INTRODUCTION
 Integrated care plan is planned with that human being
who works together to acknowledge the service user and
carer needs, keep them in control and coordinates and
offers thee services to achieve the best results.
 Five elements which are in integrated care plan that is
infrastructure, governance, context, financing,
communication and engagement. In the scenario of Mrs.
E, we are going to introduce the integrated care plan.
 In this report we are going to explore about the model or
template or framework of care plan that is utilised during
the care strategy of Mrs. E. attainable targets by the care
strategy is involving in this report.

WHAT IS INTEGRATED CARE PLAN
 Integrated care plan assist to create assisted plan to the
sufferer with evaluation of their complication.
 Mr. E uses hearing aid and walking stick but due to the
illness short term memory loss disease like Dementia, he
forgot to use walking stick and also forgot to put hearing
aid.
 Mrs. E is a lady who lived in a home alone with her husband
Mr. E, who is facing some critical disease like arthritis,
dementia which means short term memory loss, and hearing
problem.

CONTINUE
 Because of this issue, there are are very high probability
to occur an injury of Mr. E so, he requires to take care
full time, which was offered by his wife Mrs. E.
 Mrs. E also diagnosed some of the serious health
problems like glaucoma, heart diseases and vascular
dementia which requires regular medicines to her
health.
 Therefore, now Mrs E is not able to give proper take
care to her husband and herself too. So, both of them
need some integrated care strategy.

 In the case of Mrs E, she is facing dementia and she did not
remember to take medicines at appropriate time and also not able to
perform her daily activities properly, so she required a proper care
plan like:
 she required some support to stay oriented and calm.
 Assist Mrs. E with memory loss medication.
 Manage the behaviour and her sleep problem.
 Talk to Mrs. E very politely and positively.
 Try to Grooming her and make her dressing easier.
 Boost up her activities, which are both like stimulating and
enjoyable.

JUSTIFICATION OF OPTION OR CHOICE OF
MODEL
 According to the present situation of Mrs. E, apply
the holistic model which is a hybrid model.
 This hybrid model is split into the three phase that is
supportive to make support to the Mrs. E in their all
single issue.
 At where, the main features are well evaluate that
may assist the Mrs. E in their issue by applying
some theoretical features and the strategy is
executed.

WHAT WOULD BE THE ASSESSMENT
RESULTS?
 Through the evaluation of power point presentation of
Mrs. E, these are the following results: as this is evaluates
that Mrs. E is facing the condition like Glaucoma and
heart condition because of this she needed to take
medicine. But she can't due to her problem of vascular
dementia.
 So, Mrs. E require some support from the support provider
so, she is capable to make difference between the reality
and non reality base thinking. She will be able to accept
the description of wrong interpretation in the atmosphere.

SMART GOALS
 Specific: Offer relevant care and support and make
strategy that is supportive to reduce unfavourable
situation.
 Measurable: The strategy that is executed to the Mrs. E
is must be analysed and taking under the review process
or supervision that how significantly they are supportive.
 Attainable: The time period that is fixed to the
completion in the care goals is in 3 months.
 Relevant: The aim is clear and the suggestion should
also be taken which assist to perceive the targets of acre
to Mrs. E.
 Time based: The relevant time is group which is crucial
to decrease the complication of health issue.

FRAMEWORK AND LEGISLATION
 In the case of Mrs. Health and Safety as is applicable.
 The equality act is considered under this presentation at
the time of serving Mrs. E and providing some care plan
which provide appropriate care plan to without any stigma
and discrimination.

CONCLUSION
 As per the above discussion in this power point
presentation, it has been concluded that Mrs. E
require to take some of the caring treatment
because she is facing some of the health issues like
glaucoma, heart diseases and vascular dementia.
