INTRODUCTION
Integrated care is referred as the services which are managed as well as delivered so that
individuals receive a continuum of disease prevention, health promotion, disease management,
diagnosis, treatment palliative care services and rehabilitation, coordinated across the distinct
levels affair beyond and within the health sector and as per their requirements throughout the
course of life (Albertson, Irving and Best, 2015). This report is based on Woodlands Total Care
Nursing Home which is a care home located in Redbridge borough of London. This care home
was established in year 1992 and is situated in Gordon Road, Ilford, London. It is one among the
multicultural care homes which is the specialised in dietary needs for Muslims, Afrocaribbeans,
Hindus, South Indians and Filipinos.
This report outlines local resources and provision which supports integrated care working
and discussion of the current and unmet need associated with care, health and support service
provision. This report also assesses the capacity of an individual to determine their own needs
and the role of health care or support service practitioner in supporting person centred care.
Apart from this, the report describes the impact of own relationship with multidisciplinary team
members and individual included in delivery of care pathway and to promote effective
interprofessional and multi-disciplinary team working (Canhoto and Arp, 2017). At last,
different communication methods utilised to render appropriate support to distinct individuals to
meet their needs and application of appropriate communication strategies in determining and
responding to requirements of different service users in health care and support services are
discussed in the report.

P1 Outline local resources and provision that supports integrated care working
Integrated care is referred as a care which is planned with individual who work collectively
in order to understand the service user as well as their carer, put them in coordination and control
and deliver services to accomplish best outcomes. The individuals who are with multiple
comorbidities as well as complex needs, requires distinct type of professionals, who has wide
range of skills and are not tied through traditional boundaries. High quality care is very essential
for the elderly patients who are suffering from multiple pathologies. In order to work effectively
