Introduction
Health and social care sector is concerned with delivering best services
to the individuals who are having some specific needs. This
assignment will focus on role of different professions to support an
individual and relationships & partnership working within
multidisciplinary teams. The different communication methods and
a balanced risk taking approach is given below.

Appropriate leadership within the remit of
own role in a health
Considering effective multidisciplinary ream working, it has been
analysed that appropriate leadership framework is required to be
adopted for completing specific targets properly.
It is necessary for care professionals in my team to utilise
transformational leadership style for improving their entire
performance in terms of making Ms M. well being.

Responsibilities of information sharing
between multi disciplinary teams
The sharing of data is very critical aspects in healthcare because desired
data must be provided to authorised person accurately and it should
remain private as well. It is necessary to make records of every dose
of medication and clinical activity on daily basis about which
desired information is require to be shared among inter professionals
working together on same case of patient.

Competent and autonomous leadership
Competent leadership refers to perform in every situation by remaining
self motivated in order to fulfil specific criterion to gain desired
outcomes. It includes the fact to perform effectively in different
situations whether is good or bad. However, it has been analysed
that competent leaders believe to perform well in each
circumstances and put efforts to make patient well being.
