INTRODUCTION
To meet the needs of service users, effective assessment of those needs requires correct
services. There are many old age people who like to stay at their own homes and wish to remain
free and independent, with the rights to support. It has been determined that many services have
started adapting and creating new services to fulfil the needs of older people.
The case is about an old widow whose name is Mrs X and her age is 73 years. After her
husband died, she has been feeling very alone and isolated which has left her with inability of
moving freely. Moreover, she finds bathing difficult but still able to cook & manage her home.
As a manager I have appointed a carer to examine Mrs X and ask her about her need.
TASK
Question 1
Explore structures, models that are available to support an assessment of Mrs X's support needs.
Based on the study, Mrs X finds very difficult to move and also bathing is hard for her as
she unable to manage to get into the bath safely. Further, it has been identified through the case
that she doesn't drive and her family live away from her (Anderson, Dabelko-Schoeny and
Fields, 2018). As increasing ratio of non communicable acute diseases among older people is
also contributing to raise their complex health and social needs. It has been determined that due
to difficulties in caring for adults, health care professionals face many problems in giving
effective care management & also to avoid their unplanned admission in hospitals. There are
certain models that aims to build a link between the health & social care system to improve
service coordination across sectors and also to enhance the clients experience. Here are some
models and systems to help Mrs. X :
CVW model :- Community Virtual Ward Model has been designed to assist healthcare
professionals in order to assist adults at home during time of their illness or functional
decline. As per the case, Mrs. X is unable to work properly as she finds difficult to
manage at two steps which are available at her property outside front door. The model
was developed to work within available resources within both primary & secondary care.
The model is being considered as an important tool in providing timely services &
follow-up (Bereznoy and Saygitov, 2016).

There are certain risk factors available, that might arise in a situation of taking her to
hospital and those factors are; falls at home, living alone, and cognition. Under this model it has
been discovered that older age people who live with care giver have fewer chances of having
admission in hospitals rather than those living alone. Therefore, through an integrated approach
to care, a CVW model in the care of Mrs. X can reduce her unplanned entrance to hospital
(Caruana, Lou, Gehrke and Elhadad, 2015).
Rainbow Model of Integrated Care :- this has been designed to provide a description of
the concept of integrated care which is complex. The model is considered as a
measurement tool which has been created to assess integrated care within the care system
as a whole collected from healthcare providers & managerial perspectives. It has been
determined that this a critical strategy for keeping the sustainable & affordable healthcare
services. The model is considered important for enhancing the processes of services
usually provided to patient with complex needs (Important Caregiver Skills To Keep
Seniors Happy In There Home, 2017).
A New Hub and Spoke Model :- this model is also designed for providing health care
services. However, hospitals deliver complex care as needed, but the aim is to cut down
the need for these services by better hindrance & follow-up care delivery. Basically in
this model a hub is placed at the centre & the spokes are all linked to the hub. In addition
to it, it helps to having multiple practising sites that helps to connect with the person who
is suffering from the health issues.
Individual Model of Integrated Care :- this integrated care model gets both for individual
coordination of care for patients at high risk or with many conditions. The aim of this
model is to supply appropriate health care services & results to adult people. Mrs. X can
be benefited through this model's individual care plans, which are designed to bring
intended results (Del Carlo, Paglia and Foy, 2018).
ASPIRE model :- it refers to Assesment, Systematic nursing diagnosis, Planning,
Implementation, Recheck and Evaluation. It has been determined that, users take
ASPIRE as an easy to use & right way to address patient's outcomes & enhance their
confidence level.
AIPE :- it refers to a problem solving process that is used to direct individuals in a
systematic approach to problem solving. It stands for Assessment, Planning,

