Introduction:
Case management is known as the assessment process which is performed collaboratively,
facilitation, planning, care coordination, advocacy and evaluation for the services and options
which will help to meet the family and comprehensive individual health which needs by the
company by the help of communication between the members of the company. The resources
which are promoted for the safety of patient, care quality and cost of production are taken care in
case management.
In this study, the case management model used is brokerage case management model. This type
of model is a very small and brief method towards the management of case in the company. The
case which is attempted by the workers to help the client need to be identified by the demands of
the broker which supports the services in more than one contact.

one or two contacts created by the members of the company or project on which workers are
working so that they can achieve their desired results without facing any kind of problem
(Dieterich, et. al., 2017). The brokerage model is really helpful for the client to understand
themselves about their needs and desires which they think are important when they receive any
kind of services from the company. They also know that they are available when they needed
and also get to know about the procedure of handling them. The biggest challenge for the client
while handling this model is that they face problem in accessing services provided to them in this
model and they also have the issue on their availability of the services which is not easy task to
handle. In this model, the manager of the case gives very less services which are provided to
straight away to the client. Instead this model helps in creating a link between community
resources and client. It also focuses on the assessment of needs and wants of the clients so that
they can plan for the strategies of the service and also helps in connecting customers or clients
with this model.
The brokerage model may give less attention or focus on over viewing and measuring the
outcomes or results which they have to face while working with this model. The company roo
the organisation gives the services related to case management so that they can manage their case
for brokerage. It might be the coordination with the collection of data and assessment all over the
people who are providing services to the people or clients so that they can feel tension free in

The empowerment-orientation has been centrally formulated for the rehabilitation and also
performs the social service practices and also case management activities. A core community
service support gives people with the disabilities which are psychiatric in nature. The study of
empowering the community done by the practices performed by the help of case management so
that clients of customers can keep their mental health happy and clean. It is very important for
the company to understand what kind of case management model will be suitable for their
company so that they can achieve their desired goals and objectives in a very effective and
efficient manner without facing any kind of problems or issues in performing the activities. To
understand the nature and culture of empowerment-orientation company members have to
conduct interview so that they can understand what their employees think about the company and
whether they like the working environment of the company or not (Weske, 2012). They also get
to know whether there is any problem in working within the company and if any problems have
been found in the company than what kind of solutions are found by the company related to the
problems so that they can resolve the problem in a very effective and efficient manner.
