This capstone project focuses on developing and implementing a patient and community education program about diabetes. The project involved creating an education plan, including visual presentations and handouts, to educate in-patients and community members about the causes, symptoms, risk factors, and precautions related to diabetes. The target population included in-patients with diabetes and community members at high risk or pre-diabetic. The project involved collaboration with co-workers, management approval, and the formation of a team through convenient sampling. The rationale is based on evidence-based practices, aiming to reduce adverse situations and improve health outcomes through awareness and education. The program and handout provide information on diabetes, its risk factors, and preventive measures, encouraging healthy lifestyle choices and proper reporting to healthcare providers. The project highlights the importance of education in changing patient behavior and improving healthcare efficiency and quality.
