In the work place that is oil and chemical industries, it is quite natural to be exposed to the equipment and
chemical products which are hazardous for the health of the workers. Therefore, in this work place the
management of the organization must ensure the fact that all the workers must use the equipment properly and
before using that equipment, proper risk assessment of all the equipment must be performed by the
management (Sso.agc.gov.sg 2020).

 Precautions should be taken to avoid adverse incidents
 Hazardous equipment and materials should be assessed to fulfil the obligation
 All the machinery, equipment and hazardous substance should be safe and all of them should not be
harmful for the health of the workers.
 Proper maintenance of equipment present in the chemical and oil refinery factory

(b) WSH (Risk Management) Regulations

 Regulation No.: Work Place Safety and Health Act (Chapter 354A, section 65)
 Explain what the selected Regulation No. relates to (in your own words): In this regulation , the section 65 (4)
states that the employers are responsible to eliminate the risks from the work places and thereby the

management should take such initiatives to control the risks. As a part of the measures to minimize the risks,
the management may opt for engineering control, administrative control and substitution of the existing system
as well. Moreover the subsection 65 (8) states that any employer or self-employed person who braches the act
3(1), 4(1), (2) or (4), 5, 6 or 7 will be marked as guilty and will be liable on conviction (Sso.agc.gov.sg 2020).
 Describe application :
 Assessment of the work places regularly
 Use of safety measures such as engineering control, administrative control and substitution of the existing

 Maintain record of any type of risks in the work places
 Review of risky tasks regularly
 Assessment any braches of acts in the work places and if there is any braches the individual must be
charged monetary fines not exceeding $10,000 for the first offence and for the for the second time, the fine
should be not exceeding $20,000. However, imprisonment can also be given to the individuals as well
(Sso.agc.gov.sg 2020).
(c) WSH (Incident Reporting) Regulations
 Cite an Incident Reporting Regulation (e.g. Regulation No. 4(1)) and describe what it requires.

 Regulation No.: Work Place Safety and Health Act (Section 27 and 65)
 Explain what the selected Regulation No. relates to:
The regulation number 4, 5, 6, 7, 8, 9 are applicable to the work place and it can be stated that the this
regulations are related to the incident reporting cases in the oil and chemical factory (Sso.agc.gov.sg 2020).

 The employer must report the adverse incident of the oil and chemical factory within less than 10 days
 The management should mark any type of dangerous incidents in the work places
 In case, the employee face any type injury and thereby face death due to this, then the employer must
report the commissioner within 10 days.
 In case the employee face an occupational disease due to the type of work, then the employer must submit
a report to the commissioner not later than 10 days in the chemical and oil refinery factory.
 The employer of oil and chemical factory must keep record for every notification (Sso.agc.gov.sg 2020).
(d) WSH (General Provisions) Regulations

In the regulation 65 (5), proper ventilation of the chemical and oil refinery work place is described. However,
this regulation does not applicable to those areas in which it is not practicable to implement. The regulation
named 65 (19) describes about the use of hoist and lifts in the work place and its related regulations
(Sso.agc.gov.sg 2020).

 All the lists and hoists should be used under the authorised examiners
 All the lift way should be protected by substantial enclosure fitted with gates.
 Proper labelling of the load capacity should be mentioned on the lift or hoist
 In the oil refinery, proper circulation of the air must be maintained
 In case of the locations where presence of hazardous gases, vapours is reported, then it is the duty of the
employers to secure a proper ventilation in the oil and chemical refinery (Sso.agc.gov.sg 2020).

 Regulation No.: WORKPLACE SAFETY AND HEALTH (CONFINED SPACES) REGULATIONS 2009 (Section
4, section 5)
 Explain what the selected Regulation No. relates to (in your own words): The section 4 of this regulation is
applicable to this work place as in the oil and chemical factory most of the spaces are not enough for the
equipment present in the factory. Along with this, the regulation named section 5, defines the fact that the
employers must take proper initiatives to restrict the entry to the confined places of the factory (Sso.agc.gov.sg
2020).

 All the persons entering into the confined places should be checked properly

 Regulation No.: WORKPLACE SAFETY AND HEALTH (FIRST-AID) REGULATIONS (Section 65 (6) and
Section 65 (7))
 Explain what the selected Regulation No. relates to (in your own words):
The selected regulations will address the issue of first aid services in the work places and in an oil and
chemical factory the cases of accidents are quite common. So, presence of first aid room is very necessary in
the factory. Along with this the employer must provide required safety equipment to avoid any kind of
hazardous exposure in the oil and chemical factory (Sso.agc.gov.sg 2020).
 Describe application (in bullet points):
 First aid room should be present in the factory.
 The employer must arrange proper measures to combat any adverse incidents
 Complete protection of eyes and body must be ensured by the employers
 Proper equipment must be delivered along with proper training should be provided to the workers of the oil
and chemical factory (Sso.agc.gov.sg 2020).

From the risk assessment table, it is observed that the incident has the likelihood score is 4 and the score of
severity is 5 for this task. Hence, the overall risk score is almost 20.

After implementing the risk controls, it can be observed that the overall risk score has reduced to 15 due to the
reduction in the likelihood score to 3 (LxS= 3x5= 15).

 Safety officer should assess the risks of the factory after implementing the controls
 Regular audit must be performed by the health and safety officers.

root cause of the accident/incident.

tearing of fork lift chain in the work place.

 Tanker of the oil is lifted with the fork lift
 All other workers were working on the work place
 Suddenly the rope and chain of the fork lift tore and the oil tanker fall on the workers
 One of the workers died on spot and 3 others injured severely.
 Actions taken to preserve the scene of incident/accident (in bullet points):
 As the oil tanker is heavy and so it is not possible to remove it quickly and so the tanker kept on the
same place.
 The injured workers were evacuated quickly without tampering the spots of the accident
 No other people were allowed to enter the site of the accidents.
 The safety officer and emergency team were informed as soon as possible.
(c) Title of Methodology used to identify root cause: For identifying the causes of the accidents, the MTO analysis
was performed.

The MTO analysis works on the Man, technological and organizational factors that are in case of any type of
accidents, there are equal responsibilities of all these factors. In this context, firstly a structured analysis of the

accidents was performed by using an event- and cause-diagram. After that the, the change analysis process
described how the case of accident was different from the natural condition and it was found that on the day of
accident, regular audit process was not performed and no check of the forklifts was done before initiation of the
working days. After that, barrier analysis was performed to identify the technological and administrative barriers
present in the work place (Vinnem, J.E. and Røed 2020).

 Lack of proper assessment of the equipment before starting the work
 Absence of work health and safety manager as a part of the poor administrative practices.

 Use of stronger harness in the fork lifts during the lifting of the materials

 Marking the particular area as a restricted working area.
