You are an enrolled nurse working on the rehabilitation ward.
10 days ago Mr Mohammed Salak was admitted to the general ward with Influenza A.
Today he is being transferred to the rehabilitation ward and you are now required to admit Mr Salak to the
rehabilitation ward.
You were told in a phone handover from the nurse on the general ward that Mr Salak’s past medical history
(PMHx) includes weight loss and fatigue and he is 80 years of age.
The problems/issues that you have identified on admission of Mr Salak include:
Unsteadiness on his feet
He has no children in Australia but has good friends in his bowling club who help him out
He is anxious about who will care for his dog while he is in hospital
He is from a CALD (cultural and linguistically different ) background
He has Dysphagia
He has not contacted his children to let them know he is in hospital.
While you are admitting Mr Mohammed Salak, he becomes short of breath.
His respiratory rate has become rapid and his oxygen saturation has decreased to less that is considered
normal.

Her husband works full time. While Jenny is in hospital her husband will need to manage the home and
family. Jenny has two young children at home.
Ms Jenny Oakleigh diabetes has been unstable since her operation. She has been ordered medications for
her high blood glucose levels. She takes the medication but feels nauseated and doesn’t eat her breakfast
The care plan for Jenny incudes daily random BGL’s. You think the nursing care plan needs to change to
include more frequent testing of Jenny’s BGL. At 9 am you check on Jenny. She is sweaty, confused and
shaky.
Later that day Jenny is being visited by her family. Her young child is having a tantrum. Jenny confides in
you that she is worried about her child’s development and asks you for advice regarding this.
