INTRODUCTION
Down syndrome refers to a genetic disorder which can cause due to presence of third
copy of chromosome 21. this is the syndrome which is associated with the physical grown. In
this, individual is not able to develop effectively and requires different care and support through
people or form social care worker. This is the syndrome which is associated with different
problems where individual is not able to grow effectively and can cause difficulties like in
hearing, or understanding things. It also impact their personal and professional development and
can create impairment within people. To provide services and care for people with down
syndrome there is need to take care with higher level. In health and social care sector, healthcare
worker needs to take care of individual's need which should be fulfilled to improve quality of
life. In this, there is major role play of social care worker who can provide different level of care
to the people with down syndrome. There is need to restore and balance individual and family
life. In this, report there is need to evaluate the care plan which is needed to fulfil the associated
care which is required to particular patient. There is need to take care of the plan for effective
support to achieve the goals. There is need to use the personal skill as the health care worker. It
is essential to communicate with patient that can help to delivery quality of care to my patient
with down syndrome (Bunster, 2021).
MAIN BO1DY
As a social care worker, there is need to identify the care need of Ola which is necessary
to help and support to achieve his goal effectively. In this process there is need for the proper
assessment which is effective and allows to get better understand about the need that can help
social worker to delivery their best practice through using their skills and better practices. For
this there are some of the assessment process which should follow the theory like APIE or
ASPIRE. These are the assessment theory which can help to evaluate the care needs for the
better understand of the Ola's need that should be fulfil to achieve the goals (Malinzak, 2021).
APIE MODEL
APIE model stands for Assess the problem, plan the response, implement the plan and
evaluation of response. It is an appropriate model which is effective and can help to get better
understanding the problem and help in achieving problem solving with care planning. This

model is generally use in the healthcare as nursing care practices. There is discussion about the
Ola's need which are going to be discussed through the APIE component (Jacob, 2021).
Assess-
This is the element in which there is identification of the problem which is necessary to
focus on the solution which can help to provide solution to social worker. This is highly needed
and can be evaluated through getting the medical history of Ola's and talking with his parents
who can provide brief description about the need of Ola for his proper information that can help
individual to get better and effective care that is effective to understand the need of individual. In
this, there is need to find out if there is any problem with the Ola and should also identify the
issue which needs to be resolved for the better improvement in health and can support the need
of Ola. This is the systematic integration of quantitative and qualitative data. Quantitative data
includes the information related with the Ola that includes body weight, body temperature, pulse
and blood pressure and other which can provide more specific assessment (Channell, 2019).
Whereas, quantitative data can provide clue like diagnosis of patient, requirement for the holistic
nature like person-centred care to the patient that can effectively supports Ola. There is need of
patient education which is effective for self-care. This can help to take own responsibility to
perform different task and should get independent to perform various activity like going market
and other places without need of any other supervision. Ola also need various needs like
employment, daytime activities, accommodation and to live independently. Ola should also learn
to drive and improve the communication skills that can effectively allow to share the information
and feeling to others (Lunsky, 2019).
Planning-
When there is completion of systematic and comprehensive assessment phase there is
need to plan for effective care which is associated with proper care plan. This involves proper
intervention and strategies which are associated with the different effective strategies for
addressing issues. This can also help to plan effectively to care and support Ola's needs. This can
also help to achieve the goal through patient-centred care, observable, directive, credible and
related other resources. It should focus on the biological requirement and plan should includes
recording of activity, observation, direction and evaluation. I have identified that Ola needs
person centred care for the social model of disability. Disability can be caused due to social
behaviour or any due to getting age (Watchman, 2017). There is need for the patient centred care

Implementing-
This is the stage in which I is need to implement the plan which has been made by social
worker after identifying the issues which need to resolve. During implementation of plan, there is
need of proper participation of every individual including me and the patient with down
syndrome (Ola). This can be effective when Ola also take equal participation in following the
plan effectively. I need to take care of the implementation of plan that can have high impact on
the process and leads to full fill the need of patient. It is also essential to value people plan and
care act. This reflects to intervention process where implementation of care plan can be crucial to
manage care practices (Schmidt 2020). I need to also focus on the making therapeutic
relationship with patient to get involve in the delivering of proper care for their health
improvement with using the appropriate skills. In this, I need to use my communication skill
which can be highly effective to regulate the practices effectively and allow to focus on the
delivery of better care and support to the Ola. With using the skill like good communication,
attention and being calm can help to manage the plan and implement effectively. As social
worker I have also used the organisational skill which allow me to work effectively and can help
to get better and effectively collaboration with the patient and team members. This helped me to
get better access to healthcare services and focus me on the treatment to maintain the dignity
with respect to associated with patient and their family. In this, there should be some queries
which should be answered manage properly. To focus on the care plan, as a social worker, I need
to focus on the safety of the patient to maintain their respect and dignity. For this, I should also
use the documentation skills which can have high impact on patient care which outcomes should
be evaluated effectively. It can also help to find out the improvement needs within care plan
which should be analysed at the right time to manage it properly for the better outcomes (Alesi,
2017).
Evaluating-
This is the step in where, after following effective implementation process there is need
to focus on the delivery of better and effective care plan. It is essential to follow up all the
intervention plan for the better health improvement. This allow to learn the clinical process and

