INTRODUCTION
Thesis statement: To evaluate case scenario of age 75 patient by exploring his experience to
improve nursing practices and partnering in care.
Reflecting on nursing practices is the best tool for experiencing own knowledge and
improvement measures at the time of practicing. The focus of study is to explore experience of
patient of 75 years in order to prevent falls and harms which are caused from falls. The name of
patient in this case is Bill and the reflective will be based on recent experience faced by
individual to gather advice for improving care services.
MAIN BODY
Description
As per entire conversation with Bill, I determined that he experienced various type of
experience with health care organisation and service provider which helped me in understanding
needs of improvement in care services. On the basis of conversation, it can be outline he does
hesitate to share about his different health concerns and accidents experienced by Him.
Moreover, I feel involvement of Bill helped me in exploring his overall experience and
partnering in care. As per conversation, it has been outlined that he experienced couple of fall 6
years ago which affected his overall health and made him weak. During conversation my focus
was on not to hurt his sentiments or moral concern therefore I did not ask about any of the health
concern. He initiated with all issues and health care experiences faced by him which helped me
understanding the importance of communication with service user and improvement area of
health care services.
Feelings and thoughts
On the basis of conversation, I felt the need of effective interaction with servicers
because it helps health care practioner in analysing holistic needs of patients. Moreover, I felt
that Bill experienced lot of falls because which it I important to prevent further harm as it can be
risk to his life. Communication with bill was like general and helped me in gaining insights over
nursing practices which affect patient involvement in general care hospitals. In my opinion, the
most disheartening and disappointing experience of Bill was 11 hours waiting in public stone
when he was experiencing acute pain of kidney stone. As per my understanding I feel waiting of
patient in hospital with intense pain is the biggest negligence a health practioner and needs to be

addressed by implementing alternative solution. Consequently, conversation with Bill inspired
me to be more attentive over service user and their changing holistic needs.
Values and Beliefs
My values and beliefs reflects my working and nursing practices where I am focused on
understanding each and every requirement of patients. I feel nursing values are code of ethics
which helps the practioner in address health services equally. It is important for health practioner
to include integrity, autonomy, dignity, social justice and altruism which was neglected in case
of Mr bill when he went to public hospital at the time of kidney stone. The core ethical concerns
are helps care providers in managing safe and secure care services for patients (Preventing Falls
in Hospitals, 2013). Apart from this, Wilson and et.al., 2016, outlined beliefs of nursing practices
which aim at partnering in with effective communication and interaction between patients and
heath care workers.
Analysis
In accordance with entire conversation with Mr bill and E health record, I analysed the
need of preventing falls because it is possible risk to situations which he was experiencing
continuously 6 years ago. Like, first he fell while regular exercising, when he walked to shop and
at time of coming back he tripped over piece of concrete because he neglected to blow his nose.
This was the major incident of fall which wrecked his right shoulder, took skin off left elbow,
chin and nose. Further, after this fall he neglected going to doctor at same time because he
wanted see his General practioner because of trust and this overnight wait cost him his arm.
After this, he used to vision physiotherapist 4 time a week and just after 4 visit he again
experienced a fall which was because of iron deficiency and haemoglobin count.
According to E health record and interaction with Bill I analysed the need of preventing
fall in older patients. Mackey and Bassendowski, 2017, stated that health care providers play a
crucial role in controlling falls at hospitals. Actively targeting individual fall prevention is best
practice which can helps nurse in managing risk. Moreover, the research denotes that health care
provider should make proper care plan for service user where screening diagnosis regularly can
help the individual in managing effective care and preventing risk of fall which cause major
harm like in case of Mr Bill. As per my evaluation, I feel the negligence of over diet and iron
intake is the cause which led to continuous fall in the case. However, Boyle and et.al., 2015, has
discussed multiple reasons which needs to be considered by health care provider in order to

prevent risk of plunge. Some of the health reasons which needs to addressed by uses are
dizziness, balance of patient, weakness of muscles, stiffness in joints, poor eye sight, side effects
from medicines and slower reaction times (Moyle and Hughes, 2018).
Conclusion
The analysis, feelings and values summarized my understanding over conversation with
patients, E health records and nursing practices. In my opinion partnering care is the best choice
with helps health care practioner in addressing basic needs of patients and controlling
uncertainties. On the basis of experience of patient of 75 years I outlined the need of building
relationship with service user by incorporating nursing values and beliefs. In accordance with
this it can be said that it is important for health care provider to communicate with service user to
analyse his or her holistic needs to prevent risk of hazardous accidents like in case of Mr Bill
where his iron deficiency and haemoglobin blood count led to second fall which could have life
threatening. The analysis outlined the necessity of preventing plunge in hospitals for managing
patient safety. From case scenario it is clear that patients had bad experience with public
hospitals which denotes the need of integrating nursing ethics and code of values at time of
communicating and dealing with service users.
Action Plan
My action plan is based on case scenario and my learning over preventing falls in
hospitals. In future I will focus on first screening whole condition of patient by keeping a regular
check over their diagnosis. In addition, my focus will be on effective communication and
building relationship with service user in order to manager partnering care. Apart from this, my
focus will on placing patient at safest place like near to toilet and be keeping rooms clutter free
(Manojlovich and Ketefian, 2016). Further, informing family or immediate guardians about care
plan will help in controlling event of fall. Lastly, in my opinion lighting around patient is
essential for preventing plunge.
CONCLUSION
The assessment summarized the nursing care for preventing falls by assessing case and E
health record of Mr Bill, who is 75 years old man and have experienced continuous fall over
years. The report outlined nursing values and beliefs which needs to be incorporated by nurses to
manage safe care plan. Hence, it concluded by determining action plan which is framed by nurse
on the basis of analysis and will used in future to prevent falls and gain trust of patients over care

services. Hence, discussion outline various measure which needs to be screened by health care
practioner to prevent fall like, stiffness in joints, weakness of muscles balance of patient and
poor eye sight.
