It is very essential for an health care practitioner to support individual in maintaining
their own identify while taking care services. But for this it is crucial for health care practitioner
to assess the own capacity to deliver this. Being a health care practitioner I am having experience
in catering the people with different care need using a person-centred approach where I require
to take up the risk by communicating with them using different communication strategy. Here
the risk lies is relatively higher as a miss-understanding within conversation may affect the
health of patient (Coughlan and Cronin, 2016). But I have handled many of cases where people
are having different care need which lead me in understanding and making use of different
communication strategies which allow me to cater those patient effectively.
D4 Evaluating own role and practice in facilitating empowering an individual
Personalised care and support planning is considered as the systematic way in order to
make sure the individuals and their living system considering the long term and also support
through proactive conversations to the healthcare professionals. Along with this, the
conversations need to focus on the matter regarding the individual goals and also support their

they can make use of verbal as well as non verbal communication in an integrated manner. In
this while communicating with patient one can make use of written information to make the
patient clear about the things that health care practitioner want to communicate (Cheers, 2019) .
Along with this he can make communicate while speaking along with the expression and hand
moments to make the things more clear to the patient who have different communication need.
Like, when a patient having hearing issue then at that time the practitioner can use expression,
body language, sign and symbol as a part of communication. The safe way to communicate with
the patient is to talk verbally where the patient can provide with information that he want in more
effective manner and while talking it is easier to ensure that patient consider things in right
manner so that not any kind of issues may arise in future. Other than this for disable individual
and patients that are not able to understand the verbal messages, health practitioner can make use
of body nad sign language in order to communicate with patient effectively and make them
understood each and every this properly.
