INTRODUCTION
Manage health care practice to assure the positive outcomes for individual means that the
outcome based practice generate the actual effects, impacts and end results of interventions or
services on an individual's life. An outcome is the progress and result of care delivery. Outcome
based practices create a better results for the individual to improve the life of client. This report will
include the principles and theory that support outcome based practice, approaches and effects of
legislation and policy. This report will also highlight the various practice that promotes cultural,
social, spiritual, emotional and well being and include the uses of resources to boost good health
and healthy choices and appropriate method to fulfil the health needs of individual. This report will
also state the provisions that provide individual selection and control on the outcomes they desire to
achieve and value of effective working relationships with families and others for generate
optimistic outcomes.
1.1 practice of Outcome based
Outcome based practices means the progress and result of care delivery. The outcome based
practices generate true improvement in the lives of individuals (American Diabetes Association,
2018). A specific organisation includes different key events to produce the best outcomes for
individuals with specific procedures and conditions. Outcome based practices introduce effective
methods to achieve required individual care goals. It includes regularly quality betterment, outcome
and process measurement and combination of teamwork.

Outcome based care needs effective planning and operative with the individuals or service
users to helps them to achieve and identify the great things. Its major aim is to generate meaningful
outcomes and assisting individual to lead more achieving lives.
Outcome based care enhances the motivation, interest and develops the enthusiasm required
to support individual to create more achieving life. The approaches includes the result based
accountability, effective outcomes management, outcomes according to practice and logic model.

experiences or program activities. Quantitative analysis is appropriate when access the policy and
legislation outcomes and its impacts.

process for them. It is very necessary to committed with outcome based approach and align it in
