INTRODUCTION
Health and social care refers to those services that are provided from healthcare providers
to people who are in need of it. It is basically the treatment of people who are ill and are in
medical condition in hospitals. Health and social care in UK is a term that refers to services that
are available by health care providers. It also refers to a number of academic courses that can be
taken by students to learn about healthcare sector. For this report, the chosen organisation is
Carers trust 4all which is a British company and is a trading name of Crossword Care that
provide social care to people who need it. They work across Cheshire and Warrington. In first
part, report will cover power-point presentation on role of multi disciplinary teams. Further, in
second assessment it will demonstrate key issue in relation to service delivery.

A. Critically reflect on and evaluate the role of multidisciplinary teams in Carers trust 4all
organisation UK.
Caring and assisting vulnerable people who live in community can be termed as social
care. In UK, whether people like to work with older adults or within mental health institutions,
there are many courses available to provide them knowledge & practical skills that need to
pursue a career (Aveyard, 2014). I have identified Carers trust 4all Company UK as a Health and
Social Care Organisation to work with. It was established as Anglia Secure Homes in 1982 and
in 1994 became Carers trust 4all Plc. It has been discovered that it works with councils, doctors,
clinical commissioning groups to supply care for elder people and those who have mental health
issues, apart from which it also provides services for NHS patients. They follow their
commitment to providing every residence and patient with the highest standard of care service
and in the best possible way and more than 110 care homes they have across UK.
Since last 2 years, I have been working in small social care centre in the UK. I wanted to
work with Carers trust 4all company to learn more about health care services and gain
experience in this field. I want to work with them because they bring the same quality of service

to health care business as their home care services does. I made a call at Carers trust 4all
regarding my current work and told him why I wanted to work with them. I assured them that I
will also make efforts in providing quality of care to meet the high levels of services that they
expect from me. They agreed to my request and asked me to join as soon as possible.
A multidisciplinary team is defined as a group that consists of health care workers who
are considered as the member of different disciplines, for ex. Psychiatrists, social carers etc. Each
of them provide particular services to patients (Brett, J., Staniszewska and Suleman, 2014). In
Carers trust 4all, multidisciplinary team involves staff from various different professional with
different areas of expertise. When I started working with them, I found that their team have
highly skilled & dedicated team that help & care for elder people with various and complex
needs. People who need the help from more than one kind of professional, these team give
respond to them. I have experienced that the company have developed fast in various parts over
the last 30 years. In order to adopt the teams from different professions, mental health was
considered first among the professions. I believe that in Carers trust 4all, hiring reliable workers
that truly understand individual's needs is key to offering the most valuable support to services
users. They bring together the appropriate professionals persons & practitioners who are seen as
an effective means to motivate better coordination of their work (Cameron, Lart, and Coomber,
2014).
The care they provide is always seen as tailored to suit the needs of the individuals.
Furthermore, multidisciplinary teams in Carers trust 4all convey many benefits to both the
patients & health care professionals (Multi - disciplinary team, 2018). Therefore, these include
better health results & enhanced satisfaction for services users and also the more effective use of
available resources. For mental health, multidisciplinary team is consist of clinical nurses,
medical secretaries and some times other disciplines like; counsellors, art therapists, care
workers etc. While working with them, one thing I understood was that they are very effective as
a team in delivering care to patients which leads to quick and positive impact on their safety.
Apart from this, they offer a promise to bring improvement in clinical care because they can
modify, aggregate, combine and implement a greater amount of knowledge. They do this, so that
they can make decisions, solve issues, create ideas, and execute tasks more efficiently. They are
effective because :

They improve quality of service for better results and understanding of patient's
characteristics.
They collect good data, both for benefits of the client and for the purpose of audit and
research (Glasby, 2017).
They offer opportunities to patients to enter into high quality clinical trials. Continuity of care was being noticed even when varied aspects of care are delivered by
different providers.
Legislations and policies
Carers trust 4all is registered and governed by Care Quality Commission as regulated
care is delivered by them. It has been determined that there are range of policies and legislative
developments that have contributed to high use of multidisciplinary teams (Glasby and
Dickinson, 2014). Their policies aim to reduce the number of separate social work & nursing
evaluation for elder adults, which showed that services to older people must be provided by
multidisciplinary teams. As in Carers trust 4all, multidisciplinary working has increased in
area such as mental health, the sharp differentiation between professional roles has blurred. In
this context, the Mental Health act 2007 launched the approved mental health professional
including the practitioner's group who can undertake various functions which were previously
performed by the approved social worker (Morgan, Entwistle and Watt, 2017).
Health and Safety Policy
They pursue to maintain healthy working conditions and provide safe equipments for all
workers, and volunteers at workplace.
Employees participation is actively encouraged in the maintenance of a safe work
environment.
B. Key issues in relation to service delivery, organisational structure, communication, or
employee engagement.
I believe that Carers trust 4all provide every service user with good health and well being
and they also offer practical as well as emotional support. While working there for 70 hours as a
social worker, I have realised that the company is socially responsible in all their workings,
which they perform with integrity and respect carers, volunteers, patients and visitors. They work
with individuals and families with around 4500 people and employed 300 trained professionals.
All ages of people are provided care services by these professionals. Carers trust 4all Vision, is