Implementation and Evaluation. Further, this process involves organised steps made for
care providers to give excellent care.
Apart from this, a patient centred approach is considered as the best model that provides
supportive care for old age people in their isolation i.e. who live alone at home. For instance, the
approach can address the problem of isolation of Mrs. X. Another model is Active service or
Wellness model which objective is to create functional independence, social engagement, and
quality of life instead of depending on services.
Conceptual and service Model :- mainly the model focuses on maintaining and enhancing
the functional & physical health of older people using the therapeutic intervention &
those that aim to get better psychological welfare through social interventions. Moreover,
other than this it pays attention on delaying admission to residential care by allowing
older adults to live in the society for a long time (Errasti-Ibarrondo, Carvajal and
Arantzamendi-Solabarrieta, 2014).
Long Term Care Model :- This refers to those services which are designed to provide
help for a long time to compensate for the loss of functional abilities due to any chronic
illness. It has been determined that the LTC assist older adults in two ways; one is
activities of daily living and second is instrumental activities of daily living. Daily life
daily living consists of primary functions as eating, bathing, using toilet, bathing, etc. and
includes additional tasks which are essential to maintain independence for eg. Cooking,
shopping for groceries and using transportation. Hence, this be definitely help Mrs. X to
make her work easy as she already wanted home help to assist her with the house work
and also in the process of managing online delivery of food for her.
Structure of Health sector
Public health units play a very important role in giving services to elder people and make sure
the efficiency and quality are properly undertaken by health organisation, professionals, and
regulatory bodies, which also involves nurses, surgical areas and health groups. Further, the
Ministry of health has a range of responsibilities in the system in addition to being the principal
advisor & help to minister. In context with Mrs. X, with the help of this model she can have

multiple practising sites for her which are easily accessible (Kennedy, Enander and Marcantonio,

I know taking care of people is something that need a lot of Patients Therefore, when it comes to
assisting elderly people, there are a number of challenges that can be met only by having certain
skills. Challenges faced by them in daily life may occur due to many reasons and closely
connected to other health issues. Those challenging situations are; bathing, grooming, dressing or
eating and they also loss their control if unknown people get involve in their private daily lives.
For Mrs. X my goal is to maintain her ability to live, function for as long as possible while
maintaining some independents, to arrange the right care and call for healthcare professionals
when needed (Lee and Shin, 2016). Carers who are unable to provide sufficient care as a result
of their deficiency in skills, are replaced by another carer.
I am good at -
Good communication skills are considered as an essential part of healing process. I have good
communication skills which will help me in interacting and understanding her, therefore building
trust. Being able to interact well is as simple as doing the daily activities to maintain the health of
senior adult (Mollica, Litzelman and Kent, 2017). I can also perform the basic components of
geriatric assessment, which involves standard method for helping emotional, physical and social
functioning. Also i am good at showing empathy which means I am good at giving my time to
assist and care for older people. Understanding Mrs. X daily challenges, her needs, and
limitations will make things smoother for her and also enhance our relationship. Empathy gives
me a view of trying to fit myself in other's situation. To me, being empathetic will raise the
quality of care & the amount of time I provide. Therefore, this will improve the quality of life of
Mrs. X. moreover, Patience is one of my best skills which will be very effective in supporting
her. Patiently listen to her problems, and assisting her with her needs this will help her feel less
isolated (Wiig and et.al., 2014).

A fundamental skill that is key in dealing with health issues is patience, is needed in order to
accomidate and communicate appropriately with elderly people. With the help of this skill, I put
myself at second priority so I can feel her needs and understand her issues. This skill allows you
to slow down and rethink about any improvments that can be made moving foward. I know that
it is one of my best and most important skills that helps me to analyse the views of elderly people
by putting what they want and need first. Thus, positivity gives me the strength to deal each task
in easier manner. I use to think optimistically at each negative situation of Mrs X and it aided in
providing a better quality of care. I have observed that an optimistic attitude can even rub off on
the older person and they also becomes very happy whenever I behave in such a way. Care plays
the essential role in our society and it also helps to create the number of social and economic
benefits for themselves (Perry, Potter and Ostendorf, 2017). In this term, it can be said that
there are wide range of services and organisation that assist in terms of support and health, well-
being and independences of carers and the people they use to care for. The effective caregiver
must have the skills in terms to managing the stress as it assist in terms to improving well being
and help in caring role (Nicolaidis, Raymaker, and Boisclair, 2015).
This helps me to know her health issues and it develops a healthy relationship. In my opinion a
good carer needs effective observation skills for observing the circumstances of the elderly and
what they might be going through. I always visit Mrs X when she needs medical care or at times
when she feels vulnerable and wants to share something. It helps Mrs X to be calm and relaxed. I
also noted that good observation power also aids in observing the health conditions of person. In
addition to this, I have analysed that effective and better body language skills help to create a
positive and happy enviroment. It is both a mix of observation and communication. Thus,
effective communication and keen observation helps to understand a person body language, by
his facial expression I can observe the things that he or she is suffering from. I always use to put
detailed attention to the body language of MRS X or any other person she is talking or listening
to. By doing this, I can analyse what they really need at any particular time. I must say that,
being carer it is essential to be skilled in both activly and passivly.
As long I have taken care of Mrs X I have realized that ability to read body language is one of
powerful tool that supports to take very good care of her. I always put my major emphasize over

