INTRODUCTION
This report implies about the health and care of an individuals and the patients with the
requirement of their mental health care. As, I have been working there from 9 years. I have
noticed many things which should imply and be managed in the hospital or in the health care
NHS organisation. The report is mentioning about all the assessment and their processes, about
the attention unit, along with understanding and the leading control, in addition to this it will
provide contribution to the assessment (Reeves, Espin and Zwarenstein, 2011). All the terms are
clearly explained which will provide the better outcome on the working of an organisation. The
role of the individual which help them and even their family to manage it and mental ability if it.
TASK 1
1.1 Different forms of assessment
In the present case, the individual can initiate self-estimation method whereby one can identify
along with the weakness. The best way for the assessment can be done by taking the feedbacks
from the patient, their family and other experts. Classification is the kind of technique where one
can know or identify the skills and abilities that has been followed currently. By which the
improvement can takes lace and the area of improvement can be modified. This technique is very
important for the health and care department to give the better and appropriate service.
Assessment can also be categorized on the basis of different purposes and others, from the present
case, the health and social care entity is using assessment forms for the purpose of evaluating the
efficacy of health care service (Antheunis, Tates and Nieboer, 2013). Along with this it also
providers the feedback and the regards which are very important for the client to be taken in
order for the maintenance of their health and care services. This technique in being used for the
better facilities to be provided to the patient and improvement in the overall department. So that
with this, the client are able to find out the effectiveness and usefulness of health care centres and
On the basis of assessment the quality of the services can also be modified. Altogether it will
also encourage the clients to provide better health and care services.
1.2 Partnership work can positively support assessment processes
In the partnership, the work and support of an individual varies from one person to the another
on the basis of the treatment required. The work and responsibility are also distributed in
partnership. Individual can support and give feedback to each other regarding the improvement,

and work experience. They both can deliver the proper feedback to each other that how the work
and performance is going and they can improve by that basis. Partners can collect advice from
different clients through different methods and this can develop proper relationship among varied
entities. In the partnership, the partners can individually divide the work with each other. That
conduct the work easily with appropriate manner. Assessment process is very essential for the client
to get an opportunity to look around the areas where the change and development is required. In the
health care unit it is very important to satisfy the client (World Health Organization, 2015). The key
fact needs to take in action and different changes required to be made. It actually provides the overall
view regarding the effectiveness of individual. It also reduces the possibilities of the risk. In the
partnership going together it can be handled in much effective and efficient manner. Partnership
helps in managing the assessment process in proper manner. The client gives the proper details and
feedback in the delivery process. Though it has seen that, working together helps in assessment
process that assist the interest of stakeholders. With the help of partnership, the appropriate result can
be found out about the performance of the health care.

2.1 Early assessment of the individual
The abilities of an individual can be analysed during the induction process and interviews. So
nowadays the health care is conducting the early stage assessment. It will show the proper ability
of the person and it can judge though it. The appropriate roles and responsibility is identified by
the help of each individual. It helps to know which person is conducting the work, what is the
work kind and who are having the capabilities to perform that work. In the health care
organisation, the total early stage process is being set differently so every individual is being
assigned by different works. The whole process of the early stage is being managed in the
interview session. The specified rules for the selection of the individual in the health care
organisation help them to do the assessment processes with more effective and efficient manner.
All the candidates are assigned for the different jobs according to their carrying out of the
interview session or through the induction process (World Health Organization, 2014). The
selected candidate is given the basic cases to be handled and then their performance is being
recorded in the health care centre. The early stage categorization is done for to recognise the role

according to which the individual given the cases to solve it. From this process, the knowledge
and abilities of the respective can be identified according to which the role and responsibility can
be assigned. The selection of the individual can be recognised by this classification and the best
according to it is given. The job is being given after the early stage assessment. The selection is

that there would be no confusion among the people. And the one who knows what to do in the
best possible way will do their job in the effective and efficient manner.
2.2 Participation of the individual in shaping the assessment process
All the staff members are involved in the requirements of the assessment processes in the health
and care organization. In the health and care team everybody has a chance and right to show and
provide their opinion regarding the client and the case to be handled and how and what are all the
things to be seen and improved (Brownson, Colditz and Proctor eds., 2012). All the individual is
used to comment and discuss about the performance of the individual. To develop and improve
the assessment process the individual has the right to put their view. The opinion regarding the
suitability and service they can make the appropriate changes in the assessment processes.
Actively participation of the all the Client will help the health care to develop the assessment
processes. Health care build the appropriate policy for the changes made as per the requisites. To
build the proper health and care organization addition of the several aids to be made. These aids
maintain the clarity in the assessment processes. The health care evaluates the performance of
the practitioner through the assessment processes. The mental health care has to build the process
of equality among the client so that raise the concerned the interest of the person. The work and
position have to maintained and secure by this process. Equal participation by everyone has to be
requires and important to manage all the assessment. Involvement of all the client, health care all
the suggestion have to be considered (American Speech-Language-Hearing Association, 2016).
It will increase the interest of the client to be more effective which will help in encourage the to
others. To increase the assessment processes time to time the training program has to be
conducted to them. To inform all the aspect of the care unit. Everybody should be convenient
and equally given the chance to share their view and that can be implied according to
everybody's choice.

