INTRODUCTION
This report is usually following the case study of Mr. Martin who has been taking the
discharge from the hospital with the diagnosis of early dementia. Moreover, he is facing issues
with hypercholesterolemia and hypertension for a longer period. As per this, it is also identified
that they have two sons and two daughters name Jason, Kenneth, Mandy, and chu. As per this, it
is also identified that his wife's death is happen in 2005(Bally and et. al., 2020).
This report usually covers the complications and consequences which is faced by Mr. Martin
in their daily activities which occur due to the short activities of forgetfulness and after diagnosis
it is identified as early dementia. Moreover, various other factors are based on a secondary basis
such as he is also dealing with the complications of hypertension and hypercholesterolemia.
They have been taken medication and drugs for a longer period to reduce the implications which
are associated with such kind of disease. This report usually covers some of the areas which
specify the complex needs and the role of a registered nurse and other people who are associated
with Mr. Martin. In this, the use of drugs and also the discussion followed the critical condition
of Mr. Martin with the proper medication and drugs with the significant role of people in their
life. As per this, the suggested complex needs and their requirement are seen to reduce or to
minimize the complication and consequences which can create issues with the patient (Clarke

With the case study of Mr. Martin, it is required some of the aspect which is based on
wound care. Moreover, Mr. Martin face issue with hypertension and early dementia which

As per this, Mr. Martin lives with his youngest daughter Chu and husband Eric in their
home in the metropolitan city. The Chu is a pharmacist and Eric is a social worker. As per this,
the plan or the care is needed for Mr. Martin that is based on medication management.

In this, there are some of medication is followed in order to maintain the regularity of
medicine and this is best for the Mr. Martin to develop quality of health which is occur due the
proper drug monitoring and without any medication error. As per this, with proper monitoring
and care the unnecessary behavior which is forgot by Mr. Martin will be understanding and can
be remembered by analyzing that activity with the help of family members. In the unknown
place, it is very complex that all the services and care is provided while taking the aspect of

which is not ideal in the situation of mental illness which include dementia which is diagnosed in
the case of Mr. Martin. Furthermore, recommending appropriate mobility decreases the fall and
hospitalization significantly elderly living in a community (Garnett and et. al., 2018).
Role of medication with help of nursing:
It is well analyzed that Mr. Martin is living with her daughter chu and husband Eric.
Moreover, she is a pharmacist and her husband is a social worker. So, they don't have to provide

condition of dementia. The forgetful behaviour of patient show that he did not recognise their
activity which is perform on the daily basis such as bathing, dressing and other. So, the proper
support is required in order to maintain the daily activity which can create issue with Mr. Martin.
In this, some of the other aspect is provide nursing care or the family support to maintain the
daily activities.
Spiritual care
Mr. Martin is believing in the Buddhist spirituality, so, some of authentic relevance which is
associated with them that they go for the spiritual care such as using herbal treatment and do
proper mediation which can also trigger the body to show positive response. As per this, there
are number of cases which show that sometime the medication is not as effective with
comparison to spiritual care. Moreover, it is also analysing that he believe in Buddhist and he has
a level spiritual so the care is helpful for him.
