In healthcare services there are many of the gap which can be identifies in proper
delivering of healthcare. There are some of the gap which are like effective communication gap
which is highly effective while delivering healthcare services during development project. It
consist of delivering of healthcare services that includes person-centred care and evidence based
practices. In this annotated bibliography there is discussion about the clinical gap which are
present in the practice development project. This consist of effective communication gap,
decision making and management process gap which can highly effective in delivering of
effective care in the practice development project.
