Report
AI Summary
This report, prepared for the C228 Community Health and Population-Focused Nursing course, presents an analysis of health data from Sentinel City. The report begins with an overview of the city's demographics, including population size, age distribution, racial composition, median household income, and poverty levels across four neighborhoods. The core of the report focuses on an epidemiological assessment of the city, identifying key issues such as limited access to health services, concerns related to older adults, environmental quality problems (high noise levels, stray animals, and lack of cleanliness), and high rates of substance abuse and STDs. The report uses Healthy People Indicators to highlight specific findings. The student then discusses potential solutions, such as affordable housing projects and improved health clinics, to address environmental concerns and improve community health outcomes. The report concludes by outlining the assessment strategies used, including visual inspection, document review, and the application of epidemiology and health determinants to analyze the collected data. References to relevant research are also included.

B1.
In the following section the analysis of data collected from tools have been done using
the concept of epidemiology and health determinants
Healthy People Indicators Findings
Access to Health Services Heath services are easily accessible every local
districts

2COMMUNITY HEALTH AND POPULATION-FOCUSED NURSING
C.1
From the different epidemiological assessment
Environmental concerns such as high noise level due to stray animal, lack of
cleanliness due to large number of homeless population are major issues
STDs in 15to 25 years old age group are frequent where teen pregnancy rate has been
increased
Due to tobacco, alcohol, marijuana, heroin drug use ventricular fibrillation and upper
respiratory problems are high
C2. Affordable housing project and better health clinic can address the issues of
environmental measures such as cleanliness, noise level, stray animal and homeless
population. Housing project can cooperatively subsidize for some of their units to make them
available free of cost for homeless population (O'Mara-Eves et al., 2013). For this
collaboration with various charitable organization and funds to invest in new housing plots.
Affordable housing project and better health clinic can also cooperatively work together to
arrange different environmental awareness programs where the citizens can participate in
planting trees (Yuen et al., 2015). Planting trees will reduce the noise level and increase the
quality of the air as well as cleanliness.
C3.
From the above discussion the chosen issues are high associated with environmental
concerns. Therefore the chosen topic is: Improving Environmental Quality and Physical
Environment

D.
The assessment strategies that I have used for the assessment of the simulated city are
mainly associated with visual inspection or observation and evidence based data collection
from documents (Udofia, Noble & Poelzer, 2015). Therefore, I can also use visual inspection
and reviewing documents for assessing my community. Furthermore, I can also use the
epidemiology and health determents for analyzing the collected data.