As a social care senior support worker, I should be qualified and should meet all the aspect of the
required or as per the requirement for it. All the type of knowledge, good working capacities and
key skills like communication and verbal speech. With these types of knowledge, I should have
planning and management skills. Likewise, undergoing all the responsibilities of the care and
social senior support worker I should manage all the skills and have the better framework
(Kakuma and et.al., 2011). Wherever the eligibility and creativity of the health and care don't
meet I have to take care of it and has to arrange the formation. Any of specific services which are
needed to be applied to the client or any mental health needs can be provided. Medication to be
appropriately provided whenever needed. The organization of the health and care the individual
are assessed as the care provider to ensure position they can meet and the preferences. Care and
social assessment has to be planned under or within the health care organization. Meeting the

resource and it also deals I the helping of the people. The senior support worker working with the

3.1 Develop the care plan in collaboration with individual
In the health and care unit, it is important to come out or make a plan for the patient suffering
from the disease. The clients are needed to provide the best services. As in the present scenario
the patient are suffering from the adult health problem, therefore the health and care unit have to
plan all the circumstances according to it. All the specific care and concern unit have to allot
with the roles and responsibilities to manage accordingly to the convenience of the patient. The
management should look around to all the assigned and managing crew. After that all the
individuals have to assign the specified work, as planned by the management have to schedule all
the activities to be done or being seen under it. The referral process can be used or identified here
to use the best possible service and medical arrangement (Shim, 2010). The person with the best
known to the adult health disease can be applied with the team who will treat or operate it. So
that the person will get the best care unit to look after the case. Risk assessment is the key point
for me to understand what is happening to the care unit. Whether all the distribution, handling
and safety have been going with the proper requirement. The outcomes of the risk to balance

time scale. Risk assessment, includes the information, observation and, communication and
investigation activity. It is the step by step process to be considered. The proper appropriate
knowledge and skills are collected. The meaningful information for the support and services. It
can be useful for the families and the health care unit to interact with the families about the harm
and risk.
3.2 Implement interventions that contribute to positive outcomes
An individual will surely get the best result when he/she is get the entire staff operating with
him/her. Everybody should work having a single goal to get the best outcome. Everyone
employed as the team member sharing the same objectives will give the better results for the
patient. The individual can take the step to communicate the expert to take the best decision for
proper services. As the care service member, I have to take care of the services and planning of
the individual. For the conduction of the best service, the best member has to work on it. I have
seen the individual fully dedicated, and has to appoint rather than fitting any of the one. The
patient can be assisted with the positive and improvement session so that they feel encouraged
and in performing several actives (Creek and Lougher, 2011). The motivational programs and
session will encourage him and make him strong. He will get the fighting spirit from the
motivation and encouraging session. From this the health and care unit will found the areas of
improvement being there.
In this short of interventions can be varied which can be used as an asset for the
delivering of the various services such as physical in health and social care setting interventions,
psychological interventions and other self helping from which an individual is required is allows
himself in getting recovered and assist himself with the help of this other therapies. Patients who
are suffering with dementia are needed to very actively participate in the processes of service
delivering through which they can improve their opportunity for the recovery and with this
participation it will also enhance their rate of recovery. Along with all these activities, counseling
sessions can act as the best type for interventions which will definitely facilitate the patient in the
present case who are suffering from dementia and can feel encouraged. In addition to this
physical interventions includes all these activities and exercises that are required for the patients
of dementia, although some physical activities will not encourage in aspects to the fitness but
these improves the mental and physical health status of the patient. Psychological intervention

can also help to facilitate patients with the help of motivation sessions so that their mental status
can improved.

