TITLE- “Importance of communication, coordination and team work in
overcoming issue of diabetes”.
INTRODUCTION
Diabetes is defined as metabolic disease which is caused by due to high blood sugar level in
body. In human body, there are various gland that used to secrete hormone for the proper body
functioning. The hormone insulin usually move sugar from the blood into the cell and can be
stored or utilise in form of energy. With diabetes, the body is not in condition to produce to
enough insulin in order to arrange the appropriate condition of sugar in body. Untreated high
blood sugar from diabetes can easily damage the nerve and other body organs. In addition to this
diabetes is usually classified in two categories such as diabetes type 1 and type 2 diabetes. As per
this type 1 is called as autoimmune disorder, in this the immune system attacks and destroys
number of cell in the pancreas, however this is the region where insulin prepared (Lee and Doran
2017). In addition, there is a rare condition of diabetes insipidus is not related to diabetes
mellitus. Overall, it has similar name but the condition is varying which is depend on kidney
which remove the fluid from the body. With relation to this, every diabetes has their unique
symptoms which can cause and treatment. Moreover, a diabetic patient need proper care and
appropriate diagnose to treat the condition of diabetes. In this, the communication is required for
care of diabetic patient which is over age and have lots of complication that is related with their
health issue. Proper coordination is required in order to take medication and appropriate test to
check the overall condition of diabetes. Team work is also necessary for caring of diabetic
patient for their routine diet, medication and treatment. In this report, the communication,
coordination and team work is essential to improve the condition of diabetic patient.

RATIONALE OF RESEARCH
The main reason of choosing this research is useful to examine the importance of
communication, coordination and team work in overcoming of diabetes. As per this, it helps to
provide the brief knowledge about diabetes and their management procedure which is usually
taken in order to eradicate the issue and other complication which is associated with it. This
study helps me in both on personal and professional perspective. With the relation to
professional the advantage of this investigation is about proper communication and coordination
which is used in management procedure of diabetes for patient to recovery. In addition to
personal, it helps in improving the skill and formulate new idea and new intervention technique
in order to retaliate the issue of diabetes. Moreover, this provide a development of idea to
conducting the proper research for any topic. It helps me to develop effective personality which
may help health and public health care to eradicate the problem of diabetes in shorter period of
time.

is an essential approach which is taken in health and social care. Because communication failure
is both an independent cause of preventable patient harm and this make cross cutting
contributing factor which is occur in other harm scenario. As per this, the transition of care
which is arise between care areas or shift changes in their acute care setting which provide
leading opportunities for communication and interaction failure which is directly create threat in

critical information about the patient status and plan of care which is usually miscommunicated,
leading with proper allocation and using appropriate treatment may delay due to improper
communication in terms of inappropriate therapies. In addition to this, there are various
transition which is linked with approx. 30 percent of surgical adverse events which is informally
drive and respondent. Moreover, the team member who care patient is usually interact which
prevent the clinical harm.
The team work and communication usually challenge number of times in order to manifest
the issue which is arise due to the coordination generally neglected by organisation. In a
healthcare sector there are various task which is performed at a same time. There are number of
medical staff and member who is taking several responsibilities in order to provide the quality of
health and quality of healthcare. However, due to various loop and mechanism the coordination
and integration is totally disturb in aspect to conduct the quality of care. High level of
interdependence can create the framework of new core of practice in workplace to manage the
team and appoint someone to coordinate with various department for proper caring and
nourishing of patient for their issue. So, as per this it helps to provide a communication to get
interact with patient and this help to know the complication about patient, as per this
coordination is also important in order to make a collaborative behaviour with number of patient
and with service provider to provide effective and efficient quality of care. Collaboration in
health and social is refers as the health care professional which help to assuming complementary
roles and cooperatively working together for a same goal to provide quality of health to patient in
their respected workplace (Professional Communication and Team Collaboration, 2020). This
all depend on the sharing responsibility for problem and also an approach for the effective
decision making. This carry out plan and strategies for patient care.
