Community assessment is an analysis of the community by a researcher or by a group of
individuals. The goals of the assessment are to help the researcher to be familiar with the
community while analyzing some of the various aspects of the healthcare of the population. The
assessment also helps the researcher to identify the possible weaknesses, strengths, risk and gaps
that the community has. Without forgetting, though this assessment, the research can establish
professional interventions, especially if it is a group of researchers working collaboratively, and
in turn contribute to the empowerment of the community and bring a change to the concerned
population. community assessments are usually conducted by healthcare practitioners such as
nurses. The researcher can obtain the required information to plan for the resources available to
the community. In this research paper, the community assessment of Sentinel city will be done,
possible problems faced by the community will be identified and a recommendation the best
possible strategies to help solve the problems will be conducted (Johnson et al., 2018).
Demographics assessment
The demographics assessment was used to analyze some of the risks that are faced by the
community in Sentinel. Using the demographics assessment, various aspects of the population
are studies and analyzed. The population, ethnicity, level of education, occupation, living

Sentinel city is composed of four distinct neighborhoods; Nightingale, Caper Park District, Acer
Tech Center and Industrial Heights. The total population of Sentinel city is 663,862. In industrial
Heights the population is approximately s 28,855 people. The highest percentage of individuals
in this population are the Latinos who take 46.8% of the population, African Americans are
13.1% of the populations while other ethnicity account for the remaining part of the population.
in this neighborhood, 10.9% is made up of people below 5 years, 20.8% of the population is
made up of individuals who are below the age of 18 while 0.9% of the population are individuals
who are above the age of 65. According to the assessment, the median house income is $24,672.
12.9% are individuals who are living with disabilities et hey are below the age of 65. 37.5% of
the population is composed of individuals below the age of 65 who are living without any health
insurance.
In Nightingales square, the population is 103,974 persons, there are 5326 veterans in the
community and the primary ethnicity are the whites who occupy 81.3% of the total population,
while Hispanics occupy 3.7%. people under the age of 5 years are 2.4%, below 18 years are
5.9% while individuals above 65 years are 13.9%. It is estimated that the median household
income for this population is $269,550. 3.5% of the population is living with disabilities while
0.7% are living without a health insurance.
Acer Tech Center is composed of 168,390 individuals and out of his population 6208 are
veterans. The primary ethnicity in this population are the whites who occupy 70.2% while the
Hispanics occupy 13.7%. the population is composed of 4.1% of individuals under the age of 5,
7.0% are individuals under the age of 18 and 19.8% are individuals who are above 5 years. The
median household income for this population is $166,300. The people living with disabilities
occupy 6.7% of the population and 1.5% of the population is living without a health insurance.

Casper Park District is composed of 352,643 individuals and 2016 veterans. The primary
ethnicity in this community are the whites occupy 63% of the total population while the
Hispanics occupy 24%. 10.9% of the population is composed of individuals below the age of 5,
33.8% are individuals below 18 years and 6.5% are individuals above 6 years. The median
household income in this are $ 80,134. 6.8% of the population is living with disabilities and
22.7% are living without a health insurance.
Neighborhood/Community Safety Inventory
Th neighborhood and community safety inventory are a tool that is used to assess some of the
physical and psychosocial safety hazards that are found in the community. Using this tool, the
physical environment, social dimension of health and the psychological aspect of sentinel city
were analyzed. The neighborhood/community safety inventory helps to identify the possible
safety problems that are in the neighborhood and helps in planning of the appropriate
interventions. Data for this inventory was collected by observing the key informants such as
local police, fire personnel, local disaster planning groups, representatives of insurance
companies and transport officials. The key informants are very useful in providing
environmental safety information (Braun, Stepleman, & Swift, 2016).
According to the data obtained from the virtual assessment, the air quality of the city is cleaner in

The architecture of the building ins Sentinel city poses an environmental risk. Some of the older
buildings are at a risk of falling and causing harm, to the people. The older buildings also risk
exposing the community to asbestos and mold which can have serious health implications in the

reports, there has been several arrests in gang related crime incidents, violent crimes, group

intoxication. The reports of the police also show that majority of the victims of the crimes have
been females. There is need to improve the security in the area and reduce the cases of arrest
being made

Windshield survey is an informal survey that is used by health profession where the researcher
drives around the community and records their observations, in order for a community to plan for
its services, it essential to know the health related resources and learn about the gaps in the health
services. A windshield survey is usually done form the inside of a car. The windshield survey is
very effective in observing larger areas and areas in which the aspects required fir the survey can
b observed easily from a vehicle (Thomas, 2017).
According to the windshield survey, there are several industrial factories in the city and most the
industries are emitting fumes into the air which could eb a potential hazard to the community.
Majority of the roads in the area have been paved although there are some portholes. The most
common form of transport is public transport, especially buses and bicycles. There are some
individuals who own cars. The lighting system in the streets and the roads at night is poor and
this may have contributed to the high rate of crime. There is also a park that has trees, grass and
man-made swamps. The park offers a lot of space for the community to rest and relax.
There are several service centers in the Sentinel city. There are health centers and clinics where
the community can get access to daycare facilities. There were also some daycare centers for
younger children there was a school in every neighborhood. Sentinel city also has several
restaurants and grocery stores where the people can have access to food
The population is evenly distributed although some districts have more veterans as compared to
others. The primary population are the whites and the Hispanics, and they are found in varying a
number in the different districts. The median household income influences the type of housing