helps in professional development. It also helps me to develop strength and to identify the
weakness while patient care. This process ensures that every implementation of the plan should
fulfil the need of patient and should provide the positive outcomes (Aiello, 2017). This refers to
the clinical review that should be effective and allow to follow the considerable professional
development social worker and in learning process. There are various method can help to
evaluate the outcomes of the implemented care plan and also help to identify the key outcomes
analysis for the work process improvements within physical movements, speaking skills and
motor skills improvement along with social participation. This can be care plan goal which needs
to associated with appropriate body movement that can help Ola to fulfil to perform various daily
activity and fulfil daily needs. It is associated with transportation, speaking with appropriate
body movement and the social participation which should be consider to achieve the care plan
goal (Bell, 2021).
Funding assistance
There should also be evaluation for the need of governmental support that can help Ola's
to get better and quality of care through the free government services. Ola's family is not
financially strong and due to this getting the free governmental services and be helpful to get
better and quality care free of cost. This can be helpful to get allow for better and effective care
through the government services. Though this Ola's family will able to save some money and
that can be used for their other purposes. Ola's family are not able to manage their own needs and
their personal requirement due to having expenses on the Ola's treatment and their care. So,
funding assistance can be highly beneficial for their family to get better quality of life along with
Ola's treatment. There is need for the funding to the Ola's family who needs this assistance for
delivery of quality care and treatment. There are many governmental organisation who provides
care and treatment at lower cost or at free cost. There should also be other funding source that
can help Ola's family to get better and effective care which can be effective to take care of the
individual which allows me as a social care worker to fulfil Ola's need (Lin, 2019).
Person-centred care is highly effective to provide better care to Ola for their care support.
This can be effective and allow to delivery better care and support individual in their life to
achieve their goal and delivery better outcomes. Through this I as a social worker should focus

care and support services. This also involves the person centred care where patient is the priority
for the healthcare professionals to delivery services and putting person at the centre of process.
In this, as a social worker I have developed some skills while delivering care support to Ola and
need to focus on the some skills like documentation skills, organisational skills and others. I have
also improved my skills like communication skills, problem-solving skill and others. This all
allow me to use my skill effectively while providing person-centred care to Ola for his improved
health and to accomplish the set goal (Herridge, 2017).
There are various aspect of social worker role in which was there in case study that
includes the establishment and sustaining of professional relationship with the patient and their
family. I have made better relationship to provide better care and support to Ola which was
effective and due to getting better relationship with Ola and his family, it allow me to get more
about the Ola condition and helped a lot to use my communication and problem-solving skill to
delivery better and care support to Ola for his health improvement. This allows me for the
partnership working which was highly effective while taking care and support to the individual
for their better development. This allows allow for the better assessment and for proper care plan
that can help to get better delivery of treatment along with support. Through using my skill, I
was able to monitor condition of Ola health and also able to evaluate the improvement in health
condition. With better relationship with Ola and his family, as a social worker I was able to
discuss the care plan that have focus on the health improvement and support to achieve goal and
fulfil his needs (Kim, 2017).
REFLECTION
As a social care worker, there is use of many skill which needs to be developed and
improved while delivering of proper care and support to the people who can allow to provide
better and effective services. In this, there is description about the care plan and their
implementation along with the outcomes. All these information is shared briefly through
reflection which are going to be discussed further.

needs. Ola is thirty years old man with Down's Syndrome. His parents were taking care and
fulfilled his needs but due to not able to potentially care about their son, they decided to get help
of person-centred care for better health of their son Ola. In this, as a social worker, there is need
to identity Ola's need and take care of him through implementing care plan effectively (Mackay,
2021).
Feelings- While delivering care to Ola, I feel that there is need to proper help and support
to achieve the set goal for the health improvement within Ola health. I feel that there is need for
equal participation of patient for the better improvement and fulfil the needs of Ola's for health
improvement. I have observed that Ola's need for the participative work within patient and other
healthcare team. I felt that for proper care and support, there is need for focus on the quality of
life. There is need for selection of right intervention to provide personal goal accomplishment.
Evaluation- I have evaluated that due to Down syndrome, Ola was not able to get all the
activity own self. He needs care and support for which person-centred care was essential. It was
not practically possible for parents to take care of their son effectively. So, there was need to
communicate effectively and needs to use other skill while taking care of Ola's health. Through
using the APIE model, I was able to take care of Ola's health properly and support in daily
activity in performing different task.
Analysis- I have analysed that Ola's parent were old and due to this, they were not able to
care for their child and needs more support for providing all day care. I have also analysed that
due to suffering from Down's syndrome Ola was not able to perform his all activity and needed
support and proper care. I have also analysed that as a social worker I needed to use my different
skills which was effectively help to fulfil the needs of Ola.
Action plan- To deliver such type of person-centred care to patient, I need to focus on
the action plan which can help to provide better action to the individual for their better and
improved health. This can be effective and can focus on the making care plan which can be
implemented more efficiency for the better health of patient (Karuppannan, 2021).
CONCLUSION
From above discussion, it can be concluded that there is need to provide better care and
support to the individual with Down syndrome. In this, social worker should have skills that can
be improved during providing care and support to such patient with Down's syndrome. There are
various role of social worker who provides care to their client for better and improved health. To

this, there is need to follow the APIE theory which can help individual to support client to
achieve the goal and fulfil the needs. Through this model, social worker is illegible to get the
strength, aspirations, preferences and personal goals. This can also help to provide the
therapeutic relationship with client to deliver better care and support which needs to discussed
person-centred care for the improvement in social worker and patient. There is discussion about
the person-centred care which have the focus on the individual and social medal of disability.
There is also discussion about the relevant legislation like care act and Valuing people plan. In
this there is also identification of the skills which are required in delivering of person-centred
care to patient with Down's syndrome. These skills includes good communication, problem-
solving skills, documentation skills. There is also need for the learning about self-awareness,
motivation, calmness and critical thinking skills.
