Use this template to document the implementation of your program including
monitoring and evaluation processes.
1. List the roles and responsibilities of the stakeholders involved in your program

The stakeholders involved in the program delivery of the alcohol and other drugs
intake control program were the alcohol and the other drugs counsellors, the clinical
psychologists, the community general practitioners, the community health nurses, the
community workers.
Their list of roles and responsibilities are –
1Alcohol and other drug counsellors – Control and check the alcohol intake by the
community members
2.Clinical psychologists – manage the levels of stress and anxiety leading to
psychosocial disorders.
3.Community general practitioners – Pharmacological management of the cases,
prescribing the medications.
4.Community health nurses – Help to administer the medications and assist the
counsellors in the counselling process.
Presentations and meetings are the strategies that would be used to communicate
with the stakeholders.
2. List the program and service delivery documentation that you were responsible
for.
The ‘alcohol and other drugs intake control program’, I am working as a health care
manager who would managing all the workforces involved in the care processes
delivered to the public in the community setting. My role is to look after the
synchronisation of the health care and social care processes being delivered to the
community members in a pertinent manner (Hall and Christian 2017).
3. Describe the steps you took to establish the training needs for your workers in
your program and the strategies (e.g. training programs) you took to address
any skills gaps.
There are certain steps I took to establish the training needs for your workers in your
program are staff development sessions, the skill development sessions, the cultural
competence training programs and I took individual feedback from the stakeholders in
order to deliver and create the right training programs that is pertinent to address the

needs of the staffs.
The cultural competence training was undertaken to address the cultural awareness
gaps of the stakeholders and the cultural competency gaps that was affecting the
communication and rapport building of the stakeholders with the community clients.
The interpersonal skills that are required to be developed to address the collaboration
gaps between the multidisciplinary staffs, were also intervened with in the training
programs.
4. Service delivery can be monitored in a variety of ways. Important questions to
consider are:
Are we doing what we said we were going to do?
Is it having the effect we planned?
Are we achieving the outcomes we are being funded to deliver?
Is our spending on target?
Are we keeping within agreed time lines?
Are the users of the service happy with what we are doing?
Provide a summary of your monitoring process demonstrating how you
monitored the program over at least 3 months. You may use the framework
shown below or your own organisation template.

The objectives of the program is to prevent the increasing rate of alcohol and other
drug adherence or addiction behaviours amongst the aboriginal community people.
The aim is to develop awareness and knowledge in the community members in order
to prevent and manage the alcohol consumption behaviours amongst the community
members.
The aim is to develop a counselling process to address the mental health problems
and needs of the clients.
The aim to develop a self-sustainable system in the community framework that
prevent alcohol and other drugs consumption.
Performance indicators (targets):
The performance indicators are better prognosis rate, bettered state of cognition,
diminished levels of stress and anxiety in the clients,
Results (i.e. did you meet the performance targets?):
Yes the objectives of the community health promotion program was finally met on an
average note a there was lot of barrier and challenges to go with the implementation
process.

5. Explain whether your program met the agreed objectives and identify any
capacity issues that may have influenced the result. You should provide any
relevant data to support your answer.
The capacity issues that may have influenced the result were the workforce ratio that
is the number of counsellors were way more than the number of general practitioners
and the nurses and this created a gap in the collaboration and management of the
client in a proportionate basis.
6. Describe at least 1 modification or change that you had to make to your program
as a result of changes in policy or budgetary frameworks.
The recruitment of the community workers in the health promotion program was
decreased an had to be limited due to the changes in policy or budgetary frameworks.
7. Provide at least 2 examples of feedback by a service user or stakeholder
involved in your program. Describe how this feedback was obtained and explain
how you addressed their issue in accordance with organisation procedures.
The feedback regarding satisfaction of the clients were received through a survey and
various feedback pertaining to dissatisfaction were handled by applying cultural
competence in the care framework.
The audits returned feedback pertaining to collaboration issues between the staffs
which were managed by applying the interpersonal skills development programs.
8. Describe at least two (2) changes or modifications you made to your program in
response to servicer user or stakeholder feedback.
The two modification were making the team more culturally competent and by
recruiting more effective staffs with better interpersonal skills.