the Mrs X so she can feel happy with me and discuss her issues at any time she wants, as I just
need to tackle her issues so she can live happily and positively. Being a carer, it is crucial to keep
in mind that we should have ability to make life easier, happier, better and more fun loving for
senior adults like Mrs X. In addition to it, consistently putting the aforementioned skills in mind
supports to be more attentive at time of taking care of the senior citizen, Furthermore, I must say
that communication plays vital role with helps to ensures that senior received the best care.
Question 3
Your views as manager in this outcome of Mrs X.
I must say that Mrs X is good lady but she use to feel lonely at times and that makes her
not able to move freely. Sometimes, she feels deep alone and thinks a lot about the circumstances
she had faced in her past life. At this stage, this leads to create many physical issues. Being
manager, I suggest her to appoint a care taker who willingly provides care by showing both
empathy and compassion. I was asked to appoint a care given person to Mrs X, for this I decided
to analyse the health condition so I can able her to provide good person who can take care of her
like her own family member. I must say that whether you are member, neighbour or any
professional being caregiver comes with its own challenges and responsibilities. I also decided to
provide her the arrangement of online delivery of food so she does not have to visit shops to get
the stuff. I have asked to care worker to explain reason of Mrs X mobility and reason of
loneliness. The appointed person has reported me after identifying her needs as she requires
home help to assist her in house work. In terms to maintain effective carer of Mrs X I decided to
provide her a skilled take care for meeting her needs. I recommend to person appointed by me is
to make good and better communication with Mrs X so it will provide us a good chance to take
good care and keep her always happy. It is better way to live longer and by ensuring best
possible health and well-being. I also advised him to be with her at times when she goes out and
access her needs so it could be easy to provide best possible care to her. As Mrs X is facing the
issue as mobility difficulty so I advised to Caregiver to take her to Exercise programs that assists
to administered the both needs as she meet new people so she can feel lively or explore with
other. This aids her to improve her health conditions by exercising that may include balance,
strength, functional retraining and aerobic (Yan and et.al., 2015). Furthermore, group classes
with new people provides an opportunity for social interaction and it also helps to prevent

loneliness (Core Competencies for the Care of Older Patients, 2018). I have proposed various
strategies to care giver appointed by me to do following things as are-
To have the use of personal systems to overcome the barriers to medication adherence.
By having regular review of medications by the doctors or any well skilled medical
practitioner to eliminate unnecessary conditions.
To give her medicine during proper interval.
To keep on check her daily intake so it helps to have healthy routine.
I have suggested care take to have the regular medication reviews it is one of implementation of
electronic medical records. With help of it, care taker and doctor can analyse the health condition
of Mrs X. I also advices to care giver to take her out in parties, events, club or senior get together
so there she can feel lively or to change her surrounding at times so she can no more feel
isolated. I must say surroundings make us feel more positive and it provides beautiful scenario to
look for life. I told care giver to maintain frequent contact with her. As technical advancements
made this possible to see face of the person when you are not with them. I also advised care giver
to prepare one care plan that helps to address the strength and weakness of Mrs X so it aids to
demonstrate how many hours of care they are requiring in day. Thus, one of the common duty at
time of care-giving journey is to transport senior person to doctor appointment so regular check
up of her health can be taken.
CONCLUSION
From above report it can be concluded that health care management is term in which health care
managers provides effective facilities to the patients so that they can have health and well being.
The present assignment is based on Case study of Mrs X 72 years old a Widow, she has health
issues as mobility difficulties. It was caused due to feeling of isolation following her stroke. As a
manager I was asked to appoint care worker to Mrs X to access her needs. The main aim of this
report is to address good care so that she can live longer with better health conditions. It is
essential to understand the parents prior level of mobility, independence in self care and usual
living situations so better and effective care needed to be provide.
