Analysis of Different Aspects to be followed for Development of Appropriate Case
Management Plan for Clients
As commented by Mobtaker et al. (2018), the case management planning is
considered to be the process which is mainly focused on the identification of the needs of
clients, clarifying different goals along with setting priorities and necessary steps for
achieving the same. The steps which are required to be followed for development of
appropriate case management plan are as follows:
Intake- It is the initial meeting among the new client and case manager wherein case
manager utilises the time for gathering information related to demographics regarding client
and identification of the immediate needs. It helps in building trust and maintaining effective
relationships.
Needs Assessment- It is the second aspect which needs to be followed and it is built
on information which is collected during the stage of intake. In the respective process, the
depth is needed to be provided on individual challenges as well as goals of the clients
(Mobtaker et al., 2018). The primary objective of case manager is identifying interests,
problems and risks to success.
Service Related Planning- It is the third aspect which needs to be followed as it is
essential for deciding the success or failure of client. The case manager needs to establish the
specific goals along with actions which will be taken for meeting goals (Nunhes, Barbosa &
de Oliveira, 2017). The process of goal-setting is case plan which is inclusive of outputs
which will be measuring success of clients.
Monitoring as well as Evaluation- It is the last stage as it is the critical element for
analysing the impact specific kind of programs along with services that can have on client.
With usage of output along with outcome, the case manager tries to establish and evaluate the

ensure the success of client and it is helpful for improving the effectiveness.
