for this class.
1. Behavior problem(s) to be addressed:
Diabetes is a common health issue which is one of the greatest health concerns for the healthcare providers from
all over the world. It has been found in the statistical report that the number of people suffering from diabetes has
risen from 108 million in 1980 to 422 million in the year 2014 (Rutledge, Masalovich, Blacher & Saunders,
2017). Again, in the year 2016, 1.6 million deaths were directly caused by diabetes. Healthy diet, mainataining a
regular physical activity to maintain a normal body weight and reducing alcohol and tobacco uses can be helpful
in controlling diabetes (Bullard et al., 2018). However, people cannot maintain this healthy lifestyle and lead an
unhealthy life with some unhealthy living habits. This health disorder is associated with various kinds of
behavioral determinants. Recent researchs in this regard concentrated the behavioral influences on diabetes. This
behavioral intervention will address the behavioral issues that contributes to the incidence of diabetes. It is
focused to reduce the number of diabetic patients and its consequences can also be avoided.
2. Target population or community to be engaged in your intervention:
All the adult diabetic patients of some specific healthcare organizations will be the target population in this
intervention. Also, as it will be a family intervention plan, the family members of the patients will also be
included in the names of target population. The collaboration of the family members is important to assist the self-
management process of diabetes.
3. Potential stakeholders
The patients, along with their family members will be the main stakeholders for the intervention plan. Apart from
this, some videos and internet based knowledge will be share with the patients and families and so in this regard,
support from the healthcare organizations will be required and the healthcare providers will be also potentially
involved as stakeholders.

4. Why/how do you think that changing the behavior will improve your target population’s health outcomes?
With the family system framework, the interventions will provide education on diabetic management to the
family members Several studies indicated the importance of family involvement so that a health diet plan ccan be
introduced to the patients (Baig, Benitez, Quinn & Burnet, 2015). Treatnment of diabetes involves healthy diet
plan along with some ohysical activities. Achieving and maintaining a healthy body-weight is dependent on a
healthy diet plan. Avoiding sugar and saturated fat intake is also important and if the family of the patient is
following the routine plan then it can provide additional mental support to the patient because, due to sudden life
style changes, the mental stability of the patients is hampered and they might feel isolated from the society which
in turn causes depression or any other mental health disorder. Again, if the patient is addicted to tobacco or
alcohol, then family members can be supportive enough in keeping the patient away from such substances abuses
which will improve the diabetic condition of the patient. It is expected in the outcome of the intervention process
that with proper physical activities, patients will be able to manage a healthy weight and excessive body fat will
be reduced which has contribution to the diabetes incidence (Hilliard,Powell & Anderson, 2016). Therefore, the
interventions with family will be quite helpful in improving the condition of the diabetic patients and it will also
reduce the occurrence of diabetes among the future generations.
