1. Community diagnosis
As a health care staff collaborating with a public health nurse, the aim was to understand
the various psychosocial factors that affect the metal health of the African American women
living a community life in the country of United States (Jackson, James & Owens 2017).
After the needs assessment that was performed, the various factors pertaining to the lack of
awareness and education, lack of hygienic environment and negative perception about the
world, lack of economic and financial support, various sociocultural traditions has together
led to the causation of increased stress and anxiety in the African American women that is
again directly proportional to the intake and abuse of alcohol and other drugs in a
psychosocial context (Randle, Spurlock & Kelley 2019). The deterioration of mental health in
the African American women, living a community life, is determined by the distorted social
determinants of health and psychosocial addiction due to the fast shifting self-identity and
self-concept has played a very important role in the increase of psychosocial addiction in the
community living African American women (Tanner, 2018).
Hence, the community diagnosis is : ‘High risk of mental illness amongst the community
living African American women related to the low income, depression, emotional
derangements, low quality of life and psychosocial addiction’.
2. Solution to address the targeted need
There are certain interventions that can be undertaken along with the collaboration with
the public nurse and it is important to address the distorted social determinants of health with
respect to the mental health issues that has been found out in the African American women.

3. Primary, secondary, or tertiary intervention strategy to address the
targeted need
Primary intervention strategy is a through group therapy session can be conducted with
the collaboration of a public nurse and the session would be focused at empowering the
knowledge of the community living, social deprived women, regarding the ill effects of
alcohol and other drugs as well as the important mental well-being exercises will be taught to
the African American women so that they can learn to be more mindful and take right
decisions for their families and their community as a whole. Relation techniques and
recreation activities will be promoted among the African American women (Burnett-Zeigler
et al., 2019).
Community mental rehabilitation is the secondary strategy that will be targeted to control
and diminish the level of alcohol and other substance abuse in a pertinent manner amongst
the African American women and also that the seminars as well as the workshops will be
conducted to increase the knowledge and awareness of the community women regarding self-
efficacy, motivational living of life and by solving of the identity as well as identity crisis.
The tertiary level intervention will be to solve the employment issues of the community
by helping them build a self-sustainable system that support both health care, financial and
economy as well as cultural empowerment of target population.
4. Expected outcome
Outcome objective is to minimize the rates of mental illness in the population
Process objective is to develop cultural awareness and competence while addressing the
issues.
Management objective is to develop more collaboration in decision making with the
community women.

5. Evaluation strategies
A Beck’s depression inventory and addictive behavior scale will be used in order to
evaluate the strategies.
6. Identify potential resources
The resources are community leaders, public health nurse and the traditional health
care system already existent.
