Main aim of community based rehabilitation is help individuals
with disabilities by providing them physical therapy, equalization
of opportunities and medical integration rehabilitation programs
for disable or the people who are suffering with certain diseases.

Description of the model of care
Advanced care planning is a powerful resource in health system
provided to particular setting, disease and help patient regarding
treatment choice. Patient outcomes demonstrated to better when
advanced care planning is integrated to care of individuals with certain
diseases so that patients can choose their own treatment. Advanced care
planning shows potential as impelling paradigm across prevention
spectrum such as primary, secondary and tertiary using a pattern for
health providing strategies for reduction illness in earlier life as well as
managing it in later life.

Model of care and its relevance to case study
In the given case study 58 years old woman with metastatic ovarian cancer
named Carla. Carla has undergone multiple therapies such as
chemotherapy and surgery. The disease is progressed further which made
Carla to depend on others. The advance care planning include healthcare
system related to particular disease, choice of treatment and care which can
be choose by patient and also practices which helps to patient to overcome
with the disease.

Facilitators and barriers to providing effective
client-centred care
To improve quality of care and empower patients, policy makers generally
adopt systems to intensify person- centred care. Healthcare providers such
as nurses seek to improve client-centredness differ, respecting preference
and needs of people receiving care. Barriers such as traditional structures
and practices; stereotypical attitude of professionals, sceptical and factors
affiliated to development of client-centred interventions.

Conclusion
From above mentioned presentation, it has been concluded that
advanced care planning include the decision making of
patients regarding the treatment for particular disease and
setting which helps to overcome from the chronic disease.
This model helps to reduce the illness and also prevent the
disease in initial stages.