4.1 Other understandings of functions and range assessment tools
With the help of some other understanding number of people are working such as care
coordinator and senior care workers, in order to assist patients for the enhancement in their
physical and mental health. It is critically very important for to understand about the
development of the care plan and tools for the help to the patient. In order to this some training
courses are attended which are performing, these courses will assist in the completion of new
documents for the paper work and new tools which are used in the determination of other factors
of the risk (Grol, Wensing, Eccles and Davis eds., 2013). However, in this the most important
factor is awareness and training of the staff team. Initial training is important with this staff of
the company would be able to understand not only about the process but also about the reasons.
For an example, medication in the prescribed way and other training in which the medication is
being used. In this the most common mechanism is providing to the staff by their proper and
regular training & development activities.
In this the minimum requirement is that the team members will meet the minimum training
targets from the issued by National Training Organization. Also, it is the responsibility for taking
an account and differentiating between the individual teams members by the management along
with their needs for the development. Function of a range of assessment tools can be considered
as the training and manual handing such as Controlling over infection, Assessment of the risk,
planning for the patients care and their health and safety measures, along with medication and
other psychological activities which can develop their physical and mental status. In this training
part most of the focus in on the development of others and patient by using the above mentioned
functions and tools.
In addition to this there are number of physical and conditions which can damage the
mental conditions and health of the patient and others, for example an individual suffering from
diabetes as a result can promote in HYPER and becoming aggressive which could be mistaken in
displaying their challenging behavior, this is because it is very important for the staff team to be
aware and have full knowledge about where these individual are needed to provide medical

attention. Another example can be taken in which the patient is suffering from Asthma, which
leads him to anytime deal with the asthma attack in this the staff team must have knowledge and
understanding of these condition for there intervention and proper treatment for every individual
to seek this medical help.
4.2 Other Understanding that the assessment may have negative and positive impact on an
individual and their families
Users of the service and carers family and other surrounding to the carers plays a very
critical role in the success of the care planning and the management of their caring (Shortliffe
and Cimino eds., 2013). Involvement of the family and other surrounding is an integral part for
each of the thematic headings. Involving of these individuals and family in these following ways
such as sharing of the information like the reasons and information about the illness and there
cause, along with other factors and processes of the risk assessment.
Safeguarding and development of key social support network, choice of the improvement
and different others, in this the involvement of the family may cause some positive and negative
impacts such as for an example, it can put negative impact on the well being of the service user
such as may be family Is insisting to shower a service user where there may be no appropriate
equipment to accordingly provide services in this condition, this can leads the family to think
that the service user is not receiving appropriate and proper hygiene care from the hospital where
no other hands are taking for consideration to the factors of health and safety.
4.3 Other understanding of the contribution in the process of the assessment
In this it is required for the whole care staff to attend detailed care induction
programmers and their progress can be evaluated with the help of regular mentoring and
appraisals (Ogden, 2012). Proper and effective training activities for the staff is very much
important and compulsory o regular basis basically annually. In this training process for the staff
they come to learn about several activities and skills such as under the legal terms and system of
the hospital which covers health and safety 1974 act- like Hygienic food, patient personal caring,
back care, good communication, bereavement, keeping of the records, challenging behaviors,
protection of the data and confidentiality, positioning and moving, other aspects regarding to the
Fire safety measures, First Aid activities in case of Emergency, Medication, training insanity and
others (Dye, Reeder and Terry, 2013).

and are fully supported by the management of the teams. In this, all the carers are encouraged for

regulate the staff activities and are implement on daily basis throughout the whole staff and their
work in accordance with the guidelines that is interlinked with the care support plan and other
risk assessments. It is the responsibility of the management to monitor care plan and review it as
according to the allocated time scales. Some relevant legislation that can affect the work of the
management are the roles regarding to the delivery of the service and care plans abiding to the
Health and safety act 1974 are-

Under these care standards every individual have the right for the assessment of their
needs and to meet them as far as possible. All the organization working linked with Bradford

is also required for the management to carry out monthly audits which involves care homes for
an example GP, OT, users of the service, District nurse, Physiotherapist, opticians, dentists,
social services, CPN, and others. It is also the responsibilities of the management to invite other
outside agencies and professional for the completion of the service questionnaires which will
enable the organization for the maintenance and striving of the highest standards for the care.
After getting the feedback from the professional and their views on my referrals were relating
and appropriate for the success of the organization, and about various outcomes and management
of the time scale (White and Dudley-Brown, 2011).
5.2 Evaluating the outcomes of the assessment which are based on the feedback from the

according to the statement of the Gps they are satisfied with the services which is providing to
our service users and our execution of the care plan is pretty much satisfactory and updated.
District nurse have stated that they are very much satisfied to appropriate treatment to the service
users. Also, the families and friends of the service users are very much satisfied because of
appropriate treatment and well looking to their loved ones. They are also pleased by the way in
which the service users are making her choice and having review meetings for the care plan and
taking of their consideration into work.
5.3 Development of an action plan
The main purpose behind the development of the action plan is that the person who is
receiving the service is having an individualized personal plan relating to their support formally
or informally. This plan identifies the support which is chosen by the individual. In this the
desired outcomes of the support and the reason behind the support along with the chances for the
review the plan more effectively. The plan provides the written summary of the goals and about
the issues and about the planned strategies of each support, these plans are intended for assisting
the person, in order to this intent and the purpose of the support. In this few plans are triggered
