This report discusses the different models of assessment and their implementation in healthcare, the application of theoretical perspectives to the care planning process, and the responsibilities and duties of own role in promoting person-centred care planning. It also explores the barriers to implementing care planning and the benefits of using care plans in ensuring individual needs are prioritised. Additionally, it discusses the ways in which the individual and family contribute to the evaluation of the care process and reviews the aspects of the care planning process and their impact on the individual, family, and carers.