to make every carer recognisable, supported and they also want them to main their own well
being as they believe that the employees and volunteers are most valuable resources. The
organisation is considered as a registered charity with over 25 years experience and offer high
quality services assisting carers and their patients (Downey, van der Werff and Plaut, 2015).
They help young people, either emotionally or physically, with disability, illness, mental
illness or any substance misuse problem. The sessions that are conducted for patients such as
interacting with care provider, allows organisation to check that how carer is learning through
this, how well it is going on and how to make more changes. I discovered that, these sessions
minimises social isolation, raise confidence, communications skills, and can change patient's
view on life. Increase in healthcare costs, raising patient's expectations and national efforts to
ensure access to care & development of quality of health care, are all such factors that are
contributing to the need for change in the delivery system.
Carers trust 4all management structure
A local Board of Trustees who are all work as volunteers, regulate this company. With
them, they bring a range of skills and new experiences. Furthermore, the entire operations of the
service are responsibility of them. Apart from this, as discussed above that it is governed by Care
Quality Commission (Linder, Mueller and Freeman, 2017). Many structural changes have taken
place in this organisation that has been used as a remedy for all manner of deficiencies like poor
strategic attention, lack of accountability, lack of efficiency and many other issues.
The Delivery of Care and Support
Referrals from various sources are being received by this company,. Some of them are
Health and social care professionals, third sector institutions, and social services. A well trained
care planner, expert risk assessment & also care planning will go out together to visit the carer as
well as for whom they care for. They do this to discuss the services that they would like to get
and from which they would like to gain knowledge from. After this, a person centred plan is
prepared by care planner including services that need to be provided.
Employees engagement
All of us in Carers trust 4all have closely worked with our clients to build a strong image
of the company, to tailor the needs of those working in this organisation and to provide efficient
and effective services to care receivers. Since I have started working here, I noticed that there
are hundreds of employees across various departments who facilitate open communication

among all members. Furthermore, to improve employees engagement and stability in patient
care, they keep all staff members up to date. In this organisation, duties of all carers are heavier
than duties of other workforce employee (Bailey, Madden and Currie, 2015). We all have to
deal with more urgent, sensitive and personal challenges than of regular activities. For this,
Carers trust 4all's managers provide us with tools and techniques by which we can streamline our
daily work, so that we can concentrate on what matters the most (Collini, Guidroz and Perez,
2015). It is important that employee goals must align with organisation to reinforce how their
performance assist the larger mission, vision and values. If this is not done, then organisation
may fail to achieve its goals.
Employees in Carers trust 4all :
Makes eye contact with their patients, genuinely take care, and welcome them.
We protect them and support them to reach their destination.
We all put patients as our first priority.
In Carers trust 4all, I have been provided with a Hand book of a company explaining
their values, management structure, delivery of care and support, several policies and personal &
administrative policies.
Rules that I need to follow in this organisation are as follows :-
I need to carry my identification badge with myself at all time during working hours.
As a social worker, who cares for patients without being paid, this is the reason I am not
allowed to take any kind of expensive gifts or money from clients. Yet, if I accept them
then I have to inform it to my line manager.
To provide high quality services, they have introduced me their Compliment and
complaint policy through which they can better their services.
My Time-sheets:- My time-sheet arrival at Carers trust 4all location office is 12 noon on
Monday. Apart from this, the sheet was printed with basic details such as; the date, start
and finish time of my visit (Anthony‐McMann, Ellinger and Halbesleben, 2017).
Other details :
Training :- They maintain records of all type of training that I will receive. I have to
attend all relevant training sessions and these will be subject to regular review in
supervision. They will me with an individual learning & development plan from selection
onwards.

Terms and conditions :- I am going to receive an employment contract stating all terms
and conditions within two months of joining at Carers trust 4all.
Code of conduct :- they asked me to read carefully and sign the code of conduct before
commencing work.
All these things discussed above, demonstrate that what professional behaviour is
expected from me when I will work with them as a social worker. I followed all these policies,
terms & conditions express my dedication towards work.
CONCLUSION
From this report, it has been concluded that, Carers trust 4all is a care service providing
company which is the trading name of Crossroads Care Cheshire, Manchester and Merseyside
Ltd. Apart from this it also stated that, in this company there is a group that consists of health
care workers who are considered as the member of different disciplines, for eg. Psychiatrists,
social carers etc. are known as multidisciplinary team. They serve to fulfil the needs of the
individuals, moreover they are able to offer an emotional support often to those they care for.
