Program Documentation Template
Use this template to document the service program you developed.
1. Define the purpose and features of your program:
1. Summarise the client need/s.
2. Describe how you identified this need (e.g. engaging consumers, through
other services /networks, via research).
3. Describe the services offered by your program – what type of services,
how often, to whom and where.
The indigenous population of Australia is comprised by Aboriginal and Torres strait
islander people. the client group is an aboriginal community with 50 males and thirty
seven females who has a strong prevalence of alcohol and other substance disorders.
Health needs – both physical and mental health needs. Lack of primary care
networks, lack of accessibility to a quality health care.
Health literacy and knowledge needs – although the community has a strong kinship
but the lack of knowledge about the deteriorating physical health condition and mental
condition, is aggravating the condition of the community further in the negative way
and manner.
Financial needs – lack jobs and employments and the families are in a low
socioeconomic status.
Nutritional needs – has led to diabetes, hypertension and malnutrition issues in the
clients

physical, mental and social health assessment was taken and performed which led to
identification of the needs of the community living people who are the client group of
this program.

a. Health literacy service – through a English language development plan and couple
of seminars undertaking that would empower the knowledge and awareness of the
clients about the prevention and management of the physical health and mental
health conditions.
b. Alcohol and other drugs counselling – the counsellors working in the framework of
the program would be applying their skills and expertise at helping the aboriginal
clients withdraw from alcohol and other substance intake graduate and align
themselves with the leading of a healthy life

dieticians in the program’s framework would teach the client about the health ways of
eating and maintaining sanitation with eating and cooking and washing of the utensils.

2. Define the context of your program (i.e. how it fitted within the organisation
strategic plan, policies and procedures, as well as local, state and national
priorities).
The financial, physical and human aspects of the program that targets at improving
health and well-being of the community clients with nutritional deficiency and alcohol
and other substance abuse disorders – was strategized with the right workforce, the
right skilled workforce that has a mix and collaboration of nutritionists, dieticians,
alcohol and other drugs counsellors, the general practioners and the community

rather the educators who would be teaching the clients about the English language
that would again promote their language and health management skills. The plan of
the organisation to promote language and health of the community ;living people and
the practioners as well as the supportive staffs, as named and mentioned above
would be taught and trained with cultural competence in order to deliver the person
centred and the community centred service in a more pertinent manner. The policies
are that which guides the employees or rather the staffs of the organisation to comply
with the best practice or the evidence based practice guidelines and the local and
state and the national priorities of empowering the rights, livings and culture plus
health of the aboriginal people would be addressed through the action and application
of evidence based practice in assessment taking and intervention delivery to the
community living people of aboriginals group.
3. Describe how you engaged consumers and other services/networks in the
planning process:
1. State who you consulted with and how (i.e. methods you used to
ensure consumer participation in your planning process).
2. Give at least 2 examples of barriers to participation and explain how you
overcame these.
To ensure consumer participation in your planning process, the local activists, the
community workers and the community leaders were involved in the decision making

2 examples of barriers to participation of the consumers to this health and language
empowerment program were :
a.Lack of collaboration of the interdisciplinary team members in the delivery of the
community centred health and language improvement processes. Lack of interpersonal

4. Outline what plans you made to continue to engage consumers in service
delivery (i.e. how you ensured on-going consumer participation and consultation
in service delivery).
In order to ensure on-going consumer participation and consultation in service delivery
–I structured and restructured the important facets of the service delivery program, by
taking continuous feedbacks from the clients in the community and from the members
that is the staffs involved directly with the health and language promotion program
delivery. I continuously involved the community workers, the workers of the non
governmental organisations working for the best interests of the community living
aboriginal people, the community leaders, the community educators in the strategic
planning and decision making of the health and language improvement program. I took
interviews of the community leaders and the other members of the community in a
culturally competent manner in order to ensure more participation and active
participation through motivation of the clients. Giving away prizes and free services with
educational empowerment of the children in the community living area is a great way of
maintain the engagement of the rate and participation of the consumers in the Health
and English language improvement program.
5. Describe actions you undertook to get everyone on board (i.e. engage
internal and external stakeholders).
The internal stakeholders were dieticians, nutritionists, the general practioners, alcohol
and other drugs counsellors, physical therapists, community psychologists, the
community health nurses who will be looking after the health and social needs of the
community and the interpreters, the teachers (educators) who are teaching the clients
about the English language that would again promote their language and health
management skills.
The external stakeholders were the community leaders, the holistic healers, the local
activists.
The actions that were taken to get everyone on board were public speeches,
welcoming programs, client engagement activities and educational opportunities for
children and a space/opportunity for collaboration of the holistic or traditional healers
and the local community workers with the professional members of the service delivery
team. The incentive opportunity for the staffs was helpful.
6. Describe the procedures and systems you set up in order to facilitate the
running of the service on a day to day basis.
The procedures and systems that were set up in order to facilitate the running of the
service on a day to day basis were auditing, supervision, electronic program
engagement assessments, documentation and feedback analysis.
