AI Summary
This nursing assignment presents a clinical reflection on various patient care scenarios encountered during the student's practice. The report covers four key weeks, each focusing on different learning objectives. Week 2 discusses the identification of cardiovascular and peripheral vascular diseases, emphasizing the importance of accurate pulse recording and the application of ethical principles. Week 4 centers on the management of acute exacerbations of chronic respiratory conditions, specifically the administration of oxygen via Venturi masks and the importance of adjusting oxygen concentration based on patient needs. Week 10 addresses the assessment and management of patients receiving blood products, electrolytes, and fluids, highlighting the significance of safe and competent nursing practice and effective communication. Finally, Week 11 focuses on the nursing assessment of patients admitted to the CPU, including a case study of a COPD patient experiencing respiratory distress, and the importance of rapid response and multidisciplinary collaboration. The assignment emphasizes the application of evidence-based practices, ethical considerations, and the importance of safe and competent nursing care in all scenarios.

Situation For this week, I was to about to learn how to properly identify and
accurately interpret the normal rhythms and arrhythmias of the heart.
This will follow accurate recording of pulse via proper localization of the
pulse beat. It is crucial to analyze the cardinal symptoms of any adverse
physiological functioning and hence the pulse beat is important.
Moreover, brachial artery is the choicest anatomical location for
detecting the pulse.
Action I was trying to locate the pulse beat of a ward boy from his brachial
artery of right hand in a standing posture. Suddenly a Registered Nurse
came in and asked me to record the pulse from the left hand informed me
that supine position is most preferable. Heart being situated on the left

Situation CPU for this week encompasses proper administration of oxygen via
Venturi mask. External supply of oxygen is a prime need to asthma and
COPD patient and hence this week action plan centres around the oxygen
therapy to ensure proper breathing. Venturi masks are considered as
high-flow oxygen therapy devices that deliver oxygen at the required
concentration for the patients who are suffering from the breathing
problem arising out of COPD or acute asthma (Maggiore et al., 2014).
Action In my clinical encounter, I administered Venturi mask on the patient who
is suffering from acute asthma to provide relieve his respiratory distress.
I instantly switched on the oxygen supply at a concentration of 50% with
a flow rate of 10L/min. The registered nurse, who is attending the patient,

asked me to regulate the oxygen concentration at 24% concentration. She
said 50% oxygen concentration is extremely high and may cause
respiratory depression (García et al., 2017). She pointed out to note the
any signs of skin irritations in the patient, as it is common disadvantage
of Venturi mask.
Outcome I gained proper clinical understanding about the conventional procedures,
which are used to provide quality care to the patients with COPD and
acute asthma. I also planned to ensure proper delivery of the oxygen at
permissible concentration for ensuring safety.

spotted that the rate of infusion was not legible, although the rate of
dilution were clearly stated (Barras et al., 2014).

Action I approached the registered nurse without using my own assumption and
asked her to guide me. She quickly reviewed the prescription,
acknowledged by prompt action and sent the prescription to the
authorized medical personnel for proper revise. After rectification of the
prescription from the medical officer, sodium/potassium was
administered via an infusion pump so that infusion rate and volume may
be controlled effectively without any further error (DeCarolis et al.,
2016).
Outcome Nurses are obliged to perform their duties in a safe and competent
manner. Reflecting on this experience, I realize that it is always essential
to communicate with the nurse or other health professionals in the
workplace it will help to intervene the problem promptly and thereby
negating the possibilities of potential fatal threats.

recommended for the installation of the Venturi masks for oxygen
supply as his oxygen saturation rate was poor. When I was about finish
my bedtime round in the unit, I suddenly noticed that the patient who
was suffering from COPD is coughing profusely along with the signs of
laboured pain. A patient of COPD, laboured breathing is a part of the
symptoms, but since he was doing well with external oxygen supply, the
reason might be different (Price et al., 2013).
Action I immediately noted down the blood pressure, and fixed the pulse
oximetry in order to get a constant feed to the heart rate, respiratory rate
and oxygen saturation. Taking into account his severely distorted vital
signs, I placed a Medical Emergency Team (MET) call in order to
prevent further deterioration of the patient’s condition. The Registered
nurse of the MET team noted that the oxygen supply rate through
Venturi masks was not adequate. She immediately shifted him on the
oxygen supply of 24% with oxygen transfer speed of 10 litres per minute
(more than his initial supply of oxygen). After 5 to 10 minutes, his heart
rate gradually came down and he slowly started to breath comfortably
with higher level of oxygen saturation (Frat et al., 2015).
Outcome The observation of the medical proceedings that are undertaken to the
patient who was suffering from COPD gave me a detailed insight on
how things are carried out in CPU unit and the importance of carrying
out nursing assessments especially concerning patients with breathing
problem. In my future professional practice, I will abide by safe and
competent procedures. I also gained knowledge and revealed that how
interactions with the multidisciplinary team can help to fight back
against different complex clinical situation in the CPU unit of the
