Answer 1
While preparing Mr. Montoya for discharge, the nurse should first assess the home
nursing management to determine who will take care of the patient after the discharge.
According to several studies, it has been observed many patients die or return to the hospital due
to inadequate palliative care at home. However, patients with neuro-oncologic diseases require
high-quality care at home (Pace et al., 2014). Then, the nurse should prepare a diet-chart for the
patient with the nutritionist as the nutrition contributes to the healthy body, which is very
important for a good recovery. Social services play an equally important role in the recovery of
the patient. Therefore, the nurse should concentrate on social services interview for the patient to
improve his emotional well-being.
Answer 2
The fundamental role of a nurse is to support and care for every patient. For this patient,
the nurse should record and manage all the medical reports of the patient. Then, to provide better
care, she should collaborate with other team members of the healthcare team. She should
collaborate with a dietician to understand the diet plan for the patient to fulfill all his nutritional
needs. She must assess the signs to understand potential issues and communicate them with the
specialist for making right decisions for further treatment. She is also responsible for the good
emotional health of the patient. Thus, she should collaborate with social service worker to
encourage the patient that he will be fine in a few days. As suggested by some studies, continued
psychological care plays a significant part in reducing anxiety and depression in such patients
(Xiao, Xiao & Zhu, 2018).
