Introduction
This project report elaborates about the chronic illness management workshop in which the
discussion and awareness programs will be organized with regard to how to deal with chronic
diseases.
Chronic diseases are the diseases which persist for a longer time period. U.S. National Center for
Health solutions defined chronic disease as the disease that lasts for three months or more. The
prevention of such disease is not possible by vaccines or its medication is not possible, nor do
they disappear. With the passage of age, these diseases become more common. It includes
arthritis, heart attacks, strokes, breast cancer, diabetes, colon cancer, obesity, oral health
problems, etc. At least one chronic health conditions occur at the age of 65 in 88% or Americans.

The workshop will focus on chronic illness self-management education programs. Chronic
diseases have become the most common problem at present and are being faced by many people.
It includes some diseases like heart attack, cancer, asthma, diabetes which are a very common
chronic disease. Somewhere the reasons behind these diseases are frustration, stress, the lifestyle
of people. Frustration refers to the emotion or the feeling of being less confidence or annoying
because the people cannot achieve what they want to or expect in return of the action and efforts
which are applied by them. Chronic disease self-management refers to enhancing the health
behavior which can be adopted by the person with chronic illness. These programs are designed
either for improving the current behaviors or for teaching new behavior (Schulman‐Green.et.al,
2012).
Goals of the workshop –
The main goal of the workshop is to provide ideas and insights which will help the people
in enhancing their knowledge about chronic diseases.
To provide patients and other people with the information and techniques and teaching
skills for improving the self-care and interaction of doctor and patients with the main
motive of improving their living style
To make people learn about some tips which will help them in coping with frustrated
feeling and creations of options to relieve them
To give understanding to the people with regard to the importance of self-management of
chronic diseases (Novak.et.al, 2013)

aspects can be tailored. After the workshop self-assessment activity will be organized, after
which the online learning, discussion forum, and follow up will be made. The program will run
for 12 months after registration for the workshop.
In self-assessment, an online survey will be conducted which will help in identifying the current
knowledge and the areas where improvements are required. Suggested online learning suite will
be organized at the time and pace of registered people. Discussion forum addressing the issues of
chronical diseases covering the thoughts and ideas of the people and providing them with the
suggestions, proper counseling will be done with them (Rosland.et.al, 2012).
The description will be made to the participants with respect to the concept of patient self-
management and how the health outcomes are improved by this. A systematic evidence approach
will be implemented with respect to care coordination, care planning. Health care professionals
will be invited to the program and their self-management activities for the chronic illness will
