theoretical perspectives related to the care planning process are included here to improve care
planning and assessment. Regulatory and legislative framework for supporting diversity and
equality in care planning is evaluated. Benefits and risks of care planning as well as the
importance of leadership and teamwork for care planning are included in this study.

and nothing can cause them any harm.
The nurses and other health care professionals undertake a transforming leadership style to
improvements in the health care mechanisms. Therefore, strong leadership in care planning
requires effective knowledge and other leadership skills. This kind of leadership enables the care

making the patients do their regular exercise and follow a proper diet, the leadership styles

the challenges faced by the healthcare team members as well as the strategies that are been
implemented to overcome those issues. The study also dealt with the barriers, which are been
faced by the unit members. Besides, exploring the importance of leadership and teamwork in
care planning along with the skills and values in relation to care planning had been discussed in
the above context.
