INTRODUCTION
Social Anxiety Disorder (SAT) is a social disorder due to which there is a fear in facing
any kind of social situation by the person. Due to this, distress and discomfort is caused. In this
situation the person suffering from this phobia, feels that they are being negatively evaluated or
scrutinized by the other person. Its symptoms include sweating, nausea, stammering, blushing,
etc.
In this report, the case of Bonnie, Mr. Cross and Daniel is being discussed and plan for
treatment and suggestions are being provided on the basis of various issues and sufferings they
are facing. And also, the ways or plan they can adapt to overcome the same situations.
ASSESSMENT 1
Question 1 Participation in the Meeting
In the case of Bonnie a 15-year-old girl who is suffering from social phobia does not
make her first move each time due to anxiety and shyness. This satiation is due to bullying at her
school. She does not attend camp with her father, she avoids social gatherings due to fear of
meeting people, as she thinks, she will be judged and evaluated(Au and et.al., 2017). Also, does
not speak in class and even to her teachers as she feels the situation is fearful and scary if she
goes shopping, eating, and in result she has problem swallowing, dinginess, etc.
In the case meeting I would like to call Bonnie, Her parents Bill and Zelda and her
friends. It will be necessary for better treatment of Bonnie, as collecting information from the
parents of her behaviour at home and at public gathering and her way of stimulation to various
situations at outdoors, school, with friends will be required to treat her in a better way and help in
overcoming the fears.
Question 2 Ensuring all the Services are Aware of the Meetings
Bonnie needs a treatment which has a positive effect on her mentally as well as
physically. This can be done by providing various services which will improve her conditions,
and are also discussed in meeting, which are as follows:
The services which should be provided to her to help her in overcoming the anxiety can
be Stimulation of Thoughts which are Positive in Nature before she goes out with
friends and family. This can be done by making her listening to the music she likes,
playing games, etc. so that she gets relaxed before moving out(Watkins and et.al., 2018).

This will be beneficial for her as well as her friends and family, as fast recovery will be
done according to the efforts made by everyone.
Also, Avoiding Pills and Narcotics will help in reducing the future problems, as relying
on them for every situation will be disastrous in the future. Therefore, depending on
chemicals for reducing fear and anxiety should be avoided. This will help help Bonnie in
overcoming the anxiety and fear and recover soon.
Question 3 Discussion at the Meeting
At the meeting, the topics covered will be Bonnie's anxiety, fear , situations which makes
her uncomfortable and insecure, her reaction to the stress caused by bullying at school and
comments of other people, the observation by her friends and family(Mackrill, Ebsen and
Antczak, 2015). This will help me, as a consultant, in finding out the problems and issues with
Bonnie her, way of thinking and tackling the problems. All this information is necessarily
gathered to provide solutions to her problems and create a positive perspective for her in each
situation. She will be diagnosed on the basis of:
The situations which creates anxiety for her.
Impact of anxiety in daily activities.
Persistent fear which may arise in attending social gathering, in which she believes that
she will be evaluated and which will be shameful and embarrassing for her(Byrne, Jolley
and Peters, 2018).
Dealing with the anxiety in social situations.
Question 4 Goals to be Achieved
The ultimate goals after the diagnosis will be providing a treatment which is encouraging
for Bonnie as well as her parents. It will start with educating and training the parent to adopt a
behaviour which is positive in nature. And counselling of both Bonnie and her parents, setting
targets such as self-safer behaviour in school at social gatherings, better work at school, making
new friends and improving relationships with all, etc. For further progress, asking for behaviour
reports from the class teacher, preparing a checklist for behaviour. And the outcomes of the
same should be realistic, observed properly, as all these activities will help her to reach the goal
set accordingly.

