INTRODUCTION
Meeting the needs of service user can be defined as providing the helping hand to the
patient so that it can easily recover from the health issue in minimum time possible. Every
hospital and social workers, ensure that they provide quality service and resources to the person.
So in order to maintain this plan, they have to make an appropriate care plan which is an
organized aspect of the condition of patient. The present report is based on the case study of Ola
Adeyemi's, suffering from Down syndrome thus care plan is being prepared to decided
appropriate support needs to provide to patient so that it can achieve its goals. Further, the study
will discuss on the models and theories used in order to take care of the patient. Lastly, the report
will cover reflection pertaining to my experience while delivering services to patient through
making use of Gibbs reflective model for devising and supporting services.
MAIN BODY
Describing the care plan of patient
A care plan can be stated as a management plan of the patient i., e made by the doctors
and nurse in relation to the chronic condition of the individuals. In this program, the doctor will
set the goals of the patient so that it can improve the overall heath. The aim of this plan is to
communicate the issues to the sick person. Moreover, the overall care plan of Ola, include the
details of disease it is suffering from and the medication that will be provided to it in order to
achieve the aims (Tuinman and et.al, 2017).
1. Client Assessment: It was found in the assessment that patient named as Ola Adeyemi
that is thirty-year-old man suffering from the Down's syndrome and live with its parent in
Wembley. Furthermore, until the age of 18, Ola has not learned method pertaining to
cooking, the way to utilize public transport and facing change in dressing thus patient is
not self-sufficient to manage the regular lifestyle stuff (Johnson and et.al., 2018).
Moreover, the patient is suffering from heart condition that lead in problem of breathing
so Ola cannot be left alone as, it requires someone to look after itself.
On the other hand, it has a symptoms of dementia as a results Ola is not able to grasp the
information for long time thereby the patient cannot be independent as it faces hard time in
remembering anything. Furthermore, the emotional condition of Ola's parent is also not good and
they are also struggling to support their child. Along with this, the patient has few symptoms that
were acute such as infection of annual flu and thyroid problem that was chronic as it affected Ola

physical and mental reaction. Thus, negatively affected the overall development of the body
(Aronoff-Spencer and et.al., 2020).
2. Patient outcome: After the patient assessment, the next step was to set appropriate
outcome that wants to achieve by providing proper treatment to the patient. Likewise, Ola
is suffering from heart disease, so the desired result might achieve, if the proper
medication and monthly check-up is provided with a motive to reduce the pain and
suffering. Along with this, patient is also suffering from dementia condition that can be
overcome by providing proper therapy, mind stimulating activities and proper medication
as this will help suffer from to slow down the process of dementia.
Furthermore, the another goal that Ola have to achieve is to be self independently it can
be only accomplishing if proper healthy routine be followed by patience such as proper diet and
health check will allow Ola to know the things it has to work on more for good health ahead.
Along with this, the another goal that have to be achieved by Ola is to control the thyroid, that
can be possible by doing proper exercise, increasing the intake of Vitamin-D, iron, zinc and
copper beside this it has eaten gluten free food. Therefore, by following this, Ola will be
overcome the various diseases to have a better lifestyle and health condition (da Costa Ferreira
and et.al., 2021).
3. Nursing intervention: As Ola is suffering from the down syndrome and its various
symptoms it needs to be provided with the various treatment to cure patient. Such as:
4. Through using physical therapy can be defined as the treatment that improve the muscle
strength and improve the coordination of the body. As Ola is suffering from poor muscle
disorder so this therapy will improve the coordination of muscle movement and over all
posture of patient. Along with this, it can also help Ola in strengthening the muscle (What
is a Nursing Intervention? 2021).
Moreover, occupational therapy is another intervention that should be provided to Ola as
it improves or developed the skills require in daily lifestyle, at the same time it helps in
developing the healthy lifestyle for patient. Therefore, it will help Ola in performing daily
lifestyle task such as eating, dressing up, using the public transport in order to live
lifestyles as per other individuals.
Furthermore, the third intervention that could be used by the nurse is adopt the assistive
technology as it will help in enhancing the health of Ola. This therapy will cure the motor

