Introduction
Health and social care sector is concerned with delivering best services
to the individuals who are having some specific needs. This
assignment will focus on role of different professions to support an
individual and relationships & partnership working within
multidisciplinary teams. The different communication methods and
a balanced risk taking approach is given below.

Appropriate leadership within the remit of
own role in a health
Considering effective multidisciplinary ream working, it has been
analysed that appropriate leadership framework is required to be
adopted for completing specific targets properly.
It is necessary for care professionals in my team to utilise
transformational leadership style for improving their entire
performance in terms of making Ms M. well being.

Responsibilities of information sharing
between multi disciplinary teams
The sharing of data is very critical aspects in healthcare because desired
data must be provided to authorised person accurately and it should
remain private as well. It is necessary to make records of every dose
of medication and clinical activity on daily basis about which
desired information is require to be shared among inter professionals
working together on same case of patient.

Competent and autonomous leadership
Competent leadership refers to perform in every situation by remaining
self motivated in order to fulfil specific criterion to gain desired
outcomes. It includes the fact to perform effectively in different
situations whether is good or bad. However, it has been analysed
that competent leaders believe to perform well in each
circumstances and put efforts to make patient well being.

Own personal growth and development
The personal growth and development is required to make people learn
about conducting various clinical practices by considered the patient
centred care in order to achieving maximum outcomes. I have
evaluated that several factors should be focussed by professionals
such as self knowledge, comparison, modification, acceptance,
planning, reaching desired potential and self growth which facilitate
to enhance self skills and abilities for improving individual
performance in multidisciplinary.

Communication strategies
Communication plays an important role to establish mutual understand
with patient which facilitate to determine their actual needs and
desires regarding healthcare services. It is favourable to make an
individual comfortable with care professionals so that they can share
very problem without any hesitation that is suitable for care
practitioners in terms of making appropriate decision to providing
correct medications for their well-being.

Safe and clinically effective practice
In context of Ms M., it has been analysed
that effective clinical practices should
be conduct and apply safe activities or
providing secure services to her. My
team members are required to focus on
patient centred acre and involved them
to understand their actual needs and
desires which facilitate to make
appropriate decision of treatment
methods for their well-being.

Own capacity for positive and person-centred
risk taking
The medical field is itself a risk taking area in which effective clinical
practices are required to be conduct in order fulfil needs of an
individual. It is necessary to for professionals to considered four
aspects such as mental capacity, promoting independence, least
restrictive practice and emergency care plans which is favourable to
enhance ability of risk taking to maintain identity of selected patient.

to gain perfection on work. It is
necessary to check on attitude,
remain reflective, assess own
performance regularly, maintain
records of working, determine
expectations of supervisor, get
feedback from peers and so on.

 Effective relation among partners is required for working with
mutual understanding within multi-disciplinary care practices.
 It is necessary for care professionals to conduct efficient
communication with teal tos hare desired information about
patient which is helpful to make correct decision for well being an
individual.

Advantages and disadvantages of information
sharing
 Sharing of information about patient is vert important among
medical professionals in order to reduce confusion and make
appropriate decision making regarding treatment of them.
 It is essential to convey accurate information related to sick
individual very carefully because a minor mistake in sharing
information may create severe complication for them.

Analysis of own role in partnership working
Partnership requires cooperation of every individual ho is a part of a
team in order to execute specific tasks in proper way.
I prefer to communicate every aspect related to patient in team and take
suggestions about my decision regarding treatment to provide better
services to them.

Balancing individuals rights and choices with
duty of care
 Being a care professional, I have a responsibility to focus on duty
of care by following care standards by taking care of patient rights
to provide appropriate treatment for people.
 It is necessary for me to consider ethical principles including
justice, non maleficence, beneficence, respect for autonomy and
informed consent.

Different communication methods
The process of transmission of information from one individual to other
is referred to communication. This is a process of sharing
opinions, ideas, emotions, facts or messages to other people.

Analyse balanced approach to positive risk
taking
 The balanced approach to positive risk taking to meet ongoing
care needs simply means to balancing the freedom of choice and
duty of care.
 It is important to balance risk management as it facilitate to
promote the aspect of person centred care which is suitable for
health of people.
 In context of care scenario about Ms. M, out multi-disciplinary
team used to make her independent and formulate an emergency
care plan to deal with immediate complexities.

Evaluate own role in facilitating and
empowering the individual
 The communication between service user and care practitioner is
very important because pathological test are not capable to provide
information about how an individual feel internally.
 It is necessary for professionals to make an effective conversation
by motivating them in a way so that they can share views about
their actual needs and desires.

Conclusion
From the above discussion it has been concluded that, health & social
care help the community through providing medical services as well
as resolve the social problems which affect the society. Healthcare
resources help the individual to get the treatment from the various
organization and make themselves better after getting their services.
