Katy is a girl aged 24 who experiences many difficulties due to her disability. Katy finds

from her parents because she cannot stay alone and she can stay with those who understand her.
But her parents have to go to work meaning that they cannot fulfill this requirement successfully.

positively with the care providers and she is also able to participate in social activities. The social
events such as swimming and going to the cinemas have improved Katy’s performance, and she
now feels better and can even sleep better than before. It means that Katy’s life has upgraded and
her mode of interaction with the environment had become better and will no longer have
sleeping problems like when the care plan was not implemented. She can now sleep without
issues because she feels more satisfied with her surrounding and her needs are satisfied
accordingly.
Another achievement is that Katy’s mother no longer has to quit her job to take care of Katy’s
needs because there are caregivers who can provide these services. She was also under pleasure
because she could not come into the right decision on what to do with Katy. She did not have a
solution on how to take care of Katy due to the complex nature of Katy’s disability. Through the
health care plan, these problems have been eliminated, and the condition of Katy no longer
pressurizes Katy's mother. All the care is provided, and she has all the time to do her work
without worrying about the wellbeing of Katy.
Lastly, Katy no longer gets infections while trying to thrash because the caregivers understand
what she needs. It means that the wellbeing of Katy has improved and the weight has also
increased because she is given proper nutrition and is provided with the required services. But
the caregivers have not yet been able to understand Katy’s needs because her mother is the only
one who has stayed with her for a long time. It means that some things cannot be sufficiently
achieved through the employment of caregivers. Evaluating the performance of these areas,
however, will help in demonstrating what has worked and what is yet to work in the care plan.
