due to which he was no able to do his regular activities. The range of addict to alcohol of the patient is that they can't even wait for the finishing the working hours just to drink alcohol. This impacted on his professional life as well as on personal life (Ross, Tod and Clarke, 2015). By listening the whole scenario, that the patient is already known the negative influences of alcoholism. I have personally communicated with the patient & let him know about the use of GDPR legislation and provide him personalised care and support. As a health practitioner, I have formulatedan effective care treatment to the patient. In my treatment, I suggest the patient toget admitted in 3
rehabilitation centre of NHS where from the effective environment that support him to leave the alcohol. As a result, the patient has recovered from his alcoholism from with in a one month. In order to provide treatment to this patient, I have developed an effective person centred care treatment to this alcoholic patient. The main reason behind this approach of treating patient is to help the patient with extra support and care which will ultimately support them in leaving this bad habit as it is spoiling their career negatively. I have recommended this patient to get admitted in rehabilitation centre of NHS where he will get an effective environment that will motivate him to leave alcohol in complete manner. This patient has respected my views and within three days he came for registration in NHS rehabilitation centre himself. This patient have also collaboratively supported in the improvement their own health condition (Kelly and et. al., 2018). As a result, within one month of enrolment of this patient he was completely recovered and was no longer addicted to alcohol. These two cases were related to addiction but they different scenario from one another. In first case I provide medication based treatment and in second case I have considered the patient to get admitted in rehabilitation centre of NHS. B. Use the developmental plan to show evidence of develop your own skills, understanding and practice According to a particular assignment to developmy own knowledge on my personal skills and capabilities I have developed my personal SWOT analysis which is conducted as below in effective manner: Strengths- As I am care practitioners, this including as my strength because I am open minded, talk politely and give respect to them, listening carefully and apply this, interact with patients, good non-verbal communication because my body language, hand gestures, eye contact, and tone of voice all are message trying to convey in good manner with patients and colleagues in my professional life. Effectiveness in this skill has supported me in motivating patients to take treatment and follow my allow advises without any sort of issue. 4
I am very passionate about my work, it is consider as my strengths because I can easily understand the impact, in keep integrity, I don't afraid to take risks of any of patients issues easily handle them. Along with this team working is also seen as the my midpoint weak skill because it takes more time to work, sometime I clashes with some other personality person, some time blame and responsibility issues are happen in hospitals, leaderships issues. As well as some people reduced my innovative ideas. I focuses too much on the well-being of team and don't bring their creative ideas to the resulting in less innovation and deficiency of new ideas. Problem solving consist the fact that diagnosing, illness and identifying the causes of problems in hospitals, I am unable to solve patients problem in better way. Lack of confidence is a problem from which I am suffering frombecause I feel uncomfortable to talk with other people and I feel shy with staff member to talk with them. My presentation skills are not much effective as I am weak to convey information to audience in a proper manner. I have ineffective decision making skill because of confusion and need more time for the same. I am not capable to make quick decisions which is very important in terms of patient centred care.
