In the unit task 19 case I did the project assessment on reflective approaches in context to
the evaluation of the persona center care practices and situation faced or managed by the health
care professional in that. Along with the legal and ethical prospective of care and health care
provision, that how the professional manage all these aspects of the care.
Reflective approaches in health and social care is very important for the effective learning
of the health care skills and practices, along with this it helps in the improvement of health care
practices in context to patient. Professional can think about their previous experience in the similar
situation or case to make more better efforts and practices.
persona center care plan is the one of the most important practice or strategy of health care
provision which is also consider as the holistic care approach. This helps the health care
professional to provide effective health and personal care services to the patient by managing all
living circumstances which impact the health. In context to the continuous improvement of the
persona center care plan and practices that is important for the health care practitioners to reflect
on their previous experiences of persona center care plan and achievements. Within the
completion of this project I understand the responsibilities of health assistant , that Physical
support and personal care, support as the daily living needs, respecting different cultures and
values, confidentiality, equal opportunity, implementing the professional approaches, hygiene are
the important approaches of the holistic care. which can improve with the continuous learning and
development of the evidence based practice. the legislative factors of holistic are plan is
safeguarding, health and safety , risk management , equality and diversity. I developed my
communication skills which is very important to meet the nursing standard and code of conducts,
which helps to effectively communicate and satisfy the patient.
In the unit 20 activity research and study of care planing process and models in health
care practices is done. Such as the comparison of different models of the health care , evaluation
of the health care professional role in the promotion of person center care planing, application of
the care planing. Along with this analysis of the care planing barriers and benefits, role of the
patient and family in their care planing is done.
In the health care organization and setting planing is very essential activity which is

associated with the development of effective health care plan along with incorporation of relevant
health care practices and process according to the health care models and standards.
I did the brief study and evaluation of the health care planing models and standards, such as
different models of assessment are compared in context to there implementation in health care
plan. And it has been found that there are the two important model of health care such as the
social and medical model.
We had discussed on the application of theoretical perspective to the care planing process
in health care setting, such as there are the theoretical approaches like behavioral approach ,
Sociological approach. Along with the theoretical approaches there are the legislative and
regulatory frameworks which has to be implement in care planing. Because those frameworks aid
to support equality and diversity management, such as The care ACT 2014, The equality Act
2010.
In that case I worked on the discussion process of my own roles and responsibilities in
context to the promotion of person-centered care planning. And it has been analyzed that in
person-centered care planning responsibilities of the nurses and executives is to manage patient
needs and health condition at every stage. We have to advocate from the side of patient for their
choice and treatment. My role is to manage all essential information and action plans of the
patient along with other confidentiality documentation. Such as medical information and key
action point are managed by me in every patient care plan. My contribution to the care planing is
very essential because I work very close to the patient and due to that is can provide all essential
information of the patient to doctors, such as improvement and treatment needs. I also contribute
in the emotional support for the patient.
I analyzed that in context to the care planning there are different barriers such as the
unemployment and lack of the sufficient recourse or health care facilities, inequality and
discrimination is also the barrier of care planing and implementation. Effective care planing and
holistic care approach, contribute to the successful achievement and benefit the patient care plan ,
by helping in the management of familiar environment and all living circumstance. This also helps
in the patient satisfaction and organization growth with effective patient care.
Patient and family play very crucial role in the care planing and effective care plan goal
achievement. Because according to the nursing standards that is important to make informed
decision of treatment and if possible then their should be the choicer of patina and their family for
care practices and treatments. self management approaches and support of the family support the
achievement of care plan outcomes and in context to that is the role of health care profession to
educate the family and patient aspect of the care planing is involving promoting person-centered
