a) Refer to The Academy’s procedures for evacuation and explain where you can find the

Evacuation process can be located on every evacuation route which can locate visible. There
is orientation for showing direction to exit. Generally displayed on internal side of door or on
walls. There are some procedure need to be followed for evacuation. This procedures are

It is important to get ensure about the good understanding of workplace emergency
procedure because it provide help during emergency to keep yourself and other people safe
whenever there is any type of accident happens. When there is any type of accident, then it is
needed to be reported to supervisor and other senior authority who are assigned and having
responsibility.

1.5 Explain who would be the designated person Kris would report his concern to about safety in
the workplace, how would he report it and why?
Kris should report to chief executive officer because chief executive officer have the
responsibility to get ensure that any matter related with volunteer welfare should be appropriate
and reported effectively. Kris need to report it because it is concerned with safety at workplace.
This should not be ignored because it is construction site and any negligence of safety parameter
can invite to any accident. So, there should not be any type of negligence related with safety
measures. It should be reported immediately
ASSESSMENT TASK 2
2.1 Refer to image below and in small groups discuss the following and document your answers
in your own words.
a) Identify 10 potential hazards in the workplace (image)
In this image, the first potential hazards which are identified in the image of bike where the
employee use to cut the word without any equipment and the individual used to stand on the
wires and the liquid flow on the ground. Moreover, it is also identified that the machine indicates
some of the emergency warning before using the machine moreover the employee did not wear
any glasses before using that particular machine to cut the wood. This creates various risk
assessment for that particular employee. There is a petroleum or the explosive are used to
available on the ground and employ smoke in the no smoking area which also indication of
hazards. The hard hat area has some of the equipment’s are used to hank and the employee did
not focus on this and what is rate this is also create major injury to the employees. In this image,
employee used to way some of the tanks which is heavy and do not use any weightlifter.
Moreover, it is also creating a hazardous situation for the employee when the tank or equipment
which is may be acid or alkali in nature can harm the individual and create the hazards. The gate
of fire exit is blocked by various instruments of the product within the door so in the case of fire
the employees do not exit due to such kind of blockage. However, the employee is also working
at area which is prohibited. As per this, the floor is full of fluid and the open wire is their which
can create the flow of electric electricity and can create the complications of electric shock to the
number of employees. In this case, the health and safety and no need to be understand moreover

in this image it is also well identified that the notice board of health and safety is not appear as
well as because of the photo of bike image. In this, the regulations and notifications are used to
be disappear due to the such kind of photo. The employees are not responsible and do not follow
any code of conduct as mentioned by health and safety rules. They violate every rule in the
workplace. As per this, they fall of any circumstances the employees are face various critical
issues which can cause various complications and maybe situation associated with the life-
threatening issues. So, as per this, the supervisor must use to take an inspection of the employees
that do follow the work healthy safety instruction or not.
b) Who would you report the hazards to?
The hazards which are shown in the image are most reported to the supervisor or the
directors who used to conduct are these activities for the task to the employees in which they
must educate and punish for that. Moreover, it is also analysed that this type of hazards can
create complex abilities for the organisation as well as employee. Moreover, there are various
situation and consequences arises due to the poor handling of equipment’s and product which
may be acidic in nature can create the life-threatening issues for the employees. In this, case the
supervisor and directors must take the responsibilities for that. In this, type of report of the

2.2 Read the scenario below and complete the activity
You are now required to come up with work instructions and procedures which the
apprentices must follow. This could either be simple instructions and diagrams which can put
displayed and be visual for the apprentices to see. You could also develop a sign. (Guide: 30 -50
words).
It is very necessary and adequate manner to perform any task other work in any workplace.
Then, the instruction is must be followed before starting the working procedure. Moreover, there
are various signs and diagrams are used to indicate various complex abilities which can be arises
during the working. So, it must be displayed to every employee to develop productive and
healthy environment for working. In this, I used to develop a sign which is mentioned healthy
and taking the attention toward the work safety and use proper personal equipment before doing
any task which is related with the machines. Because, any type of hindrance can cause various
consequences which is related with the life-threatening issues.

