emergency department for the recent fall and have a primary complaints of difficulty in
breathing, influenza A and increased confusion. The patient has a past medical history of arterial
fibrillation, chronic kidney disease. After assessing the patient, it is identified that the patient is
suffering from dementia. He lives alone after the death of his wife. The family of the patient is
involved in the care of the patient, and requires further help of the aged care and social worker
for the proper management of the patient.
The professionals chosen for the discussion to improve the physical and mental condition
of the patient is the social worker and the physiotherapist.

participate in the same. The social worker will ensure that the patient and the family is involved
in the care planning and will also work to synchronize the discharge arrangement. Hence, by the
collaboration of both of the professional, the patient will get the appropriate care and a fast
recovery.
Nurse plays an important role in the inter-professional practice as they communicate the
worries and concerns of the patient with the other healthcare professional. In this case, the nurse
should communicate effectively with both physiotherapist and the social worker. At first the
nurse should take appointment from both the professional and during meeting should convey
each and every detail related to the health condition of the patient.
The nurse should maintain an effective relation with the healthcare professionals to
enhance the health outcome of the individual of the patient.
The health condition of the patient, pervious medical history and the risk-factor
responsible for the mental and physical health condition of the patient is effectively
communicated to both professional with the help of the written communication and verbal
communication.

assessment reports, previous medical reports, diagnosis reports and medication report.

to convey all the information which is not conveyed through the written communication. For
example, the social condition of the patient that is, the patient lives alone after the death of his
wife. He has two three children and all of them are involved in the care plan of the patient.
Inter-professional collaboration enhances the quality care of the patient, reduces the
chance of clinical errors, minimizes healthcare costs, however there are certain barriers identified
which hinders the effectiveness of interprofessional. One of the major barriers identified during
the communication is the difference of personal values and beliefs between the health
professionals, which can be overcome by maintaining an effective therapeutic relationship. It can
be achieved by interacting more and discussing their goals for the patient with each other.
From the discussion, it can be summarized that inter-professional practice in the
healthcare is effective in the effective treatment of the patient. For enhancing the result outcomes
of the patient by inter-professional practice, effective communication between the healthcare
professionals and the nurse is vital. In the inter-professional practice, certain barrier have been