Majority of the buildings that are in Sentinel city are private enterprises used for residential and
commercial purposes. The most common types of houses are residential houses and majority are
either apartments or multifamily homes. The new structures are in good conditions but there is
some old building with signs put up for demolition.
Population Scavenger Hunt
Population health scavenger hunt are very effective in observing key areas in the community.
Through a population scavenger hunt, areas such as health services, emergency unit, access to
foods, domestic violence centers, fire department and police department can be analyzed. The
researcher has a one on one interaction with the various service providers and a set number of
questions are asked. Population scavenger hints help to give the individual a better understanding
of the community. By visiting the different sectors of the community, the various services are
identified and possible strategies to improve the services are developed (Girgis, & Waller, 2015).
In Sentinel city, one of the areas visited was the local health department. The clinic offered
various services for example cancer screening, infertility services, pregnancy testing and
counselling, contraceptive eservices, services for sexually transmitted diseases, antenatal and
post-natal clinics among other services. in the clinic, the healthcare workers, and the staff were
very kind to the patient
The next area analyzed was the children’s clinic in Sentinel city. It was found that the number of
children receiving vaccination has considerably increased the last few years. There are amore
mothers who are bringing their children to get vaccinated against the common diseases. The data
from this facility, also shows that the number of infant mortality rates has significantly decreased

Analysis of Data Collected
The environmental health requires some changes to help the next generation. The rate of growth
of the industries indicate that there is a risk for air pollution in the area. In addition, appropriate
policies need to be put in place to prevent water pollution from occurring. This will improve the
overall health of the community in Sentinel city (Cooke, 2017).
Epidemiological data shows that there is an increase in the number of children who are being
vaccinated. This shows that mothers are more aware of the benefits of vaccination to their
children and they try to bring their children for vaccination. This in turn has led to a decrease in
the spread of preventable diseases in Sentinel city. In comparison with the rest of America,
Sentinel city has shown a significant decrease in the occurrence of diseases such as tetanus polio,
measles, mumps among other diseases
The application of the epidemiology can be used in the implementation of various policies that
will help make Sentinel city a better place. For example, policies can be implemented to help
curb the high rate of arrest in the areas. This will reduce the number of deaths occurring from
crime. More security personnel can be deployed into the area. In addition, the lighting in the
various roads and streets can be improved. Policies can also be introduced to ensure that all
individuals have a medical insurance. The demographics show that there is a significant number
of members in a population that still do not have a medical insurance.

Problems facing Sentinel city based on the healthy people goals
The healthy people 2020 goals are a project that was developed as a ten-year agenda to help
develop a society in while the people live long healthy lives. There goals were designed to be
implemented in a diverse variety of individuals and organizations. The first goal of the 2020
healthy people was to attain longer lives that are high quality and free from preventable diseases
and premature death. The second overarching goal was to achieve health equity in all age groups.
The healthy people 2020 also had an overreaching goal of creating a physical and social
environment that promotes the good health of all individuals. Finally, the last overarching goal
for the healthy people 2020 was to promote quality of life and healthy behavior across all life e
stages.
The primary problem facing the people of Sentinel city based on the goals is that there is lack of
a physical environment that promote good health. There is evidence of air and water pollution
which could pose as a health risk to the member of the community. The air pollution can result in
health diseases such as pneumonia, bronchitis and lung cancer.
Another problem facing the community at Sentinel City is premature death. Due to the high rate
of crime in the city, the individuals are more likely to die due to murder, homicide, domestic
violence, gang related crime or in the process of a drive by. The police report of Sentinel city
shows that there is an increased rate of crime in the last three years. It is predicted that the
increase rate of crime was triggered by difference in the economic status among the members of
the community. Drug and substance abuse have also played a role in the increase in the rate of
crime. Most of the victims who are female, end up in premature death

groups. In the city, thee are several clinics for mother and child and pediatric clinics, in addition
there are also several daycare services for children, but there are no aged care homes. There are
no specialized treatment services for the elderly patients. More efforts should be put in ensuring
that the population above the age of 65 have access to specialized treatment and care
Community practices to address problems
In order to address the problem of air and water pollution the community should develop
policies. Together with the legislature, the community can pass a bill that prevents industries
form releasing untreated wastes into the water bodies the policy can also be used to control the
air fumes released into the air. All industries should install filters in their exhaust fumes to ensure
that the air released is free from harmful particles that will affect the population (Leiker, &
Whitham, 2016).
Another strategy involves planting more trees. This initiative will help clean up the air and
reduce the level of carbon dioxide in the air produced by the factories, the trees will also provide
oxygen. Being in the presence of nature has known to improve the psychological well-being of
the individual which in turn influence the physical health positively. The trees can be planted in
the parks or in the back yards of residential homes
Primary prevention topic
The primary prevention topic will be ‘Creating Awareness on Pollution.’ Through creating
awareness about the issues, the community becomes empowered about the dangers of industrial
smoke and waste, which will help to prevent the industry from polluting since the community

The simulation has provided hand son experience on the community assessments. The simulation
has helped in gaining research skills such as windshield survey, neighborhood/community
survey, population scavenger hunt and demographics assessment. These skills learned will be
used to conduct a need s assessment in my community. The interventions used, for example in
achieving the healthy people 2020 goals can also be applied in my community (Wilson, Reedy,
& Krebs-Smith, 2015).