A: Explain what is the reporting procedure if you identify an injury?
Any injury to any staff for the employee aur any visitors should be recorded within
VHIMS by the manager of the reporting staff member to acknowledge the consequences and
complications. If a person has been enjoyed then the first acquired with the first aids and after
that the necessary action should be taken in the appropriate way to handle the injury.
B: Who would you report the injury to as per The Academy’s Policy and Procedure?
If any hazards are determined during day-to-day activities which must be reported to the
appropriate manager who is responsible for the safety and health of the employees. In this, the

hazards can be removed it immediately then the manager should take necessary action with the
proper consultation with the health and safety representative to eradicate the hazards which can
create complication and consequences with the number of employees.

2.4 Everyone has a role to play to ensure that health and safety is a priority in the workplace. The
Work Health and Safety Act 2011 (WHS Act) and Work Health and Safety Regulations
2011 require persons who have a duty to ensure health and safety to ‘manage risks’ by
eliminating health and safety risks so far as is reasonably practicable, and if it is not
reasonably practicable to do so, to minimise those risks so far as is reasonably practicable.

3.1 Case Study - Discussion Read and discuss the case study with your

A: Explain how Kris can contribute the WHS meetings and what items can he bring to the
meeting? (Guide: 30 words)
Kris can contribute to the meeting by demonstrating some safety work and provided some
information and guidance to act during any type of hazard or accident. There is need to take
some items which are related with personal protective equipment to bring the meeting to
demonstrate effectiveness a personal protective equipment.
B: During Kris' employment, he would come across jobs that had the potential to cause his team
harm. What consultative activates or steps should Kris take to minimise these. Who should
Kris report these issues to, how would he report the issues and why? (Guide: 40 to 50
words)
Kris should provide information to his team member related with safety at the workplace
that can help Kris to minimize any harm to his team. Chris need to report to his supervisor or
senior officer related with these issues because higher authorities should have the information
about any potential cause of our to any employee.

Scenario 1 There is no need to ask the reason for leaving pile of boxes to that team
member who has left them and need to ensure that provide information with related concern and
should not make such type of mistake again. There is also need to inform every worker to make
sure after finishing of swift that there should not be left any equipment or a pile of boxes in
wrong place.
Scenario 2 In such type of situation, there is need to take quick action and inform
employees about the condition of cattle. This should not be used but replaced. This can help to
avoid the hazard and reduce the risk of getting any type of medical condition or accident.

3.4 Practical task
A: Review the above YouTube Video and discuss what steps you would take to use a fire
extinguisher if there was a fire in the kitchen.
Pull- There is need to pull the pin. This will break the the temper seal.
Aim- Aim point the nozzle to the fire base. There is no need to touch the on CO2 extinguisher
because it can get very cold and can damage the skin.
Squeeze- Squeeze the handle for releasing extinguishing agent.
Sweep- Sweep from side to side at the base of fire or any fuel sources until fire get stop.
B: What steps should you take if you had a grease fire on the stove?
Covering the flame with cookie sheet for metal lid and living cover until get cold turning
off heat source. Pour baking soda or salt to smooth fire spray. Use dry chemical or fire
extinguisher. Don't extinguish fire with water. Don't attempt to move pan or pot outside.
C: What other factors should you take into consideration when cooking in the kitchen.
Children should not play near fire. Need to take all the protective measure for safety

3.6 Explain the difference between hazards and risks in the table below
Hazards Hazard refers to a situation or resource with potential to harm to any human or
create injury or ill health, any damage to environment, damage to property or

combination of these. This is an unwanted event or situation when there is any type
of loose of control and leads to harm.
Risks Risk refers to the possibility of of something bad happening. It involves uncertainty
related with different implication or effect regarding any activity which related with
humans value like wealth, health, well-being, property or environment and leads to