and functional skills of the individual so that it can use the cell phone, computer and thus
result in Ola to be independent and get the job.
Moreover, the patient will be given prescription related to the drugs and supplements
such as antioxidants as it is safe and effective for controlling the dementia. Ola will also
be provided with the emotional and behavioural therapies as it calms down the patient
and guide them to manage the day to day challenges and reach their full potential (Abd
Elalem and et.al., 2018).
5. Rational intervention: As there are range of treatment mentioned in nursing intervention
in this stage the importance of therapies used by the Ola will be stated. There are varieties
of therapy that can be used on the patient so to improve the overall development,
productivity and independence.
Ola is not able to do the basic lifestyle work it will be provided with occupational
therapy (Reitz and et.al., 2020) as it helps patient to adjust in the everyday life style
tasks and improve the abilities of the suffer. Moreover, this therapy teaches the patient
about the self-care skills for e.g., dressing, writing and eating. This treatment will help
Ola in enhancing the skills that will match their interest and in finding the appropriate
jobs. Thus, it will help Ola for being self-sufficient and independent that will result in
achieving it goals.
However, patient is also facing the problems related to heart it will be treated by
controlling blood pressure, maintaining healthy lifestyle and by managing stress.
Therefore, this remedy will help Ola in promoting efficient blood flow that will lower the
chances of heart attack and moderate the risk of heart infection. Hence, it will help
patient in coping up with the physical and emotional stress.
Further, the physical therapy (Kinney and et.al., 2020) will be very effective in curing
the down syndrome as it strengthens the weak muscle and improve the foundation skills
thus, overall develops the health of the Ola and play an important role in patient life.
6. Evaluation: After the rational assessment, the next step is to check the outcome of the
therapies patient is using such as it has been evaluated that occupational therapy will
make Ola independent as the main aim of this treatment is to improve specific self-care
skills i.e. is dressing, eating and competing hygiene task. This cure allows the patient to
do the necessary task. This therapy not only improve the daily life skills but also the

functional skills of Ola that will contribute in making the patient self- supporting. The
outcome of this treatment will help Ola to earn and live the life happily.
7. However, the outcome of physical therapy is also excellent such as it has strengthened
the muscle that will promote the ability to move along with this it will reduce the muscle
pain of Ola. Further, it will also improve the physical fitness of patient and enhance the
quality of life that will allow the patient to do the things without anyone else support.
Moreover, it will prevent the early symptoms of disorder and impairments that will result
in reducing the injuries and other diseases. This treatment will motivate Ola in living it
life by own and reduce the stress from the mind of it parents. Thus, both these therapies
will allow Ola to be financial and physically independent.
Skills that are used to treat the patient
As health care assistant, I have used my various skills so that I can help Ola in recovering
from the health problem. Along with this, all these skills help me in copping up with Ola such
as:
Communication skills: I made sure that there is a transparency between me and patient
so that he can easily communicate his feeling and issues facing by it during the treatment
process. The effective communication helped me in understanding the health problems as
well as I make various nursing intervention to improve the overall health of Ola (Cohen
and et.al., 2021).
Time management: I have focus on managing the nursing intervention such as
administrative, educational and other task to be completed in limited time. So that I can
quickly recover my patient health by using therapy such as physical and occupational and
complete the activities in efficient manner. Thus, this skill helps me in effectively
organizing therapy and prioritize patient care.
Positive attitude: Positive attitude motivates Ola to stay positive, clam for its better
health and well- being. At the same times it has helped me to stay calm and stress free
while performing the task. Moreover, it increases my consistency while taking care of
patient that lead in better outcome.
Strong work ethics: I follow or abide to all work ethics such as devoted towards patient
and treating him with due care and keeping him as a priority to provide appropriate care.
This result in building the good relation with Ola.

Empathy: I tried understand my patient weakness and also challenges faced by him so
that I can make appropriate plan to improve the health. Thus, work effectively.
Flexibility: The health care system is changing at fast speed so I have tried to adapt
myself in the changing dynamic of healthcare. That contribute in great quality services to
the patient as well as efficient utilization of resources (Kim, 2020).
Model and framework used while preparing the care plan
The care plans models provide the specific information regarding the needs of the patient
and the treatment that will be effective for them so that they can recover from there disease
quickly. As the health care assistant I have used two theories such as Orem’s and Roper, Logan
and Tierney’s model to support Ola or make him self- sufficient so that it is able to perform the
numerous task in life.
The first theory that I have used on Ola was the Orem’s principle that can be defined as
an act of assisting the individual so that it can manage health and well -being by practising self-
care. Further, I have followed the three stages of this theory that was very helpful for me to
understand my patient requirements (Noohi and Abbaszadeh, 2020).
Self-care: The first stage self-care can be used to determine the performance of Ola while
it is practising various physical activities. These action will maintain the style and
function of the patient and can contribute in development of the healthy condition that
makes Ola independent.
Self-care deficit: In the second stage, I have provided him due care as most of the time
patient require help because Ola was not able to do the basic task alone. Thus, I helped
him in learning various activities such as dressing, bathing, eating. I have also used the
music therapy in order to reduce the stress of patient to allow him to understand things
clearly.
Nursing system: The final stage is about nursing system, it basically means the relation
between the patient and the health care assistant. I have tried to build the good relation
with Ola so that I can understand his behaviour and action of the patient and try to build
new intervention. So, that I can provide him with quality care and help him being
independently quickly. Lastly, I have also used educative system as it will Ola to grasp
information easily (Saeedifar and et.al., 2018).

