Introduction
This paper seeks to address in details how student nurse can transition into Registered
Nurse within the Australian Nursing. The transition process should be effortless once an
individual is able to integrate theoretical knowledge and work practice (Jones & Harper, 2014).
The expectation is that, a new graduate should exhibit knowledge and competencies so as to be
absorbed in the competitive job market. Academic and skills qualification should match the
National Competency Standards for Registered Nurse as chartered by the Midwifery Board
Registration Standards.
Registered Nurse uses the Nursing Practice Decision Flow Chart and other existing
thinking plans in making key decisions; so as to ease and encourage patient safety and quality of
care (Perry, Potter, & Ostendorf, 2015). The paper adopts the STARR format so as to effectively
address the essential selection criteria.
Essential Selection Criterion1
Situation
Caring for an elderly patient in the hospital as a Nursing Intern to a community hospital
in Australia. The patient is at risk of multiple poor outcomes including but not limited to
prolonged length of stay, functional decline and readmissions (Arvold & Reardon, 2014). The
patient has refused to perform routine exercise, which was designed by the physical therapist to
help to improve the patient’s mobility and reduce falls. I am a member of the coordinated
multidisciplinary team that is providing treatment to this particular patient. Together with a
fellow Registered Nurse, we are taking care of this patient.

It is of vital importance to secure the patient’s cooperation. Make the patient feel a sense
of purpose. This makes it easier to work together with the patient so as to achieve the common
goal; improving the patient’s condition. There is need to assist the patient to transform and
become better self-managers of their health care (Lum, Sudore, & Bekelman, 2015). Organize
my team to ensure we succeed in implementing the tailored health care to the patient.
Action
The standards of care embody the least acceptable level of practice that a registered nurse
must take into consideration so as to avoid being found negligent or guilty of malpractice. Nurses
should employ good judgment using their nursing education to the best of their ability at all
times (Halcomb, Stephens, Bryce, Foley, & Ashley, 2016). In search a scenario, leadership and
good decision traits come in handy. For me, I calmly and firmly reminded the patient of
importance of participating in this exercise, but this time round asking him to be the team leader
(Maurice-Szamburski, et al., 2015). I allowed him to play his favorite music; he led us to do the
body exercise by asking the team to through our hands in the air while clapping to the rhythm of
the music.
While the other nurses were talking to the patient in a tone of I know-it-all, I knew they
were not going to succeed in making the patient perform his routine exercise.it is very important
to think like an intelligent nurse but communicate in the language of the patient. When the
patients feel a sense of power or control then they own the caring process. Gradually but steadily
they change their behaviors and adopt healthy self-care habits. Critical thinking and analysis of
the nursing practices outlined the Midwifery Board Registration Standards, is an important
aspect that help to solve nursing problems (Östlund & Persson, 2014). It is indispensable
technique for a benign, well-organized and proficient nursing facilitation.

Results
I find this approach effective as it helped to calm and persuade the patient. The fellow
nurse has since adopted the method whenever speaking to the difficult patients. Treatment and
care for the patients requires consistent and cooperative efforts by both the health care providers
and the patients (Hanson, 2014).
Reflection
I have learned the importance of working with a team, and being flexible enough to make
necessary adjustments when changes are required. Registered Nurse is accountable for their skill
and proficiency development and to contribute to the career development of others. I have
realized the importance of making healthy first impression with the patients, this makes them to
trust you and be cooperative (Tofil, et al., 2014). These factors collectively contribute to safety
and well-being.
I have resolved to provide health education to the patients. Health education is able to
empower the patients to make informed decisions about health care. Also, I will continuously
attend the in-service education, simulation and continue to teach colleagues as a way of
influencing better health outcome for patients.
Criterion Scenario2
The environment in which the nurses are operating usually determine the quality and
safety of patient care. Nurses are expected to apply their skills, knowledge and experience to
provide care to the numerous and altering needs of the patients. When quality of care falls short

allocation or due to absence of suitable policies and standards (Jones & Harper, 2014),
Registered Nurses shoulder much of the responsibilities of excellent patient care.
Situation
Placement at a regional hospital where I actively engaged in community life caring for
patients with wounds. I visited the patient who had chronic leg lesions. The hospital team had
been visiting the patient for a while now, about one year (Lopes, Almeida, & Almada-Lobo, B,
2015). On this occasion the patient has requested my fellow nurse not to put the K-lite dressing
on, and instead I should do this. The patient pointed out, previously when the other nurse dressed
the wound the dressing became loose.
Task
Map the dressing so that the notes had updated record on the wound size. Wash and dress
the wounds as per the plan of care. Clean the wound and then apply aqualcel Ag silver which is
helpful to wounds that have high exudates level (Malamed, 2017).
Determine the patient’s condition and monitor their progress. Ensure patient tailored
objectives are achieved within the time frame. Accurately obtain data and information from the
patients. Show compassion to the patients as some are experiencing profound pain (Hanson,
2014). Wounds can become life threatening if not handled properly. They can be slow healing
with expected to reoccur. This should be prevented from happening.
Action
The nursing professional standards are responsible for ensuring that the registered nurses
are accountable for their clinical decisions and actions. These standards are patient centered and
aim at promoting the best possible outcomes, holistic nursing. This what prompted me to analyze

data and information collected through comprehensive and systematic assessment of the patient’s
condition (Halcomb, Stephens, Bryce, Foley, & Ashley, 2016).
The outcome is then communicated for purpose of basic intervention. There are
numerous data collection methods that is used to collect accurate and up to date data from the
patients. The methods included; history taking, interview and observation (Al Sayah, Szafran,
Robertson, & Bell, 2014). Also physical scrutiny such as inspection percussion, palpation and
auscultation was also used. As a Registered Nurse I used appropriate appraisal tools and
approaches to ease the data collection process.
Planning for continuity of care becomes fundamental after conducting thorough
assessment of the .this aids in achieving the specific patient goals within the right time schedule.
Nursing care plan is constructed in consideration to priorities, goals, actions and timeframes
relevant to specific patient (Perry, Potter, & Ostendorf, 2015). Strictly adhering to the nursing
handover, I conducted admission, shift and focus assessments. This helped to monitor progress in
the patients’ conditions and make informed judgments about the patients’ health.
Result
Evaluation of the nursing practice ensured progress towards the expected goals and
outcome of the care service. I was able to demonstrate my ability to provide safe, suitable and
receptive quality nursing care as the objectives were met effortlessly, and positive feedback was
collected from the patients.
Reflection
I have realized that nursing care plan is fundamental to providing safe and quality care. It
is grounded on the Registered Nurses evaluation of complete, pertinent information and evidence

that is documented and communicated. It is evident that health care services are delivered to
patients in a multifaceted environment, which has several interactions among many factors, for
example the disease process itself, clinicians, procedures and processes. In such an environment
harmful and unexpected errors may occur (Perry, Potter, & Ostendorf, 2015).
To eliminate or minimize such outcomes from occurring then it becomes fundamental for
the Registered nurses to strictly adhere to the already existing nursing standards. Hence, in my
future engagements as a Registered Nurse I will always endeavor to practice ethically while
identifying and addressing complex issues. Try to understand the effect of the determinants of
health on client wellbeing and advocating for change where appropriate.