Question 5 Participation of Bonnie in the Meeting
According to the situation of Bonnie, it is necessary for her to attend the meeting and be a
part of it. As it is for her improvement, and she must also know the problems and issues coming
in her treatment. If she will participate in the meeting, it will be easy for setting goals and targets,
as she is the only one who has to put efforts in achieving them. For building a positive and
healthy future, she also has to contribute by taking therapies for behaviour, its education, which
though is a complex process of medication, but she has to be patient all through this treatment.
Therefore, for getting better information from Bonnie and treat her in a effective way her
presence is necessary in the meeting.
Question 6 Case Plan for Bonnie
Objectives for Planning: As the level of severity is mild in the case of Bonnie
Therefore, the first step will be of doing CBT which is a model for observing the various issues
and problems with their prevention.
CBT Model: CBT is an anxiety model which includes three components in it which are: Cognitive: Fear of being noticed, evaluated in public gatherings, which results in
embarrassment(Lawrence, 2017). Psychological: Increasing dizziness, heart rate, panic, stammering, etc.
Behavioural: Avoiding attending the social get together, and outings and even attending
schools.
All of the above problems are being faced by Bonnie, which can be overcomed by taking
continuous sessions through application of the above stated model accordingly.
Outcomes: All this can be prevented by Sessions which Boosts the Morale, teaching the
way of Co-ordinating with people, creating Awareness and establishing Positivity by self-talks,
Monitoring the Anxiety by own, giving Therapies for Relaxing, etc. which will help in improving
the health issues of Bonnie(Johnson, 2017).
Question 7 Monitoring and Evaluation
The case of Bonnie will be monitored on periodic basis, through the record of reports
kept by teacher, parents and the consultants. Each activity and effort made by Bonnie for
improvement and reaching the goal will be noted in the report and will be analysed and monitor
by regular visits at Bonnie's school and home. Also, her behaviour at the outings with friends

and family and the level of positivity attained by her will also be reviewed. Then further steps
that should be taken for more improvement will be taken.
Question 8 Approaches to Delivery of Services Strengths-based Approaches: It basically focuses on the "Work Practice theory" which
target on the self-determination, positiveness, strengths of individual. In this process open
communication takes place between the counsellor and the person. And assembling of
personal, emotional information along with the capacity and strengths of person is done
by the consultant. Right-based Approaches: It is a positive approach towards the development of the
patient, so that a transformation takes place. It also helps in addressing the contingent
emergency situation that may arise and should be overcomed through delivery of service. Person-based Approaches: The development of behaviour of patients to deal with
variety of situations is the main aim of this approach. This can be done by keeping in
mind the perspective and situation of the patient and the way they adopt for using the
same in different situations. This development process can be done by the consultant
through quality research and acquiring in-depth information of the patient.
Needs-based Approaches: It is traditional approach which focuses on finding out the
ultimate needs of the patients on the basis of characteristics, behaviour, status,
overcoming fear and anxiety, etc., through proper analysis and study and then solving the
problem by taking appropriate decision in that context.

Question 1 Allocation of Task to a Case Worker
A case worker is a person employed for providing social and care services to the person
who are troubled and in need of help by the government or any institution. Experience: The experience has to be kept in mind while appointment of the worker, as
he must have the knowledge to deal with different types of people and their situations,
which will be easy for him to reach to the solutions to the issues and problems. Work Load: Handling case is not an easy task, it requires a lot of hard work, confidence,
patience, and knowledge. Knowing the case and acquiring full information about the
person takes time and effort(Byrne, Jolley and Peters, 2018). Therefore, the case given to