The second model used by me was the Roper-Logan-Tierney Model it is a theory based
on the 12 essentials that are required for healthy human being. In this model the patient is
examined on the bases of these 12 activities to check whether the patient is able to perform the
basic life function independently or not (Williams, 2017). The twelve activities of Ola that I have
noted was:
Maintaining safe environment: As a healthcare I have ensured that safe and secure
environment is provided to patient so that it can easily overcome from the health problem
and live better lifestyle.
Communication: I have effective communicated with the patient so that I can
understand his feelings and solve the issues he is facing regarding the health.
Breathing: Ola already suffer from the breathing issues, so I had make sure that it does
regular exercise in order improve the breathing issues.
Eating food and drinking fluids: I made sure that Ola is in-taking proper diet that will
helps in maintaining healthy lifestyle and reducing problems related to stomach.
Eliminating body wastes: I have added fibre intake more in the diet of Ola so to
eliminate body waste and improve the digestive system.
Personal cleansing and dressing: I made sure to treat Ola with the occupational therapy
as it will help him to do daily life task efficiently and be more independent.
Controlling body temperature: As healthcare my first task is to check body temperature
of Ola so that I can know that patient is not suffering from fever.
Mobilizing: I have treated Ola with physical therapy so to improve the body movement
so thereby it can easily perform the exercise and live independently (Fitzpatrick and
et.al., 2020).
Working and playing: I make sure that Ola involves in various physical activities in
order to maintain body weight.
Sleeping: I have also ensured that Ola is taking proper sleep of 8 hours so that his body
can take proper rest.
Gibbs Reflective cycle in relation with implementing care meet:
This model allows individual to work through experience or enable the person to reflect the
incident and occurrence. Moreover, reflection means the analytical approach of the writer
describing an event or imaginary situation. Furthermore, by using this model I would like to

describe about the things I have experience and learn while making the care plan of Ola.
Through this model I will also describe the feelings I came across during working with the
patient (Gibbs reflective cycle.,2021).
Description: It is an event related to one of my previous experience that is of a patient
named Ola. Ola is thirty- year old man suffering from the key issues related to slow
down syndrome. During the assessment of patient, it gets to know that it was not able to
do daily task such as dressing, cooking methods and way of using public transport.
Furthermore, Ola parents want their child to be independent and get a job so to
accomplish the life goals (Adeani and et.al., 2020).
Feelings: During my previous event related to Ola, I came across various feelings such as
while making the care plan of patient I was very nervous because Ola was suffering from
various disease such as heart problem, thyroid and dementia. So it was challenges for me
to prepare a good care plan for benefit of the patient. Along with this, while making the
nursing intervention, I was confused because I was not able to decide that which therapy
will be effective enough for the patient and will help in recovering quickly. Moreover, I
was happy while learning the personnel skills as it was easy for me to grasp.
Evaluation: Throughout my last event of treating Ola I have evaluated that care plan was
very hard for me as it is a very time- consuming process. Moreover, model such as s
Orem’s and Roper, Logan and Tierney’s was not challenging as I already have little
knowledge regarding it.
Analysis: During the last statements I have learned about the theories such as s Orem’s
and Roper, Logan and Tierney’s. Along with this I have analysed that occupational and
physical therapy is very effective in recovering the patient health.
Conclusion: I have learned that an appropriate care plan is required for Ola so that he
can live a healthy life like other. Along with it the experience has helped me in learning
that nursing theories play very important role in developing the motor and functional skill
of patient.
Action plan: I will like to strengthen my personnel skills as they are very helpful in
understanding the patient situation as well is helps me to come up with the stressful
situations. I will also like to work on my time management so that I will be able to make
the care plan quickly (Li and et.al., 2020).

CONCLUSION
From the above report it can be concluded that care plan is very necessary for the person
suffering from down syndrome as the plan contain the details regarding the patient, nursing
intervention that will be very effective in recovering the patient. Furthermore, the study has
helped in understanding models such as Orem’s and Roper, Logan and Tierney’s and their
importance in treating the patient. Likewise, the way uses of model and framework contributed
in lowering down the effect of disease. In addition, to this the report has also depicted the skills
such as communication, time- management used in nursing and their contribution in
understanding the overall health of sick person. Learning from report also portrays, on nursing
intervention such as occupational and physical therapy used in order to make the patient
independent. Lastly, the study will throw light on the Gibbs reflective models that has shown my
experience on making the care and the challenged I have to work on so that I can be able to treat
the patient better in future.
