Introduction
Main aim of community based rehabilitation is help individuals
with disabilities by providing them physical therapy, equalization
of opportunities and medical integration rehabilitation programs
for disable or the people who are suffering with certain diseases.

Description of the model of care
 Advance Care Planning Model of Care is a process of treatment given
to elderly people in order to understand and share their values, goals
and preferences to meet up with their future needs.
 It suggests the flow of effective communication between patients,
their family and doctors to establish an effective team to treat the
patient efficiently
 This treatment consider the culture and relationship of patient in
order to design a treatment.
 This mode of treatment is very flexible in context of patient, their
needs and the treatment required.

Facilitators and barriers to providing
effective client-centred care
To improve quality of care of Carla it needs to ensure that Carla herself feel
that she can be cured. The very first step of her treatment should include
the right level of motivation and increase her will to live. The facilitator
can be her increasing will to see their first grandchild and which can be
accomplished by ACP as this process design the treatment according to the
personal beliefs of an individual.
The barrier in the case of Carla is her will to live. She feels that she has
become a burden on her family and is responsible of the increasing
arguments.

Conclusion
From above mentioned presentation, it has been concluded that
Advance Care Planning Model of Care model include
particular disease and setting which helps to overcome from
the chronic disease. This model helps to reduce the illness and
also prevent the disease in initial stages.
