help health workers comprehend the level of support which a patient needs and also how to
deliver it. Addionally, they help them to provide care which meet the needs and emotions of a
patient. All care plans guarantee promoting person centered values because they contain full
history of a patients. The things which need to be in place to enable care plan promote person
centered care include personal biography, medical history, mobility and nutritional care plans.
Therefore, health workers should go through all that information to understand what is required

Because data can be collected from co-workers, friends and family members of the
patient it can be possible for the health workers to analyze it to see whether there are areas that
need to be improved. Feedback data is analyzed through categorizing the comments.
Categorizing comments means going through your feedback to see the repeated patterns and how
clients are saying about your services concerning speed and deliveries. After that, you should
divide those categories into sub categories to see areas which requires unique services. Thirdly,
one should specify according to the nature of the comment. People will give both positive and

looks at harms which are brought about by medicines. In this case, this assessment tries to look
whether medicines are stored correctly and if they are exposed to sunlight which can spoil them.
It also checks whether the medicines given to patients are not expired and are those prescribed by

On the other hand, nutrition risk assessment looks at harms which are brought by foods
and drinks. It ensures that foods and water given to the patients are not contaminated. Also, it

of the patients to more risks. The health worker will not collect the necessary information
required because he or she is in a hurry to be left alone.

external body which carries out regular checks within the care settings to ensure health workers
are applying necessary in preventing and controlling infections. Additionally, they check
whether the equipment used are clean and secure. Health workers should sterilize them before
using them. Also, they ensure that the kind of care givens meets the patients’ needs and
preferences. Moreover, care quality commission ensures that the people employed have the
necessary skills and qualifications, portray good character and are experienced. Lastly, the
commission ensures that the health care systems are updated and the workers are able to perform
work they are assigned for.

against control and prevention of infections for adults. The commission also ensures that adult
people are given quality care. Moreover, it ensures that they are given nutrition which aims at

performs their tasks from the beginning of their project to the end. The second model of team
working is create an ace team which argues that it can be hard for a team to work while it’s
highly productive. To form an effective team, all members should fully participate not just
leaders.
Team development is important because it brings views of all workers together which
helps in achieving organizational goals. Tuck man explained five stages of team development in
his model. According to tuck man, the first stage of team development is norming. In this stage,
members comes up with rules and regulations of the team. Members are usually new to each
therefore level of interaction is usually low. Storming is the second stage in team development.
In this stage members slowly stars interacting with others but do not term themselves as a team.
They are usually hostile when group leader try to control them. Norming is the third stage
whereby workers feel that they belong to a team and can achieve goals if they accept opinions of
others. Performing is the fourth stage whereby individuals work openly with others and are able
to share ideas for the achievement of goals. Lastly, adjournment is the stage of the tuck man
model and in this stage workers conduct an assessment every year to see the performance and
also to see if their rules and regulations needs an adjustment.
Shared goals plays a vital role in team and everyone is advised to demonstrate good
practice within their role. Workers should understand their roles and its impacts within a team.
At the beginning workers are not aware of their team members and in order to familiarize
themselves with each other, group leaders can create time for social activities. Through this
social activities, they will get to who is who, who does what and their boundaries in workplace.
Every team has their rules and members are requested to follow them. They are also supposed to

misunderstanding between two employees, group leaders should solve the issue in a respectful

underpinning teamwork? This means that members have confidence when giving out their views
and opinions and that they are being taken seriously. Moreover, communication plays a vital role
in underpinning team work. Communication is important because members are informed about
the progress of the team and be reminded of their roles. Also, recognizing and rewarding team
members can lead to effective teamwork. This will encouraged members to work extra hard
because they feel their efforts are being recognized and appreciated. Also, members should be
flexible with each other. They should feel free to share ideas and opinions. They should also
discuss some of the problems they are facing in order to get solutions. Lastly, there should be a
healthy competition between members of the team. This will make each member to concentrate
on their roles hence minimizing work conflicts.

when they are needed. For example, one can show respect to a person when communicating to
them. This include paying attention to what their saying and responding when necessary. There
are things which can show disrespect during communication like tapping of legs or crossing
arms. Moreover, one can show respect within a workplace through obeying what he or she being
told by others. For example being asked to wipe seats when they have a meeting. Obedience is a
sign of respect and it creates effective teamwork.
Change is important for organizational success because it involves adapting more
effective measures to achieve goals. At first, employees are reluctant to adapt to change because
they feel they are being mistreated. However, there are ways in which teams can manage change.
This involves holding meetings to discuss any challenges faced and how to solve them.
Communication is important in teamwork for it helps workers to know their progress and
whether the change implemented is effective. Another way in which a team can manage change
through effective training. Once change has been implemented, it’s hard for employees to
implement it without proper training. Team leaders should therefore organize for training to

Values of an organization can affect how members of a team work. Values are derived

to the manager if he or she believes a change of one’s behavior can affect the performance of the
group. To the nurse, team members should report when they feel there are some of the health
issues which is preventing them to perform well. In this case, nurses will advise them on what
they should be doing before and after performing tasks. To the occupational therapists, members
report to them if they feel a certain task requires an expert. An expert will help them with
elaborate ideas on how they should perform it.
Every team member has his or strengths and contributions towards achieving goals.
Occupational therapist treat injured and disabled people using therapeutic daily activities. They
ensure patients are well. Occupational therapy assistant helps occupational therapist in ensuring
injured and disabled peoples treated well. Psychologists takes care of the patients who are
mentally ill. Psychology assistant helps psychologists in taking care of the mental ill persons.
Ward manager ensures that patients are comfortable within the wards. Consultant ensure that
patients get necessary information needed about the hospital and they also solve hospital
problems. Health and well-being facilitator ensures that patients receive food which is not
contaminated. They also ensure they receive medicines correct for their illnesses. Finally,
registered health nurse ensures patients are given care according to care plans.
Psychosis. This is where a person is detached from reality by delusion or hallucination. It
is therefore hard to give necessary information required for their care. For example, one may not
be aware of their medication history

she is not able to communicate well with others. For example, they may get upset when being

background which is important.
Dementia. This is damage and lack of oxygen in the brains. For example, People with
dementia tend to loos memory and may not provide all the necessary information required in a
care setting.
Learning disability. This can be caused by brain damage or during child birth. Also the
problem can result due to other factors which influence the normal functioning of brain such as
anxiety, depressions among others. Characteristics of people who have this condition being
unable to understand questions asked hence making it hard be given quality care.
Psychosis. It is essential that people with this conditions be given enough care. Staffs
should be highly trained on how to handle these people.
Depression. Contacting people who are experts in dealing with depression will help the
patient come out of these situations.
Anxiety. Anxiety is the same as depression therefore it requires experts to help patients in
solving their problems. People should also be treated well to overcome anxiety.

encourage them to take foods which are free from cholesterol.

of simple sugars and it’s mainly found in fruits, honey and root vegetables.
Not all people are advised on balanced diet because of various reasons. For the veganism
they are advised to take foods which are less in proteins. Islam also take foods which is low in
proteins. They adapts to this because their religion requires them to. For the pregnant mothers,
they mostly advised to take food rich in folic acids and increase their fruits intake. Diabetic
people take food with less sugars. Gaul stones are advised to take food rich in carbohydrates.
Apart from seeking nutrition and hydration guidance from a nutritionist, I would read
from online website to increase my knowledge. Members of my unit would access the additional
information about dehydration and nutrition though googling on their phones.
