INTRODUCTION
This report implies about the health and care of an individuals and the patients with the
requirement of their mental health care. As, I have been working there from 9 years. I have
noticed many things which should imply and be managed in the hospital or in the health care
NHS organisation. The report is mentioning about all the assessment and their processes, about
the attention unit, along with understanding and the leading control, in addition to this it will
provide contribution to the assessment (Reeves, Espin and Zwarenstein, 2011). All the terms are
clearly explained which will provide the better outcome on the working of an organisation. The
role of the individual which help them and even their family to manage it and mental ability if it.
TASK 1
1.1 Different forms of assessment
In the present case, the individual can initiate self-estimation method whereby one can identify
along with the weakness. The best way for the assessment can be done by taking the feedbacks
from the patient, their family and other experts. Classification is the kind of technique where one
can know or identify the skills and abilities that has been followed currently. By which the
improvement can takes lace and the area of improvement can be modified. This technique is very
important for the health and care department to give the better and appropriate service.
Assessment can also be categorized on the basis of different purposes and others, from the present
case, the health and social care entity is using assessment forms for the purpose of evaluating the
efficacy of health care service (Antheunis, Tates and Nieboer, 2013). Along with this it also
providers the feedback and the regards which are very important for the client to be taken in
order for the maintenance of their health and care services. This technique in being used for the
better facilities to be provided to the patient and improvement in the overall department. So that
with this, the client are able to find out the effectiveness and usefulness of health care centres and

also encourage the clients to provide better health and care services.
1.2 Partnership work can positively support assessment processes
In the partnership, the work and support of an individual varies from one person to the another

different clients through different methods and this can develop proper relationship among varied
entities. In the partnership, the partners can individually divide the work with each other. That
conduct the work easily with appropriate manner. Assessment process is very essential for the client
to get an opportunity to look around the areas where the change and development is required. In the
health care unit it is very important to satisfy the client (World Health Organization, 2015). The key
fact needs to take in action and different changes required to be made. It actually provides the overall
view regarding the effectiveness of individual. It also reduces the possibilities of the risk. In the
partnership going together it can be handled in much effective and efficient manner. Partnership
helps in managing the assessment process in proper manner. The client gives the proper details and
feedback in the delivery process. Though it has seen that, working together helps in assessment
process that assist the interest of stakeholders. With the help of partnership, the appropriate result can
be found out about the performance of the health care.

2.1 Early assessment of the individual
The abilities of an individual can be analysed during the induction process and interviews. So
nowadays the health care is conducting the early stage assessment. It will show the proper ability
of the person and it can judge though it. The appropriate roles and responsibility is identified by
the help of each individual. It helps to know which person is conducting the work, what is the
work kind and who are having the capabilities to perform that work. In the health care
organisation, the total early stage process is being set differently so every individual is being
assigned by different works. The whole process of the early stage is being managed in the
interview session. The specified rules for the selection of the individual in the health care
organisation help them to do the assessment processes with more effective and efficient manner.
All the candidates are assigned for the different jobs according to their carrying out of the

the suggestion have to be considered (American Speech-Language-Hearing Association, 2016).
It will increase the interest of the client to be more effective which will help in encourage the to
others. To increase the assessment processes time to time the training program has to be

As a social care senior support worker, I should be qualified and should meet all the aspect of the

planning and management skills. Likewise, undergoing all the responsibilities of the care and
social senior support worker I should manage all the skills and have the better framework

physical interventions includes all these activities and exercises that are required for the patients
of dementia, although some physical activities will not encourage in aspects to the fitness but
these improves the mental and physical health status of the patient. Psychological intervention

can improved.

4.1 Other understandings of functions and range assessment tools
With the help of some other understanding number of people are working such as care
coordinator and senior care workers, in order to assist patients for the enhancement in their
physical and mental health. It is critically very important for to understand about the
development of the care plan and tools for the help to the patient. In order to this some training
courses are attended which are performing, these courses will assist in the completion of new
documents for the paper work and new tools which are used in the determination of other factors
of the risk (Grol, Wensing, Eccles and Davis eds., 2013). However, in this the most important
factor is awareness and training of the staff team. Initial training is important with this staff of
the company would be able to understand not only about the process but also about the reasons.
For an example, medication in the prescribed way and other training in which the medication is
being used. In this the most common mechanism is providing to the staff by their proper and
regular training & development activities.
In this the minimum requirement is that the team members will meet the minimum training
targets from the issued by National Training Organization. Also, it is the responsibility for taking

as the training and manual handing such as Controlling over infection, Assessment of the risk,
planning for the patients care and their health and safety measures, along with medication and
other psychological activities which can develop their physical and mental status. In this training
part most of the focus in on the development of others and patient by using the above mentioned
functions and tools.
In addition to this there are number of physical and conditions which can damage the
mental conditions and health of the patient and others, for example an individual suffering from
diabetes as a result can promote in HYPER and becoming aggressive which could be mistaken in
displaying their challenging behavior, this is because it is very important for the staff team to be
aware and have full knowledge about where these individual are needed to provide medical

attention. Another example can be taken in which the patient is suffering from Asthma, which
leads him to anytime deal with the asthma attack in this the staff team must have knowledge and
understanding of these condition for there intervention and proper treatment for every individual
to seek this medical help.
4.2 Other Understanding that the assessment may have negative and positive impact on an
individual and their families
Users of the service and carers family and other surrounding to the carers plays a very
critical role in the success of the care planning and the management of their caring (Shortliffe
and Cimino eds., 2013). Involvement of the family and other surrounding is an integral part for
each of the thematic headings. Involving of these individuals and family in these following ways
such as sharing of the information like the reasons and information about the illness and there
cause, along with other factors and processes of the risk assessment.

Fire safety measures, First Aid activities in case of Emergency, Medication, training insanity and
others (Dye, Reeder and Terry, 2013).

appropriate treatment and well looking to their loved ones. They are also pleased by the way in
which the service users are making her choice and having review meetings for the care plan and
taking of their consideration into work.
5.3 Development of an action plan
The main purpose behind the development of the action plan is that the person who is
receiving the service is having an individualized personal plan relating to their support formally
or informally. This plan identifies the support which is chosen by the individual. In this the
desired outcomes of the support and the reason behind the support along with the chances for the
review the plan more effectively. The plan provides the written summary of the goals and about
the issues and about the planned strategies of each support, these plans are intended for assisting
the person, in order to this intent and the purpose of the support. In this few plans are triggered
