a) Introduction to the essay including NMC confidentiality statement
Over the years, individuals from the most part of the world remained untreated and
preventable constant sicknesses like hypertension, diabetes, weight, and cardiovascular ailment

industry has formulated an effective program i.e. Integrated Care (Carey, 2018). Present essay
will be enclosed with patient scenario, evaluation of ways healthcare provider & other
individuals as well considering multi-disciplinary team were involved in planning integrated
patient care, and learning experience as well. Also, this essay is based on integrated care of an
individual whose name is Mrs. Jones (Hypothetical name) who is 67 years old dealing with
Dementia (Alzheimer's Disease), Type 2 diabetes along with a history of 20 Years regular
smoking which brought her to deal with Chronic obstructive pulmonary disease (COPD), but she
stopped smoking in 2018 (This is a developed scenario for Mrs. Jones to understand the
intergrated care). Lately from 2019 she started taking treatment in an effective Integrated Unit
of National Health Services (NHS) England. Basically, due to Alzheimer's Disease it has been
analysed that she forgets to take daily pills and insulin, which helps her in reducing the chances
of fluctuations in her health conditions and may lead her to severeness as well.
Away with this, if it is talked about the NMC Confidentiality Statement which has been
taken into consideration for Mrs. Jones then it is “Created in a joint effort with numerous who
care about great nursing and health assistance, the Code can be utilized by medical caretakers,

At the time of treating Mrs. Jones, I (nurse) and my team members and Two carers

Evidence based individualised patient care is considered to be the faithful, express, and

(Inzitari and et. al., 2018). If it is critically analysed then, it can easily be said that EBP would
directly may involve a range of steps and these are: formulating a clinical question for the case of
Mrs. Jones. Second step is to identify the health issue faced by Mrs. Jones and then develop an
action plan and gather the best evidence, which would help patient i.e. Mrs. Jones to get treated
in much effective and in efficient manner. Third step that came in front was to analyze the
evidence, as it would lead the team to deal with the complexity of both Type 2 Diabetes and
Alzheimer's Disease, which Mrs. Jones is having.
After this, fourth step was to applying evidence to clinical practice so that right treatment
could effectively be given to Mrs. Jones based on how her body responds on medication that has
been given and so on. Away with this, assessing all the results that came in front could be
considered as the last stage of Evidence based individualised patient care as this would help Mrs.
Jones confidentiality of diseases and it will also lead her to get treated right on time.

Being a nurse, I have learnt many things considering the experience which will help me

issues by alluding to a patient's history, looking at them and directing neurological tests. Another
responsibility that is required by me to focus on advicing the patients on neurological scatters
and share my experiences with carers so that they could also learn as well.
