Case managers and clients need to review progress and required to make changes in
the plan of case management as and when needed. Case managers are able to see progress but
changes can be gradually and slowly made in the process which can be frustrating for both
worker and tenant. Also, reviewing the service plan is for the purpose of evaluating the
progress of tenant towards meeting objectives and goals. In case management plan, it is
essential to implement strategies for monitoring effectiveness based on relevant services,
agreed goals and programs to stakeholder and client satisfaction (Lukersmith, Millington, &
Salvador-Carulla, 2016). It is reviewed to support by checking on achievements and
exploring ways for overcoming barriers if arises along with changing regularly to reflect
changing goals, achievements, or new priorities.
However, it is informal to monitor and daily client and workers contact part where
client note the changes and workers responds to it. Also, it is formal review process essential
for progress checking as made on support plan. This consists of deciding reviewing or
monitoring frequency, tool development for reviewing or monitoring, and written procedures
and policy development along with providing staff training (DHS, 2011) (DHS, 2011). Thus,
if no progress made within the support plan while working than there is no need to stretch the
situation. Or else, review arrangement is done for looking at the things if not achieved and
make changes where necessary in the support plan. Thus, case managers, workers and client
can review progress and make changes in the case management plan as suited for preceding
the working of managers with clients effectively.
