Intervention: Patient and Community Education Program with Handout (Diabetes)
Summary of Intervention and Implementation
The purpose of this project was to educate the in-patients and the community people
about the Diabetes and its causes, symptoms, risk factors and precautions through Patient
Education and Handout. For this an education or training plan was developed in which the both
visual presentation will be shown and handouts will be distributed to both the in-patients and
local community people.
Target population of this project was the in-patients who are suffering from various
physical conditions due to diabetes and the local community people where the prevalence of
diabetes is very high. The community people include the pre-patients who were already treated
for comorbid factors of diabetic disorders and vulnerable people who have higher risk to suffer
from Diabetes due to obesity.
As initiation I communicated with my co-workers and asked them to review my plan and
provide feedback about attainability and relevancy. After collecting feedback and making some
changes in the plan, a written proposal was presented to the management board of the healthcare
center. The proposal was consist of the planning, timeline, budget and required human resources.
In order to develop a team a convenient sampling procedure has been used where among my
coworkers interest individual could join voluntarily (Miller, 2016).
Through the inter-professional collaboration and communication it has been found that a
large number of healthcare or social care worker believe that education program can helpful to
control the emergency or adverse situations for the comorbidity associated with diabetes. It has
been also found that through team work with proper interpersonal communication and

coordination, implementation of any healthcare quality improvement tool becomes better
structured, efficient and effective for the target population.
Evidence based Rationale professional product development
In order to improve the health conduction and lifestyle through educating the target
population for reducing the risk factors and prevalence of high-risk situation an education
program and handout have been developed. In the education program the target population will
be educated by the professional healthcare workers and social service care worker regarding the
diabetes and its causes, symptoms, risk factors and precaution. For education audio visual
presentation will be used and a handout will be distributed. The handout is filled with preventive
measures, risk factors and risk reporting instructions with simple and easily understandable
approach with pictographic information.
Diabetes is a disease that occurs due to presence of excessive blood glucose in blood
because of lack or less secretion of insulin hormone. Increased glucose level in blood can
increase the comorbid issues such as kidney disease (diabetic nephropathy), eye problems
(diabetic retinopathy), nerve damage (diabetic neuropathy), dental disease, physical weakness
foot problems and other. The risk factors are too much body weight, consumption of sugar, lack
of physical activity, high blood pressure and others (Waters et al., 2013). Both admitted patients
and common public can become exposed to these risk factors every day. Exposure to these risk
factors not only adverse the current condition of the patient but also can cause terminal organ
failure. Awareness within the target population and conscious precautionary actions taken by the
population in their everyday routine can prevent them to live this kind of un-desirable lifestyle

(Ansari, 2013). The chosen health education program with hand out is aimed at reducing the risk
of adverse and emergency situation through making the target population aware.
The audio visual education program will inform the audience about the causes and risk
measures of Diabetes both type one and type two, which will allow them to realize the
essentiality to comply with the preventive actions. For in-patients, the program will be also
inform about how to report to the healthcare workers and healthcare facility management about
certain kind of exposure during admitted period. The hand out will allow the audience to keep
the detailed preventive activity plans with them so that they can apply the knowledge
accordingly. It will also encourage them to avoid unhealthy risky practices such as consuming
junk food, high glucose beverages, smoking and others.
According to Nazar et al (2016), educating patients through training changes their regular
lifestyle and behavior significantly. Training through audio visual presentation allows the target
population to strongly realize the significance of preventive measures and importance of living a
healthy life. A study on different awareness building interventions shows the pictographic
handout with instruction can help the target audience to keep track of their regular activity while
measuring effectiveness of the preventive measures (Vas et al., 2017).
After being aware of the risk factors and preventive measures both patients and local
community people can change their regular activity so that the frequency of severe occurrence of
comorbid emergency situations associated with diabetes can be reduced. Through proper
reporting patients can help to increase their health quality and reduce their hospital stay (Htoo,
2015). Both of these will be helpful for cost efficient and time efficient healthcare service
providing.
