INTRODUCTION
Health Care and Social Services can be defined as the treatment of illness whether mental
or physical in hospitals, health centres and in the community (Health & social care, 2018). This
assignment is based on a person named as Mr. Jonathan Tertullien whose some of the needs were
unmet because of his some illness. It will provide a knowledge about the needs that remain
unmet, who can help Mr. Jonathan to fulfil these needs. Further it will also focus on the how the
integration of social and health care services can be helpful for an individual (Aveyard, 2014).
MAIN BODY
Task 1) Identifying Unmet Needs
In the following case study provided, Mr. Jonathan Tertullien is suffering from various
diseases which are creating a severe effect on his health. He is suffering from diseases such as
Vascular Dementia, heart problem and high blood pressure. Due to these problems the unmet
needs of Mr. Jonathan Tertullien is that he needs a caretaker so that he may take care of all his
needs such as providing time to time medication, making food for him and doing shopping for
him on his behalf (Badzek, and et. al., 2013).
Another unmet need is the need was of the food. As his neighbour was also in the
hospital due to some illness so there was no one to help Mr. Jonathan Tertullien in making food
which also is a basic needs than needs to be fulfilled. Non fulfilment of this needs also leads to
create a bad effect on the health of Mr. Jonathan Tertullien.
Task 2) Outlining Health Care and Social Care Service
Mr. Jonathan Tertullien needs some special services to be provided from the health care
service which will help him in recovering from his disease. In the health care service, he could
be provided with proper medication related to the diseases of vascular dementia, high blood
pressure and heart problems. This facility can be provided to him by the hospitals.
Another service that can be provided to Mr. Jonathan Tertullien as the social care service
is a family environment and care takers so that he may feel good and not alone. These services
can be provided to him by the social care service providers which will help him such as in giving
medicines of diseases from time to time, the family environment will remove his loneliness and
interaction with other people will also help a lot in recovering from him from the disease.

These services will are needful and important to be provided to Mr. Jonathan Tertullien
from the making improvement in his health and are appropriate according to the following case.
Task 3) Integration of Health and Social Care Services
These services which are provided to Mr. Jonathan Tertullien by the health care and
social care service providers can work in an integrated way which will help in the upliftment of
health of Mr. Jonathan Tertullien. As the caretaker can provide him medicines from time to time,
can also help him by preparing and providing food to him which will help him in his being
physically well.
And for making him mentally well, the family environment provided to him will work a
lot. It will help in making him in overcoming his illness as this environment will totally remove
the feeling of loneliness from him (McKee, 2013). The interaction with others also make transfer
of feelings, ideas as they may remember him daily about taking medicines which will be an
addition to the medicines for recovering the illness of Vascular Dementia. This environment and
proper treatment will not only improve his physical condition but will also improve his mental
condition as well. In this way the interaction of these services can work together and can help in
the improvement in the health of Mr. Jonathan Tertullien.
Task 4) Need to be Remain Unmet
The need which will remain unmet is the family need. The caretaker can only do his
work such as giving medicines on time and making food for him but he is not as same as his own
family (Skinner, Hanlon and Halseth, 2012). The care and affection which is shown by the
family members cannot be expressed and expected from the care taker. The fulfilment of this
need will contribute towards the improvement in not only in the physical condition but also in
the mental condition. This need of Mr. Jonathan Tertullien will remain unmet.
CONCLUSION
From the above given data it can be concluded that the basic needs of an individual
should be fulfilled at the initial level so as to avoid him from any kind of illness or disease. The
improvement in the health can be made with the help of services provided by health and social
care. With the fulfilment of the needs of an individual, his mental and physical condition can be
easily cured.
