a major role in serving the society people under the thorough-based programs. Among the
reputed healthcare community service departments, Kentucky Departments for Public Health and
Medicaid Services is a clearly mentionable name who dedicate their aiding features to the
community people. The organization has considerable processes to resolve diabetic, heart
disease, stroke and immunization problems of their respective patients. The community always
seeks for improvement in their sessions. Details show the introduction of new check-ups to blood
sugar, influenza, foot exams and vaccinations of pneumonia has been highly important to the
expansive nature of the community in the honorable society.
During the study of the respective community health organization, the department was
found to lack appropriateness of certified programs to the chronic diseases. According to the
research material by Wallerstein and Duran (2015), chronic diseases has grown its impacts in
contemporary decades, while affect to the society has been calculated to be nearly 50% of adults
in US (2016). The diseases to chronic disorder include arthritis, asthma, hypertension and others.
Though, the effect is not directly life-threatening, yet it receives enough significance due to the
unwanted burden to quality of life of healthy people. Thus, the absence of such program is
eminently important to be addressed and resolving the issues through exact implementation of
chronic disease program will create betterment and opportunities to both the organization and
society’s limits. Moreover, the company has been productively responsible enough to acquire
beneficial and visible opportunities and implement such programs for the welfare of beings.

The healthcare program is chronic-disease based, thus called as Chronic Disease Self-
Management Program. The self-management program will be subjected to assist patients in
management of their own discipline and behavior to exactness of decision making, in their
lifestyle. Adaptation of health-promoting behaviors and educating patients are the prime
strategies of the community services programs, as well as of the respective service community
(KDPHMS). Following are the characteristics of the program:
Sorting patients to similarity of chronic diseases for enhancement of discussion to the
disease-connected tasks and health-improvement behaviors.
Improvement of patient’s responsibility with the implementation of daily monitoring of
the involved diseases.
Inexpensiveness and greater promotion of the activities to expand the idea of self-
management’s effectiveness.
Further, patient’s overall health and quality of life would be maintained with less utilization of
healthcare resources (including emergency care and physician visits). This also promotes the
strategies of the community services. Also, the issues to the program also needs to be addressed.
Conclusively, a 2014 study(Goodman, Bunell and Posner, 2014) accessed self-
management outcomes of a program in a reputed community department(National Center for
Healthy Housing in Columbia, Maryland), which has a significant improvement of 90% patients
lifestyle and health-records (790 out of 860), after a year program had been completed.
Moreover, the whole project would be targeted to meet WHO’s standards of “sustainability of
social, environmental and economic resources for well-being of people to promote aspirations
and enhance the satisfaction of unique environmental needs” (Sanders, 2014).
