Subject: Letter regarding the breach in health report
Thank you for choosing Richmond Road Diagnostic and Treatment Centre and allowing us to
serve you.
We are writing this letter to sincerely apologise on behalf of the entire medical team of Richmond
Road Diagnostic and Treatment Centre. We want to inform you that, unfortunately, there has been a
breach in your report, as your health records have been inappropriately accessed by someone else.
On Friday, we came to know from Alberta Health Services that one of their staff had the health
records of 178 patients, including your record too. There has also been a breach of reports in the AHS’s
Calgary Zone Clinibase system and the Alberta Netcare records system. The breach happened from
February 2019 to July 2019. The staffs at the Richmond Road centre, including treatment, diagnostic and
care information might have accessed the data. Fortunately, there was no effect on patient care and data
accuracy. This matter is taken seriously and is reported to Alberta Health and the Office of the
Information and Privacy Commissioner. We hope to find a quick resolution to the problem, and we hope
to keep you updated on the same matter too. Todd Gilchrist, the AHS’s vice-president people, legal and
privacy has expressed his disappointment and would take action against this to protect the health
information of patients. People found guilty or negligent or both would be dismissed.
We assure you that this kind of incident will never happen again. We will take necessary steps
and reach a conclusion as soon as possible. Please continue to have faith in us and be supportive as
always.
Thanking you
Yours truly,
Dr. Stansky