him should not create any sort of load as it will directly have a negative impact on the
effectiveness and efficiency of work.
Geographical Location: Location also plays an important role as the background of the
worker can be taken into concern, a worker's perception, language any other sort of
geographical barriers that can can arise should also be taken into concern. And the way of
treating and dealing with different situations can also differ if the location from which the
worker belong is not the same as of patient.
Question 2 Overview of Case Plan
The case can assessed through focusing on the problems of depression of Mr. Cross, as
due to loneliness he has gone under depression. Also, he is suffering from diabetes and heart
problems which is a serious issue of concern. If his Depression is not treated his health can
become more severe. By providing help through learning about his conditions, making him do
good physical exercise, eat healthy food and treating him with therapies which provides positive
mental and psychological health. This can help in improvement in the condition of Mr. Cross,
which can assessed through regular monitoring and evaluation process.
Question 3 Steps taken for Development
By providing health care services needed by Mr. Cross to overcome his health and mental
issues, which will help in improvement in his condition and will raise a urge to live with
positivity and lively.
Through a process of proper assessment, planning about the services needed and by
personal communication with the client, the provision will be made for providing various
resources and services to improve the health both mental and physical(Gerhards and Siemer,
2016). And reach to the solutions which are both qualitative as well as effective in cost.
Communication will be done personally so that there is no variance in the information.
After getting the information required from others, personal communication with Mr. Cross will
be required for gaining overall information, his perspective towards the issues and the steps for
overcoming the same, so that there is no confusion.
Question 4 Goals and their Evaluation
The ultimate goal is to remove the loneliness of Mr. Cross, which will help in curing
other sufferings directly. This will be a short-term goal which will be achieved through
participating in social gatherings, meeting with peoples, talking and living with his son regular

basis, watching T.V., doing activities which he enjoys and used to do earlier, etc. This can be
achieved in a time frame of approximately 6 months to 1 year. This can be monitored by the
worker himself and evaluation can be done on the basis of set targets and goals and their
achievement at the end of the day(Mills and et.al., 2017). In addition, culture and traditions
should also be kept in mind as throughout the process as they may vary in every situation.
Question 5 Feedback for Progress
Feedback is necessary, as they help in finding out the barriers and problems arising in
successful completion of a task. So that, proper steps can be taken for improvement. This can be
done with the help of review process, through record keeping by self and with the client, so that
progress is attained.

Question 1 Factors Considered in Context of Case Worker
A case worker provides social and care services to the person who are needy, and are appointed
by the government or any institution. Experience: The experience is necessary, as the worker must have the knowledge to deal
with different types of people and their situations and provide appropriate solutions. Work Load: Handling case requires a lot of hard work, patience, and wide knowledge.
Acquiring full information about the person takes time and effort(Johnson, 2017). Hence,
the work load must not be present.
Geographical Location: The background and location of the worker has to be taken into
concern to know about his perception, language and any geographical barriers that can
can arise while handling the case(DeNard, Garcia and Circo, 2017).
Question 2 Case Plan
Daniel needs to be rehabilitated and treated carefully, as he may be suffering from social
anxiety due to which he started alcohol and drug consumption, and remain away from friends
and family. A plan needed to be framed which will be monitored on the basis of activities and
training will be provided for behaviour management, along with medication and also educated,
and the targets set should have realistic outcomes that is the goals set should be achieved in
order to gain improvement by Daniel.

Question 3 Appropriate Actions Taken for Progress
Daniel will be provided a treatment which will include counselling by psychologist,
behavioural therapies, detoxification along with medical and mental health stability(Srivastava
and Pradhan, 2018).
Also, through motivating him to live positive life and recovery support from family as
well as community. This will help in speedy recovery of Daniel.
Through proper and personal communication with him as well his family will result in
acquiring full and appropriate information necessary for further decision making and minimising
ambiguities.
Question 4 Overview of Goals and their Achievement
The treatment will be a long-term process depending on the severity and need of Daniel.
The targeted goals should be acquired within a time frame of approximately 2 to 3 years. The
goal is to accomplish the process of rehabilitation and improving the mental and physical health
through medication and application of various theories for changing the thought process,
behaviour, etc(Saraf and Newton, 2017). They will be monitored and evaluated by keeping the
records and maintain reports of the various activities and positive contribution of Daniel in the
same. This will further help in taking next step towards improvement.
Question 5 Feedback For Problem Solving
Feedback will help in finding out the problems and the bottlenecks arising in successful
completion of rehabilitation process(Ford and et.al., 2018). This can be done with the help of
review and evaluation of process, and appropriate steps can be taken for development.
CONCLUSION
It is concluded from the above report that the needs and development of a person requires
a good case worker and consultant, who can promote the health and other related issues, by
providing with appropriate solutions. Also, it is studied that, through various steps and making
successful plan which includes, collection of information, resources, and the aids needed are
being provided efficiently for full and speedy recovery of a person and overall development.
